Neurology 8E cares for patients experiencing acute and chronic neurological conditions such as strokes, seizures, brain tumors, and brain bleeds. Team members gain valuable experience supporting complex neuro assessments, safety-focused care, and close interdisciplinary collaboration with therapy and neurology teams.
Overview
Utilizes the nursing process to provide patient care under the supervision of person(s) licensed by a State regulatory board to prescribe medication and treatment, or under the direction of a Registered Professional Nurse (RN) . Maintains professional accountability for provision of patient care for the assigned patients. Coordinates the patient care in conjunction with other departments. SUMMARY
Provides medically prescribed physical therapy treatment for patients suffering from injuries, deconditioning, or muscle, nerve, joint, and bone diseases to restore function, relieve pain, and prevent disability.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
- Evaluates program participants through appropriate evaluation procedures and develops written treatment programs for each participant.
- Administers appropriate treatment programs and communicates program participant needs to appropriate staff. Provides needed re-evaluation and programmatic changes as necessary.
- Maintains timely and appropriate program participant/administrative and department documentation.
- Ensures adherence to accreditation standards, licensure standards, professional code of ethics, and confidentiality.
- Attends programmatic meetings on request for team reports, policy setting, exchange of information, etc.; attends other appropriate meetings/conferences as needed.
- Attends clinical meetings to exchange, discuss, and evaluate program participant treatment programs.
- Serves on rehabilitation treatment team(s). Communicates with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources as needed to ensure effectiveness of physical therapy program.
- Plans and implements in-service programs for staff and other groups as requested/assigned by supervisor.
- Participates in orientation programs for new staff, program participants, and PT/PTA students.
- Writes service summaries as requested by case manager/program director and maintains daily billing summaries and turns them into the appropriate staff.
- Performs other related duties and activities as required.
SUPERVISORY RESPONSIBILITIES
Disciplines specific supervision of PTAs and Interns as required.

The primary patient population is composed of stroke, brain, and spinal conditions that range from; seizures, brain aneurysm, laminectomy, cervical fusion, stable craniotomy mechanical thrombectomy after 24 hours of ICU recovery.

Responsibilities
- Provides direction, supervision and evaluation of medical social services to patients in the home to ensure compliance with state practice act(s) and our policies and procedures, applicable healthcare standards, governmental laws and regulations
- Instructs, treats, observes and evaluates patients exhibiting significant social and emotional problems affecting their health status
- Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
- Participates in the development and periodic re-evaluation of the physician's Plan of Treatment for patients needing social work services
- Utilizes special skills and judgment to resolve social and emotional responses connected with the illness
- Observes, records and reports changes in patients' condition and response to treatment to Clinical Manager and the physician
- Participates in the discharge planning process
- Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses
- Provides ongoing assessment of patient and family needs and responses to teaching
- Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
- Utilizes appropriate community resources to achieve identified objectives
- Supervises, instructs and evaluates the performance of the Social Work Assistant (BSW) to assure that all medical social services are provided to patients in compliance with Company, government and professional standards
- Maintains and submits documentation as required by the Company and/or facility including any case conferences, patient/physician/community contacts, visit reports, progress notes, and confers with other health care disciplines in providing optimum patient care
- Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policies and regulations

Qualifications
- Master's Degree preferred from a school of social work accredited by the Council on Social Work Education with six months' social work experience in a healthcare setting

clearance prior to assignment and periodically as required by Company policy and state regulations - Good organization and communication skills and ability to function with minimal supervision also required
