Job Responsibilities

Job Summary

  • Provide comprehensive virtual nutrition assessments and individualized care plans using the Nutrition Care Process (NCP).
  • Deliver motivational interviewing, goal-setting, problem-solving, and relapse-prevention techniques.
  • Coach patients on GLP-1/GIP and other AOM nutrition strategies (managing satiety, protein targets, fiber, hydration, GI side effects, micronutrients) and on non-pharmacologic weight-loss approaches.
  • Coordinate with prescribers to align nutrition plans with medication initiation/titration, labs, and comorbidity management.
  • Use OpenLoop platforms for self-monitoring (weight, food/activity logs) and provide communications for micro-coaching/messages between visits where contracted.
  • Screen for disordered eating risk and escalate when indicated; make warm referrals to behavioral health or specialty care per protocol.

Who You Are:

  • Clinical Excellence & Compliance: Ensure timely, accurate completion of the dietary portion of MDS, RAP, and Resident Care Planning. Maintain descriptive progress notes that reflect the resident's actual response to nutritional care.
  • Quality & Palatability: Monitor meal service to ensure every plate is nutritious, well-prepared, and palatable, strictly adhering to standardized menus and physician diet orders.
  • Resident Connection: Assess patients' nutritional needs, medical histories, and lifestyle to develop individualized nutrition plans. Provide counseling and education on healthy eating habits, disease prevention, and management of chronic conditions.

Qualifications:

  • Credentials: Bachelor's degree in Dietetics/Nutrition, current Registered Dietitian (RD) credential and licensure to practice as a dietitian in the state of Michigan.
  • Experience: 3+ years of clinical experience preferred; 12 years of management/supervisory experience.
  • Skills: Mastery of federal/state regulations and a high level of professionalism in a team environment.
  • Preferred: Proficiency with electronic health records.
