Participate with the interdisciplinary team in developing, implementing and evaluating ways to achieve patient goals and minimize rehospitalizations.
Mentor and educate nurses, working with physicians and other medical professionals, and serve as a resource to all staff within the nursing unit regarding the quality of services provided.
Oversee medication management to ensure adequate supplies and that all medications are handled in accordance Genesis policy
Provide direct patient care along with the other unit nurses to maintain skills. Qualifications:

Must be a graduate of an accredited school of nursing.

Associate Degree in Nursing, three (3) years of nursing experience.

Nursing Diploma, three (3) years of nursing experience. BSN, two (2) years nursing experience.

At least one year of nurse leadership experience is required.

Must possess current licensure by the State Board of Examiners of Nurses in the nursing center state.

Must maintain current BLS/CPR certification.

  • Licensed physician as a Medical Doctor (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school and completion of residency through an accredited residency program required.

Provides evaluation and physical therapy treatment in accordance with agency standards the laws and regulations governing the provision of physical therapy services in the state of Maryland District of Columbia or Virginia and other regulatory requirements.

Primary Duties and Responsibilities


Analyzes and documents patients and family's response to interventions. Evaluates measurable progress toward goals and revise the plan of care. Identifies and modifies underlying factors that impede progress toward goals. Makes recommendations for updates in plan of care using knowledge of interventions and resources.Establishes and documents teaching plan for patients/families based on patients' level of knowledge diagnosis prescribed treatment and available resources. Communicates (verbal written demonstration) respectfully with patients/families supervisor peers and other health team members. Establishes visit schedule and informs patient/family and team supervisor.Contributes to the achievement of established goals and objectives and adheres to department policies procedures quality standards and safety standards. Completes annual Code of Conduct Compliance and other mandatory training.Implements an effective physical therapy treatment plan in conjunction with the patient their family/caregiver the patient's physician and other members of the health care team and modifying treatment plan as needed.Evaluation includes interview observation palpation specialized assessments including but not limited to goniometry muscle strength testing and sensory testing. Such assessments require use of tools (e.g. goniometer sphygmomanometer stethoscope) and the possession of sight and hearing senses or the use of prosthetics that will enable these senses to function adequately.Independently implements and documents plan of care for patients with a routine to complex problems to facilitate continuity of care. Documented interventions are related to planning of care. Interventions reflect the standard of care for patient condition/diagnosis. Interventions include utilizing agency and community resources. Performs therapy interventions consistent with home care protocols. Interventions reflect knowledge of standard home care supplies and equipment used inpatient care.Independently organizes patient assignments for completion of tasks within acceptable time frames. Seeks assistance from PT III Rehab Care Manager or supervisor regarding complex patient problems. Maintains necessary clinical records collects data and prepares reports on activities. Submits completed admission records within 48 hours and daily records within 24 hours with a Ninety-five percent accuracy on technical audit. Recertification completed prior to recert date. Maintains caseload mix and productivity for acuity level of patients.Initiates and participates in regularly scheduled case conferences; includes all disciplines. Includes patient and family in mutual goal setting and care plan revision.Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.Performs history and assessment related to episode of patient illness to determine the patient's habilitation/rehabilitation needs. Recommends adaptive equipment and/or home modifications to maximize the patient's functional abilities and safety. Identifies primary patient problems from assessment. Identifies discharge planning needs including referrals to appropriate community resources. Interprets abnormal clinical data to anticipate problems associated with changing patient status.Performs procedures and modalities including but not limited to therapeutic exercise functional mobility training gait training heat/cold application and ultrasound. Minimal Qualifications
Education
  • Bachelor's degree in Physical Therapy from an accredited school required
Experience
  • 1-2 years Clinical experience required
Licenses and Certifications
  • PT - Physical Therapist License to practice Physical Therapy in the District of Columbia Maryland or Virginia or any combination based on work location(s) Upon Hire required and
  • DL - Valid State Driver's License in good standing with a dependable vehicle for transportation Upon Hire required
  • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
  • Additional unit/specialty certifications may vary by department or business unit.

General Summary of Position
Assists physician with routine to complex invasive diagnostic and therapeutic cardiovascular procedures such as cardiac catheterizations; cardiac and vascular percutaneous transluminal angioplasties structural heart and other interventional procedures. Performs a variety of related ancillary support activities and other duties as assigned.

Primary Duties and Responsibilities

  • Assists/scrubs in with physician during routine to complex invasive diagnostic and therapeutic cardiac and vascular procedures. Prepares catheters balloons guiding wires etc. to physician as needed.
  • Prepares cleans and drapes appropriate site prior to invasive procedure ensuring a sterile field according to Association of Operating Room Nurse (AORN) standards.
  • Enters required patient data into the computer connects electrocardiograph (EKG) and pulse oximeter monitors. Provides physical and psychological comfort to patients prior to during and post-procedure.
  • Monitors electrocardiograph (ECG) blood pressure and other hemodynamic parameters during procedure in the Cardiac Catherization Laboratory (CCL).
  • Assists with intra-aortic balloon pump (IABP) other cardiac assist devices and sets-up/removes arterial and venous lines as needed in the CCL CCU and ICU. Draws arterial blood gases (ABG) as requested by physician. Performs activated coagulation time tests to determine appropriate arterial/venous sheaths removal. Accompanies and monitors patient during transports.
  • Prepares/cleans arranges and stocks room/equipment for next procedure. Maintains and stocks inventory such as catheters defibrillators monitors and temporary pacemakers. Provides monitors/external pacemakers to various locations within the hospital.
  • Document required data prior to during and post-procedure and enters appropriate codes/charges/equipment used into computer for statistical/billing reports.
  • Discharges/transports patients from procedure room. Communicates/instructs patient regarding post procedure site care instructions and precautions as appropriate.
  • Provides patient's family/significant others with information related to procedures according to departmental policy and procedures.
  • Participates/responds to post-cardiac catheterization site bleeding hematomas pain loss of distal pulse etc. as appropriate and documents findings and resolution.
  • Participates in the orientation training and inservices of other/new personnel as defined by CCL manager and departmental educator.
  • Administers contrast at the direction of the physician while the physician is in the room directing the administration and dosage.

Minimal Qualifications
Education

  • High School Diploma or GED including completion of a Cardiovascular Technologist training program and/or related college level education in basic sciences. required

Experience

  • Experience in cardiovascular laboratory procedures particularly as relates to cardiac and vascular interventional procedures preferred

Licenses and Certifications

  • RCIS - Registered Cardiovascular Invasive Specialist with Cardiovascular Credentialing International (CCI) within 1 Year required or
  • R.T.(R)(CI) - Registered Technologist in Cardiac-Interventional Radiography within 1 Year required
  • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required
  • Additional unit/specialty certifications may vary by department or business unit.
  • Health Services\ACLS required

Knowledge Skills and Abilities

  • May require the use of specialized medical equipment (i.e. EEG EKG blood analyzer) or data processing equipment (i.e. tape drives consoles).
