Job Summary:

The Inpatient Transitions of Care (TOC) Pharmacy Technician will function as an integral part of the medication management patient care team in collaboration with TOC Clinical Pharmacy Specialists. The Inpatient TOC Pharmacy Technician will be responsible for completing the following tasks to reduce medication-related errors and ensure high-quality patient care. Conducting a best possible medication history (BPMH) on high-risk patients with specific disease states serviced by the TOC Team (Heart Failure, Chronic Obstructive Pulmonary Disease, and Diabetes Mellitus) admitted to an inpatient unit at Wellstar Cobb Medical Center Documenting medication discrepancies for further assessment by the TOC clinical pharmacy specialist Assessing patient medication compliance Completing an insurance coverage assessment Completing prior authorization requests Providing information related to patient assistance programs Facilitating utilization of the Dispensary of Hope (DOH) and Meds-to-Beds programs Completing follow-up care calls post-discharge

Core Responsibilities and Essential Functions:

Perform thorough patient chart reviews a. Conduct a BPMH on high-risk patients with specific disease states serviced by the TOC Team b. Demonstrate a basic understanding of the electronic health record (e.g., Epic) and computer functions c. Identify and clarify medication discrepancies upon admission and discharge, as needed, and communicate findings verbally and/or via the electronic health record Correspond with patients, patients family members, providers, pharmacists, and other members of the healthcare team a. Assess patient medication compliance and communicate findings verbally and/or via the electronic health record b. Coordinate the dispensing of discharge medications, as needed c. Complete follow-up care calls post-discharge and communicate findings Engage in patient assistance programs a. Facilitate utilization of the DOH and Meds-to-Beds programs b. Execute prior authorization requests c. Complete insurance coverage assessments Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct.

The Pharmacy Technician performs department administrative functions that do not require the Pharmacy Managers judgment. Responsible for the administrative and clerical support directly involved with the day-to-day operations of the multiple programs performed by the department. General Duties/Responsibilities
  • Assists with the daily operations of the various Pharmacy Department programs, multiple projects and participate in Plan and regulatory agency audits, as necessary.
  • Makes high volume of outbound calls to members, prescribers, and pharmacies in regard to the Adherence program.
  • Assists members in refilling their medications and reaching out to the prescriber’s office if a member requires additional refills on their maintenance medications.
  • Drafts routine reports and correspondence.
  • Supports all CMS required oversight processes of PBM including but not limited to: Rebates, Explanation of Benefit reports, Transition Reports, and Part B vs Part D.
  • Responds to inquiries from physicians, members, and pharmacies related to formulary and prescription benefits.
  • Retrieves accurate, concise, applicable, and timely drug information.
  • Interacts with Plan's contracted PBM to ensure communications and information to other internal Plan departments as applicable.
  • Interacts with Plan’s contracted PBM in regard to Prior Authorizations, Overrides and in resolving issues pertaining to member’s medications.
  • Makes outbound calls to pharmacies and prescribers to resolve prescription issues such as medication requires Prior Authorization, assist retail/mail order pharmacies in resolving third party rejections, and reviewing daily rejections while ensuring we are CMS compliant.
  • Interacts with internal plan departments to address pharmacy related issues.
  • Works closely with compliance department to make sure Pharmacy Programs meet the criteria established by CMS.
  • Ensures all HMO’s members information is kept confidential according to HIPAA.

Summary:

The Specialty Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Specialty Clinical Pharmacists provide proficient direct patient-centered care in a decentralized practice setting. Specialty Clinical Pharmacists serve as departmental resources and liaisons to other departments, or external groups. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Specialty Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy students and pharmacy residents. The Specialty Clinical Pharmacists will participate in the quality management program with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve, and evaluating the effectiveness of those interventions.

The Specialty Clinical Pharmacists will be responsible for the clinical management of patients on a variety of specialty therapies for medical conditions including but not limited to Hepatitis C, inflammatory conditions (gastroenterology, dermatology, asthma, and allergy), pulmonary conditions (idiopathic pulmonary fibrosis, cystic fibrosis), neurology (multiple sclerosis, migraine headache), dyslipidemia, osteoporosis, and oncology.

Responsibilities:

Job Requirements:

Education/Skills

  • Doctor of Pharmacy degree from an ACPE accredited school of Pharmacy is required.
  • Must accrue American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.

Experience

  • PGY1 residency or 5 years of clinical practice (community, ambulatory care, PBM, or hospital) required.
  • PGY2 residency preferred.
  • Specialty pharmacy experience preferred.
  • EPIC suite products experience preferred (EMR, Compass Rose, Willow Ambulatory)
  • Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in areas appropriate for clinical assignments required.

Licenses, Registrations, or Certifications
