The Journey Clinic RN provides comprehensive nursing care for patients in an ambulatory care environment based on individual physical, emotional, spiritual needs, and appropriate care strategies throughout the lifespan. This position facilitates care coordination by using the nursing process to assess, identify, plan, implement, and evaluate all aspects of care and teaching needs via direct and telephonic/telehealth or electronic encounters. This role works in collaboration with other members of the health care team. The Clinic RN maintains compliance with professional nursing standards and regulatory requirements and supports clinicians in delivering quality health care to patients. Monitor staffing levels to balance exceptional care with financial responsibility.
Ensure clinical operations reflect current coding, billing, and reimbursement expectations.
Develop, implement, and evaluate a comprehensive Continuous Quality Improvement (CQI) plan.
Respond promptly to patient concerns, incident reports, and safety events.
Ensure compliance with all regulatory, accrediting, and licensing requirements.
Proactively identify and correct unsafe conditions.

Minimum Qualifications
Bachelor of Science in Nursing (BSN) required, or actively pursuing a BSN with completion expected within 3 years of hire.
Minimum of 5 years of nursing experience, including at least 3 years in nursing leadership.
Current Iowa RN license required; Illinois RN license required if practicing in Illinois.

Position Purpose:
Our Physical Therapy Assistants, PTA, provide physical therapy services to Mt. Carmel patients under the guidance and supervision of a Physical Therapist and following a written plan of care established by the physician and physical therapist. Our PTAs assist in assessing our patients’ need for physical therapy devices and interventions and instructs our patients and their family members in the use and care of therapeutic appliances and devices, such as wheelchairs, braces, etc.

Minimum Qualifications:
- Graduation from a two-year college-level program approved by the American Physical Therapy Association.
- Minimum of six months experience under a qualified Physical Therapist.
- Home health experience preferred.
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
- Must accept responsibility for maintaining clinical practice skills learning and adhering to agency’s policies and procedures on an on-going basis.
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
Must have current Driver’s license and reliable transportation to and from work site.
