Position will be the system Transfusion Medicine manager, overseeing the Transfusion Medicine departments at all sites. This position will be a member of the Sayre Lab Leadership Team. This position will have the responsibility for the technical aspects of all testing within all Transfusion Medicine departments for the system. This position will be responsible to organize the workflow to result in optimum efficiency utilizing appropriate metrics to maximize the departmental operational efficiencies. The department manager will collaborate with departmental pathologist and other known experts on such technical issues as technical procedures, Quality Assurance, and appropriateness of testing. The Transfusion Medicine manager will report directly to the Director of the Clinical Lab. The Transfusion Medicine Manager will be expected to oversee, assist, and guide the Transfusion Medicine department at Sayre, as well as provide guidance to the Transfusion Medicine departments, Medical Directors and Administrative Directors at affiliated sites.
Education, License & Cert:
Must meet the qualifications of a Laboratory Manager as defined by the New York State Department of Health, Pennsylvania Department of Health and CLIA. Certification as a Medical Technologist by the ASCP Board of Registry or Board eligible preferred.
Experience:
Experienced Medical Technologist with a minimum of 6 years’ experience in their respective specialty. Certification by ASCP Board of Registry or Board eligible is preferred. Certification as Specialist in Blood Bank by ASCP Board of Registry or equivalent is a plus.
Essential Functions:
1. Provide technical leadership in assigned specialized areas within the Sayre Lab and affiliated Transfusion Medicine sites. Keeps up to date with current laboratory practices, procedures and techniques within their respective specialty. Maintains the required level of expertise through a variety of continuing education activities such as attendance at workshops/seminars.
2. Is an active member of the Sayre Lab Leadership team.
3. Must possess demonstrated supervisory and organizational skills.
4. Assists the site Directors and the assigned Pathologist in the evaluation of new instruments, equipment and/or new technical procedures. Makes recommendations for the acquisition of new instruments, equipment and/or new technical procedures.
5. Has primary responsibility for validation of all new equipment, instruments, testing procedures, and computer software updates for the Transfusion Medicine department in the Sayre Lab areas of responsibility as well as provide appropriate guidance to affiliate Transfusion Medicine sites.
6. Has primary responsibility for training staff at the Sayre Lab on all new equipment, instruments and testing procedures.
7. Coordinates and takes the lead in integrating Sayre Lab functions throughout all shifts and sites, including scheduling, monitoring, data collection, data presentation and the implementation of approved new initiatives.
8. Has primary responsibility for the proper maintenance of all equipment at the Sayre laboratory.
9. Assures all policies, procedures, test record keeping, quality control, quality assurance documentation required are maintained and up to date which includes the assurance that the accuracy of all testing is monitored through statistical analysis and developing quality reports at all sites. This applies for all Transfusion Medicine policies for the GMG labs and any Transfusion Medicine related polices held by the hospitals.
10. Appropriately handles and documents proficiency samples to meet regulatory requirements to include rotation among staff on all shifts at the Sayre laboratory, and provide guidance at affiliate sites.
11. Maintains an adequate inventory of expendable supplies for immediate use in the Sayre department.
12. Brings any problems of a technical nature to the attention of the Medical and Administrative Laboratory Director(s). Uses good judgement in the communication of all relevant issues, problems and/or developments in the department. Communicates thoroughly to staff all relevant changes, updates and developments.
13. Assures the department is in compliance with all regulatory agencies for their areas of responsibilities. Communicates any potential deficiencies to the Medical and Administrative Director(s) as necessary.
14. Works collaboratively with the Medical and Administrative Directors and designated Pathologist to establish standards of practice, performance improvement, quality control protocols, Quality Assurance activities to monitor pre‐analytical, analytical and post‐analytical aspects, reporting guidelines, and productivity benchmarks to meet the acceptable guidelines of FDA, CAP and NY state and at least the meets the requirements of the AABB Quality Program.
15. Responsible for validity of test results, quality control and other statistical reports generated in the Sayre department. Reviews reports for accuracy and compliance with established practices at designated intervals. Takes appropriate action as needed upon review with Medical and Administrative Director. Provides appropriate guidance to affiliate Transfusion Medicine sites on validity of test results, quality control and other statistical reports.
16. Performs, documents, and maintains records for non‐routine and routine automated, semi‐automated and manual laboratory tests and related functions such as quality control and instrument maintenance procedures at he Sayre site and provides appropriate guidance to affiliate Transfusion Medicine sites on records and related functions such as quality control and instrument maintenance procedures.
17. Schedules personnel and develops staffing plans and departmental assignments / re‐assignments for the Sayre Lab areas of responsibility to assure optimum performance, workflow and technical accuracy under overall guidance of Director Clinical Laboratory.
18. Performs performance evaluations and participates in/or conducts disciplinary sessions as needed and maintains the documentation for the Sayre Lab areas of responsibility. Provides staff feedback for performance evaluation to the Administrative Directors of the affiliated sites.
19. For the Sayre lab, conducts employee interviews and orientation sessions as needed, maintaining proper documentation.
20. Sets guidelines and performs employee competency and education sessions for all GMG employees who perform work in the Transfusion Medicine department and maintains proper documentation for the Sayre lab. Provides appropriate guidance to affiliate Transfusion Medicine sites on employee competency and education.
21. May perform testing in the department if needed.
22. Is actively involved in the instruction of the Medical Technology students.
23. Prepares for and conducts biannual on‐site inspections by FDA, CAP, and NY state.
24. Observes proper safety practice in performance of job duties.
25. Follows established lab policies and procedures.
26. Participates in and on inter‐ and intra‐disciplinary teams.
27. Uses good judgment and maintains a professional, mature, positive attitude while performing the job duties.

