Advanced Practice Provider – Pain Management (Nurse Practitioner OR Physician Assistant)

Location: Multi-site Pain Management Practice (SNF/LTAC + Clinic Coverage)

Position Summary

The Advanced Practice Provider (APP) will deliver high-quality, patient-centered pain management services across skilled nursing facilities (SNFs), long-term acute care hospitals (LTACs), and clinic locations. This role includes routine patient rounding in post-acute settings, providing provider coverage during PTO/leave, and performing specialized “Greenlight” visits to ensure accuracy, compliance, and optimization of patient care plans before procedures or medication coordination.

The APP will collaborate closely with physicians, clinical staff, facility teams, and administrative teams to ensure timely, compassionate, and compliant delivery of pain management services.

Compensation & Benefits

• $150,000-$200,000/year or more! (variable compensation plan based on production.)

Schedule & Work Environment

• Rounding at SNF/LTAC facilities
• Clinic coverage as needed (Pasadena, Clear Lake,and Texas City)
• Local travel between facilities and practice locations as required
• Monday–Friday schedule with 1 weekend (Saturday & Sunday) required for LTAC/SNF rounding a month.

Key Responsibilities

Clinical Care & Rounding

• Conduct comprehensive patient rounding at designated SNF and LTAC facilities
• Perform thorough histories, physical exams, and pain assessments
• Develop and manage pain treatment plans in collaboration with the supervising physician
• Order/interpret diagnostic tests and adjust medications as appropriate
• Evaluate therapy response and document progress
• Provide palliative and chronic pain management services as clinically indicated

Greenlight Visit Responsibilities

• Complete pre-procedure or medication authorization readiness visits
• Ensure charts meet documentation standards for medical necessity
• Verify compliance with opioid and controlled substance regulations (including PMP checks)
• Confirm all required imaging, labs, and prior therapy documentation are in place
• Communicate findings efficiently with providers, authorization teams, and schedulers

Provider Coverage

• Serve as coverage for scheduled PTO, CME leave, and occasional unplanned provider absences
• Ability to work across multiple clinic locations as assigned
• Maintain flexible availability to support business continuity

Care Coordination & Compliance

• Collaborate with SNF/LTAC interdisciplinary teams to optimize pain plans
• Communicate proactively with facility medical staff and families as needed
• Ensure documentation meets CMS, state regulatory, and practice standards
• Participate in quality assurance, compliance reviews, and risk-reduction protocols

Teamwork & Professionalism

Qualifications

• Previous Pain management or rounding (geriatrics) experience

• Active DEA registration (or ability to obtain)
• Board certification (PMHNP, FNP, AGACNP, or PA-C)
• Valid driver’s license and ability to travel to multiple locations

You will:

Position Overview

The Director, Inpatient Advanced Practice Providers (APPs) for Neurosurgery serves as the senior operational and clinical leader for the neurosurgical APP service line at University of New Mexico Hospital, the state’s only Level I Trauma Center and academic medical center. This role is responsible for leading, integrating, and advancing a high-acuity neurosurgical APP program that spans inpatient care, critical care, operative services, and ambulatory practice.

This is a rare opportunity to lead one of the highest-acuity neurosurgical APP teams in the region while maintaining an active clinical footprint. The Director partners directly with the Chair of Neurosurgery, Executive Director of Inpatient APPs, faculty leadership, and health system stakeholders to drive service line strategy, workforce integration, clinical excellence, and cultural transformation.

The service line supports approximately 2,000 neurosurgical cases annually, with ~70 percent trauma, a strong mix of elective and non-elective neurosurgery, and a 24-bed collaborative Neuro ICU. APPs are fully integrated across inpatient, OR, and clinic environments, practicing at the top of their licensure in a team-based academic model.

Key Responsibilities

Clinical Leadership & Integration

• Provide strategic and operational leadership for a 22-member neurosurgical APP team(employed and locum providers)

• Not considering new graduates at this time; The Rapid Response Advanced Practice Provider is responsible for leading and supporting hospital-wide rapid response initiatives aimed at improving early recognition and management of patient deterioration. The APP will provide expert clinical intervention, follow-up care, and collaborate across disciplines to reduce unplanned ICU transfers, prevent in-hospital arrests, and positively impact hospital quality outcomes. This role also emphasizes education, staff development, and interprofessional collaboration to foster a culture of safety and clinical excellence.

Provide ongoing assessment and follow-up of RRT patients, ensuring continuity of care.*Round on units with prior RRT activations and on all post-ICU transfer patients within designated time frames.*Initiate and monitor sepsis and hypoglycemia protocols via EPIC dashboards.*Support neurology during Code Stroke activations and collaborate with hospitalist teams as needed.*Serve as a resource for clinical deterioration management, providing bedside teaching and debriefs.*Document all RRT and code interventions accurately and in a timely manner.*Contribute to hospital quality initiatives by monitoring data trends, improving processes, and promoting operational efficiency.*Participate in professional development, staff training, and mentorship activities.

• Master's or Doctoral degree in Nursing (APRN) or Physician Assistant Studies.
• Current licensure as an APRN/PA in the state of practice.
• Certification in Acute Care (ACNPC-AG, AGACNP-BC, or equivalent).
• Minimum 2–3 years of acute care/critical care experience required.
• Position responsible for providing comprehensive patient care by systemic assessment, diagnosis, treatment, management, and in-hospital follow up, affecting direct patient care through the disease process according to the Texas Nurse Practice Act and Memorial Hermann Health System Standards of Care. Graduate of Nurse Practitioner Program approved by the State of Texas or graduate of an Acute Care Nurse Practitioner Program approved by the State of Texas if area of intended practice is Trauma; Master of Nursing preferred. Acute Care Nurse Practitioner with at least two (2) years' experience in palliative care or hospice; or Family Nurse Practitioner with Advanced Certified Hospice and Palliative Nurse (ACHPN) certification, or post master's certificate in Palliative Care required if area of intended practice is Palliative Care/Supportive Medicine.
