Job Summary
Plans and implements specific treatment programs for individual patients according to principles and practices of occupational therapy. Participates in operational and clinical functions for the department, hospital and corporation. Provides direction and assistance to Occupational Therapists, Occupational Therapist Assistants, Occupational Therapy Students, and Therapy Aides in the area of patient care. Complies with laws and regulations of appropriate state and federal agencies and Joint Commission guidelines. An advanced level of clinical and interpersonal skills are required. Essential Functions

- Performs appropriate initial occupational therapy evaluations and re-evaluations and designs an occupational therapy plan of care
- Demonstrates advanced knowledge of diagnoses/conditions seen in occupational therapy
- Selects, performs, and interprets the results of OT examinations of neurological, musculoskeletal, cardiovascular, and pulmonary systems
- Performs and interprets standardized evaluations appropriate for the patient
- Identifies actual or potential problems that would impact the OT treatment plan as well as the overall patient plan of care
- Establishes specific treatment goals based on the results of the initial assessment
- Designs the treatment plan appropriate for the patient's age, educational level, and cultural background
- Designs a plan of intervention that is coordinated and integrated with other services.

- Implements and manages a specific and comprehensive treatment plan, and participates in patient discharge planning

Distinguishing Characteristics: This is the entry level class in the professional planning series. This class is distinguished from the Associate Planner by the performance of the more routine tasks and duties assigned to positions within the series including the responsibility for assignments in the fields of current or advance planning which require the application of fundamental planning principles. Since this class is typically used as a training class, employees may have limited or no directly related work experience.
Position Snapshot/A Day in the Life: Under the direction of a Senior Planner, this is the entry level class in the professional planning series. A typical day consists of reviewing development projects, collection of data to prepare staff reports for Planning Commission/City Council, provide information to developers and general public, evaluate and approve sign proposals, review and process design reviews, conditional use permits, variances and work on other planning related projects. Assist the general public via telephone and email.
Essential Functions
The employee must have the ability to:
- Prepare agenda items and support material including resolutions and agreements for the City Council and Planning Commission, various committees and advisory boards as directed.
- Review development projects and serve as liaison for the Planning Division in meetings with developers, architects, engineers, consultants, outside agencies and the general public regarding City development policies and standards.

Working Conditions: Position requires prolonged sitting, standing, walking, reaching, twisting, turning, kneeling, bending, and stooping in the performance of daily activities. Additionally, the position requires near and far vision when preparing and reading written reports and other work related documents. Experience and Training Guidelines
A combination of experience and training that would likely provide the required knowledge and abilities is qualifying. The employee must have the knowledge of:

- Aligning the rollers in the equipment to meet all dimensional requirements of job order. This includes adjustment for taper, TIR, roller surface finish, and finish dimensions.

Here are a few of the daily responsibilities of an LPN:
- Direct the daily activities of the certified nursing assistants and resident care associates in accordance with current federal, state and local regulations and established company policies and procedures.
- Follow the nursing model to promote holistic care for the residents.
- Ensure that policies and procedures are complied with by nursing personnel assigned.
- Participate in the admission, discharge and transfers of residents as required.
- Assess for changes in residents' status, notifying the physician and resident's family or representative and documenting accordingly.
- Transcribe physician orders to medical record and carry out orders as written.
- Prepare and administer medications as per physicians' orders and observe for adverse effects as indicated.
- Perform wound treatments per physicians' orders, observe for changes and documents accordingly as indicated.
- Schedule follow up appointments for residents and transportation needs as indicated.
- Provide nursing leadership to nursing personnel assigned to the unit.
- Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures.
- Perform rounds to ensure resident needs are being met and personnel are performing their assigned duties.
- Collaborate with other members of the interdisciplinary team as needed to ensure residents' needs are holistically met.
- Report any incidents or unusual occurrences to the community's leadership team.
- Ensure that there is adequate stock of medications, supplies, equipment and notify appropriate personnel of needs.
- Initiate, review and update care plans as required.
- Participate in the survey process when inspection surveys arise.
- May participate in QAPI activities as needed or assigned.
- Ensure that infection control principles are followed and help correct deficiencies in practice as noted.
- Perform other duties as assigned.
- Comply with all policies and procedures and any updates.
Position Requirements
Education: A nursing degree from an accredited college or university
Licenses/Certifications: Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state
Experience: 1+ years of Nursing experience preferred, Dememtia care experience highly preferred

What qualifications you will need:
- Graduate of Nursing School or Bachelor's degree in healthcare field.
- Two to five years clinical nursing experience. Two years clinical quality improvement or clinical education experience desired but not required. RN Strongly preferred
- Florida RN license and/or approved multistate RN license required. Compact license holders have 60 days to obtain the Florida license after relocation.
- BLS-AHA Provider issued card required within 30 days of start