As the Physical Therapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. The primary role of the Home Infusion Nurse is to provide clinical care and education to home-based patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy (the "Company"). The most common route of administration is intravenous, however medications may also be given via subcutaneous infusion or injection, intramuscular injection, or intraspinal (intrathecal and epidural) infusion. Service delivery takes place in patient homes and also in ambulatory infusion suites, where available. All infusion services and medication delivery is provided in accordance with Optum Infusion Pharmacy policies, procedures and therapy-specific protocols, and based on comprehensive orders received from a physician or licensed practitioner. The Home Infusion Nurse fills an integral role in the clinical operations of the organization, working collaboratively with the multi-disciplinary team needed to prepare, deliver and seek reimbursement for the infusion therapy that is safely and effectively administered to each patient.
Schedule for this position will be Monday, Wednesday, Thursday, and Friday, 10am-8pm. This is a heavy travel position, mileage is reimbursed and drive time is part of the schedule. Patient population includes both adult and pediatric patients.
Primary Responsibilities:
- Establish and maintain positive clinical rapport with patients and their families, working closely with them in the provision of infusion therapy, education related to self-care and self-administration, and clinical monitoring of patient response to therapy
- Assess patient appropriateness for home infusion therapy, including safety of the home environment, availability of caregiver support, and willingness of patient and/or caregivers to become as independent in therapy administration and monitoring of response to therapy as possible
- Obtain and document patient's medical history at the start of care and ongoing, particularly as related to the diagnosis being treated with the prescribed infusion therapy
- Complete all required clinical documentation thoroughly and timely, including nursing plans of care (POC), plans of treatment (POT), visit notes, clinical summaries, medication profiles, discharge summaries and any updates provided to members of the patient's care team, as well as obtaining patient and/or caregiver signatures on relevant consents and required forms
- Participate in case conferences as needed to comprehensively plan patient care with the multi-disciplinary care team
- Provide skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders and the plan of treatment
- Establish, secure and maintain vascular access devices as prescribed, including (but not limited to) peripheral IV (PIV) placement, PICC and tunneled central venous catheter dressing changes, and implanted port access
- Administer medications as prescribed, performing therapy-specific clinical assessments in accordance with the Company's policies, procedures and protocols, which may include frequent vital sign monitoring, management of acute infusion reactions, monitoring of side effects, and procurement of laboratory specimens
- Communicate observations of patient response to therapy and overall physical assessment/condition with members of the care team in a timely manner, which may include the nursing supervisor, dispensing pharmacist, registered dietitian, and licensed prescriber, among others
- Establish collaborative relationships with home health agencies (HHAs) delivering services on behalf of / or in conjunction with the Company, providing agency education and support as requested by the nursing supervisor
- Maintain a comprehensive knowledge base of infusion therapies administered in the home and ambulatory infusion center, utilizing Company policies, procedures and protocols in addition to all available training resources to ensure understanding of each therapy before initiating treatment
- Maintain current nursing licensure in all states in which patients are serviced, as required by the respective State Boards of Nursing and/or Department of Health
- Comply with all State and Federal regulations and laws, including employee health and safety protections mandated by the Occupational Safety and Health Administration (OSHA), patient privacy and data security required by the Health Insurance Portability and Accountability Act (HIPAA), as well as all home infusion standards of practice
- Complete all training required upon hire and annually per policy, including cardiopulmonary resuscitation (CPR) certification
- Serve as a professional representative of the Company when meeting with and/or providing education to external customers, payers and referral sources
- Assist in staff orientation and training, serving as a clinical educator and resource as requested
- Drive personal/rental vehicle (per travel policy) in the completion of job duties, including (but not limited to) travel to the Company office, patient homes, area hospitals, physician offices, home health agency offices, and the Company's Ambulatory Infusion Suite (AIS)

