The RN Oncology Infusion Therapy Clinician functions under the direction of the Manager Infusion Therapy or other designated leader. The RN Oncology Infusion Therapy Clinician demonstrates knowledge, competency, and clinical expertise in oncology infusion therapy including but not limited to hazardous drug therapies for treating malignant and nonmalignant conditions.
This RN will regularly administer chemotherapy, immunotherapy, antineoplastics agents, hazardous drugs, and targeted therapies. RN will work in an outpatient infusion center though alternate settings could occur.
This individual is responsible for providing safe patient care with an understanding and awareness of age-appropriate and culturally aware needs. The RN Oncology Infusion Therapy Clinician addresses the patient/family biological, emotional, developmental, psychosocial, and education needs and is responsible for delivery of patient care in complex situations. The RN Oncology Infusion Therapy Clinician serves as a clinical resource to nursing staff, operational leadership, and medical staff.
It is expected that all RNs are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implement the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected that the individual upholds the
voice of the patient, system policies and procedures while supporting service excellence goals.
They are responsible for attending and participating in staff meetings, in-services, special projects and continued education. Performs other duties as necessary. In this role they can serve as a preceptor for newly hired RN colleagues. Local travel required. Float staff will be required to float between Cobb Hospital HOD's based on need. All sites are within 35 miles of Cobb Hospital.

A Brief Overview:
The RN Transitional Care Navigator (Population Health) is responsible for the case management, care coordination management, and utilization management of his/her population of patients across multiple care levels and settings. Serves as a catalyst to promote patients understanding their diagnosis, treatment options, and available resources and ensure that they are connected with the optimal resources across the continuum of care. This role will coordinate and facilitate smooth and safe care transitions while ensuring quality cost-effective patient outcomes. Serves as a liaison between their patient population and all other providers. Will be responsible for key metrics of success, which include improving the overall cost of care, length of stay optimization, reduction in excess days, reduction in SNF utilization and improvement in SNF care transitions, reduction in 30-day readmission rate and ED utilization.
What you will do:
* Guides high-risk patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care. Eliminates barriers to patient's access to health care services and facilitates continuity of care/care coordination.
* Establishes and documents an individualized plan of care for assigned patients using evidence-based treatment guidelines considering the patients individual health goals with a focus on wellness, health management, disease prevention and chronic disease management.
* Partners with the healthcare team to ensure clinical decision-making, implementation of recommendations, and discharge planning are timely and appropriate.
* Performs daily coordination between multiple departments, multi-disciplinary team, medical clinics, and community outreach to gain knowledge of patient, assure patient safety, smooth transitions of care, and manage utilization and total cost of care.
* Acts as advisor/educator by partnering with social work in providing emotional support including goals of care and counseling. Provides and/or arranges clinical education including medication management, community resources, financial resources, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
* Facilitates appointments for appropriate consultations and support services within established protocols
* Completes Utilization Management for assigned patients.
* Applies Milliman Care Guidelines (Indicia) criteria to monitor appropriateness of admissions and continued stays and documents findings based on Department standards.
* Monitors LOS and ancillary resource use on an ongoing basis. Takes actions to achieve continuous improvement in both areas.

This role offers the opportunity to practice within one of the largest and most diverse nephrology patient populations in the Upper Midwest, while working in a highly collaborative, physician-led environment. You will join a cohesive department of three nephrologists and two advanced practice clinicians, supported by experienced clinic and dialysis staff. Our team delivers comprehensive kidney care across the continuum, including chronic kidney disease, acute kidney injury, hypertension, glomerular disorders, and end-stage renal disease. Program Commitment:
β’ Team Members are required to complete: 1 (one) 13-week assignment between Oct 1st - March 31st (surge season) each year, with the option to work additional 8-13 week contracts in between.
β’ While on assignment, Team Members will be scheduled 36 hours/week.
β’ Team Members function under a contingency staffing model as a PRN Team Member (since only held to 1 contract per year), with no guaranteed hours, and no benefits offered.

Job Summary:
The Nurse Care Manager is responsible for providing care coordination including in-home assessment, planning, facilitation, advocacy and authorization of covered plan services to meet the member's health needs while promoting quality cost effective outcomes.
Essential Functions:
- Ensures consistent care along the entire health care continuum by assessing and closely monitoring membersβ needs and status.
- Authorizes covered services and coordinates care regardless of payer.
- Collaborates and communicates with member/family/caregivers, primary care practitioners, and the interdisciplinary team.
- Works with member/family to maintain the most independent living situation possible
- Assesses, plans and provides continuous care management across all venues of care, including hospital, sub-acute, long-term and home settings.

- Performs home visits as required to assess membersβ living situation, cultural influences, functional and cognitive needs.
- Collaborates with the primary care physician and Inter-Disciplinary Team (IDT) to develop the Patient Centered Service Plan for the member.

- Identifies same day grievances, investigates and documents accordingly. Documents any grievance according to plan policy.
- Identifies and presents members with complex care management needs or in difficult to manage situations at Intensive Care management meetings (ICM).
- Responds to membersβ requests in the designated timeframes and completes Initial Adverse Determinations (IAD) as indicated
- Identifies members requiring Care Management Review (CMR), evaluates documentation provided by the IDT including hospital or nursing home discharges planners, and formulates appropriate plan of care.
- Documents care management/coordination according to company policy to the specific plan the member is enrolled in, which may include monthly telephonic and in person recertification notes.
- Develops efficient plans of care, authorizing only needed services at the most appropriate levels, utilizing network providers and ensuring that services are based on membersβ needs.

Education and Experience:
- Associates degree in Nursing from an accredited nursing program required
