Job Summary and Qualificationsย

Informs the Wound Team RN, CWOCN and the physician/wound clinics of changes in client needs, wounds that require a change to the plan of care for wound care treatments and disease management

Assists with staffing needs to meet service demands.

Accountable for team performance in achieving desired clinical and operational performance measures.

Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program.

Participates in training, coaching of staff or onboarding to the Wound program

Participates and facilitates professional development needs and competency for wound program.

In addition, the position involves other duties such as:

1. Will actively participate in the performance improvement process known as H3W.