Required Qualifications:
- Active and unrestricted licensure as a Registered Nurse (RN) in the state(s) of practice and others as requested

- Current Basic Life Support (BLS) and/or CPR certification, or willing to obtain prior to starting employment

POSITION PURPOSE
Provides and directs safe, effective, and culturally competent care for patients with acute medical and surgical conditions. Through evidence-based practice, utilizes critical thinking to assess, plan, implement, and evaluate outcome-based care. The Acute Care RN manages a diverse patient population with varying levels of acuity, including post-operative, medical, telemetry, and neurologic patients. Supervises and coordinates care provided by LPNs, Patient Care Assistants, and other nursing team members. Facilitates communication with physicians, patients and families, and interdisciplinary team members. ESSENTIAL FUNCTIONS
1. Conducts an initial assessment to gather data.
Makes an initial assessment of new admissions within 30 minutes of the patient’s arrival to the unit.
Completes all areas of the admission assessment database within 24 hours of admission.
2. Identifies patient problems and develops them into an outcome/goal-based plan of care.
Initiates the patient problem list and incorporates relevant medical history into the plan of care.
Analyzes and interprets the assessment data to identify desired patient outcomes/goals.
Determines nursing interventions to achieve those outcomes/goals.
Prioritizes patient care needs/ activities.
Identifies and makes appropriate referrals (i.e., Patient/Family Services, Nutritional Services, Spiritual Care, Diabetic Educator, etc.), and education needs.
Collaborates with other health care disciplines to plan interventions/ patient outcomes/goals.
3. Implements the outcome/goal- based plan of care.
Assigns and coordinates patient care incorporating patient acuity and the skills, knowledge and abilities of the team members.
Organizes and prioritizes care for the patients within the team.
Delegates tasks appropriately to other team members.
Implements the plan of care including procedures, treatments, physicians orders, nursing orders, education plan and medications.
Coordinates implementation of plan of care with the Case Manager, Social Worker and other health care disciplines.
Assists to decrease the patient’s length of stay by helping the patient and family identify and secure appropriate services to satisfy health-related needs upon discharge.

4. Evaluates patient progress and revises the outcome/goal-based plan as needed.
Evaluates patient response(s) to interventions.
Review/modifies/revises the outcome-based plan of care based on ongoing reassessments.
Updates the problem list and outcomes/goals.
5. Communicates relevant clinical information to the team to optimize patient outcomes/goals.
Communicates relevant clinical information to physicians regarding the patient’s condition.
Reports progress toward outcomes/goals and/or changes in condition to other health team members.
Maintains ongoing communication with interdisciplinary team members throughout the shift, at shift change, and transfer of care.
Communicates with other team leaders and members throughout the shift to reassess and prioritize the needs of the unit.
Documents completely, comprehensively and legibly and in accordance with policy.
6. Demonstrates clinical competence.
Utilizes Patient Care Policies and Procedure as references to guide nursing practice.
Assures medication safety by following the “5 Rights” of medication administration.
Prioritizes and implements intervention according to patient assessment.
Demonstrates competency in computer systems and applications required for role.
Receives, transcribes and processes and implements physician orders per policy including dating, timing and verifying accuracy.
Maintains communication with the manager/house supervisor and staffing office regarding admissions, unit needs, acuity numbers and significant events.
Completes assigned shift duties and assures resolution of discrepancies.
7. Manages telemetry and cardiac monitoring when applicable.
Demonstrates clinical competency and technical knowledge in cardiac rhythm interpretation and telemetry monitoring.
Recognizes and responds appropriately to cardiac arrhythmias and other changes in patient condition.
Must be capable of managing multiple patients requiring continuous cardiac monitoring.
Provides appropriate interventions for patients experiencing cardiac events or changes in rhythm.
May be required to assist with data collection and quality improvement initiatives.
Must be willing to serve as a resource to other hospital services (nursing, pharmacy, laboratory, radiology, respiratory, etc.).
May be asked to participate in unit-based committees and provide education to nursing staff as requested.
