Purpose:
The Hospice Registered Nurse is responsible for pain management, symptom control in the delivery of care to hospice patients, as established by the plan of care, the interdisciplinary team, attending physician and Hospice Medical Director.
Territory: Etowah, Calhoun, Cherokee and Cleburn
Schedule: Full-time hours Mon - Friday, Weekend on call every 8-10 weeks and 1 week night, every other week
Position Overview:
- Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family, operating within the plan of care as established by the hospice team and attending physician.
- Assesses the total needs of the patient/family during regularly scheduled and after hour home visits; Documents: assessment, identified problems, nursing interventions, goals, and outcomes of interventions.
- Coordinates total patient/family hospice care under the supervision of the Director and with the interdisciplinary team, the attending physician, and other providers; documents such coordination.
- Collaborates with the interdisciplinary team in the development, review and revision of the clinical component of patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions and documenting outcomes.
- Ensures continuity of care between patient/family, team members, ancillary providers, long term and inpatient care facilities, and the attending physician.
- Communicates patient/family/caregiver needs, ongoing nursing assessment, interventions, goals and outcomes through the interdisciplinary process.
- Provides and documents continuing education of the hospice concept of care to patients and their families/caregivers.
- Supervises the delivery of patient care provided by Hospice Aides, Licensed Practical Nurses; ensuring compliance with the established plan of care and completing required documentation of supervision.
- Facilitates the delivery of hospice services to patients residing in long term care facilities, documenting joint coordination of care with facility professional staff, and attending facility care plan meetings as appropriate.
- Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays.
- Provides ongoing monitoring of patient appropriateness for hospice services and completes required documentation for certification and recertification.
- Interfaces with the patient/family/caregiver, the hospice team, other health care providers in a respectful, professional and courteous manner.
- Maintains compliance with Medicare Conditions of Participation, Joint Commission standards, Hospice and Professional Standards of Nursing Practice, and agency specific policies and procedures.
- Supports community education of hospice for growth and development of the program.
- Dispenses medical supplies in a cost effective manner, as dictated by the plan of care and patient needs.
- Participates in quality improvement, utilization review, and infection control activities as requested.
- Attends appropriate inservices, and participates in continuing education.
- Ensures accurate and current patient chart information by timely and consistent documentation.
- Maintains a professional, well-groomed appearance, adhering to the agency dress code.
- Maintains sound privacy and security practices and prevents privacy or security breaches. If breach occurs, takes corrective action.
Qualifications:
- Current license as a Registered Nurse in the State of Alabama

Reports to Manager of Emergency Department. The Emergency Department Paramedic is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. They practice in a clinical environment that is administered by Nurse Managers and other leaders.
As an integral member of the Emergency Department team the Paramedic will support treatment in the hospital emergency department (ED). The paramedic will be required to assist with patient care treatment at the direction of the physician and/or the Registered Professional Nurse (RN) that directs plan of care, team goals, and performance. The paramedic shall cover the range of services provided in a medical treatment facility in accordance with O.C.G.A. 31-11-59 which outlines the services of paramedics in hospitals and defined hospital specific duties. In addition, the paramedic promotes and restores patients' health and the overall patient experience by collaborating with physicians, RNs, and multi-disciplinary team members, and providing support to patients, families and significant others. The ED Medical Director oversees the scope of practice of the paramedic in the ED in accordance with the Georgia Paramedic Scope of Practice and the Wellstar Paramedic Scope of Practice.
It is expected that all Emergency Department Paramedics possess a Georgia paramedic license and the national registry certification for paramedics. The paramedic must be knowledgeable and adhere to the scope of paramedic practices as outlined in the Georgia Department of Human Resources Division of Public Health Office of Emergency Medical Services scope of practices. Core Responsibilities and Essential Functions:
Patient Care
- Assists medical and nursing staff with the care and treatment of Emergency patients
- Assists in the intake process by greeting and interviewing patients and inputting pertinent medical information into the computer system.

Documentation — complete required documentation promptly and accurately to support resident care.

Education Requirements:
Must be a high school graduate.
Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
Must be licensed with current registration in New York.

Pay Range: $24.00 - $31.10, ** Increased Shift Differentials starting at $4.00 per hour on weekdays, and $8.00 per hour on the weekends**
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Job Summary:
Nursing Assistant - Care Partner - Acute Care Medical Telemetry - Full-Time Day Shift
The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The Care Partner primary focus will be to promote a positive patient experience by partnering with the nurse, physician, healthcare team and other healthcare providers to provide excellence in patient experience; and assist with daily living activities and comfort measures. The Care Partner assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions; i.e. ADL's, ambulation, and vital signs.
The Care Partner may require approved advanced technical training skills may be required i.e. phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters. Performing additional advanced skills may be required to support clinical care and will fluctuate depending on the needs of the unit. Performing clerical functions such as: retrieval of data, maintaining unit supplies, and transporting supplies/patient medications will be required. The Care Partner must be able to work in a fast-paced environment which requires multi-tasking and effective communication with the Registered Nurse and ancillary team. Must be able to perform under stressful conditions with a compassionate, respectful and service mindset. Must be able to take direction from a variety of care providers. Must be organized with excellent communication skills.

Communicates completion of assignment/duties to RN or Charge Nurse.
Documents completion of patient care in EMR (Electronic Medical Record).

Uses positive and respectful communication with all members of healthcare team, patients and families.
Maintains equipment i.e. blood pressure machines, glucometers, and other patient care equipment. When devices are not properly working sequester, tag, and report to appropriate personnel.

Knowledgeable, maintains, and acts as unit expert regarding patient care documentation tools related to their role in the event of downtime.
Safety and Infection Control
Provides a safe environment for the provision of patient care.
Consistent use of proper hand hygiene.

Follows appropriate PPE/Infection control policies in between patient care and equipment usage.
Reinforces the importance of universal precautions with patient and patients family.
