Optum Behavioral Care is seeking a licensed clinician to join our Collaborative Care Model (CoCM) team, embedded within Summit Medical Group primary care clinics in Cookeville and Crossville, Tennessee. This is a hybrid role with one to two remote days per week and the remaining days onsite in clinic.

About the role

Primary responsibilities

• Screen and assess patients for common mental health and substance use conditions

The salary range for this role is $60,000 to $76,000 annually based on full-time employment. • Execute treatments as necessary while document status and observes reactions to medications and treatments.
• Coordinate admissions, discharges, and transfers to deliver quality customer service.
• Plan resident admission as ordered by physician, perform assessment, verify orders with physician, and notify pharmacy, laboratory, and other departments, as necessary.
• Introduce an accurate and thorough report to the on-coming shift to ensure the continuity of maximum resident care.
