Required Qualifications: Current and unrestricted medical license or willing to obtain a medical licenses in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required
Graduate of accredited MD or DO program of accredited universityBoard Certification or Board Eligible in Family Medicine or Internal Medicine (Geriatric Medicine preferred)Active and unrestricted DEA license

A day in the life of a Conviva physician

  • Monday-Friday 8:00am-5:00pm; no weekends, and 8 paid holidays off

  • Join an experienced team with two Advanced Practitioners and a Care Team
  • See12-15 patients per day. Opportunity to spend more time with patients
  • Longer authorized appointments (45 minutes)
  • Acts as a lead and active participant and key source of medical expertise with the Care Team through daily huddles
  • Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Required Qualifications

  • Doctor of Medicine or Osteopathic Medicine

Primary Care Position Details:

  • Role: Primary care physician (18+)
  • Location: Southington, CT
  • Setting: 100% office-based, outpatient primary care
  • Schedule: Full-time, weekdays only
  • Teaching: Optional with faculty appointment
  • Phone Call Only: Averages only 2 call weekends per year and 1 weeknight ~ 2 months
  • Offerings: Guaranteed base, uncapped incentive bonus, loan repayment support , sign-on/stipend, and relocation
