Role Overview

The Manager, Care Management provides strategic and operational leadership for a multidisciplinary Care Management Department within a large acute care hospital setting. The role carries full accountability for care coordination, case management, discharge planning, utilisation management, social work services, and pre-admission case management across the department.

This is a senior management position requiring a blend of clinical expertise, leadership capability, and financial acumen. The Manager serves as a key liaison across patients and families, clinical staff, physicians, managed care organisations, community agencies, and skilled nursing facilities, ensuring that care is delivered safely, efficiently, and in alignment with both clinical best practice and the organisation s financial integrity.

Key Responsibilities

  • Provide day-to-day management and operational oversight of the Care Management Department including all staff, workflows, and performance metrics.
  • Lead care coordination, case management, discharge planning, utilisation management, and social work service delivery.
  • Act as the primary liaison between patients, families, department staff, physicians, the broader healthcare team, managed care representatives, community agencies, and skilled nursing facilities.
  • Establish, implement, and monitor clinical standards and best practice protocols to assure quality of patient care and protect the financial integrity of the organisation.
  • Oversee pre-admission case management processes to support appropriate patient placement and resource utilisation.
  • Partner with clinical and administrative leadership to drive continuous quality improvement, regulatory compliance, and departmental performance.
  • Recruit, develop, mentor, and evaluate Care Management staff, fostering a culture of accountability, professional growth, and patient-centred care.
  • Analyse department data and produce management reports to track KPIs, identify trends, and support strategic decision-making.
  • Manage utilisation review and payer interactions to ensure appropriate reimbursement and reduce avoidable days.
  • Maintain current knowledge of relevant legislation, accreditation standards, and payer requirements, ensuring departmental compliance at all times.

Qualifications & Requirements

Education

  • Master s Degree in Social Work (MSW) from an accredited institution required

Licensure

  • Active Washington State Clinical Independent Social Worker (LICSW or equivalent) licence required

Experience

  • Minimum 6 years of progressive Case Management experience in an acute care setting required
  • Minimum 2 years of Managerial or Director-level experience, preferably within a Case Management department in a large hospital required
