PURPOSE AND SCOPE:
The Inpatient RN provides direct patient care in accordance to company policies and procedures which includes FMCNA compliance programs and contracted hospitals’ policies and procedures. Areas of practice are diverse, including, but not limited to, hemodialysis, peritoneal dialysis, continuous renal replacement therapies, apheresis, and ultrafiltration. The Inpatient RN will partner with the hospital to provide quality care to the patient and deliver care that provides a positive patient experience.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Under limited supervision, applies considerable knowledge to perform a broad range of tasks which include but are not limited to direct patient care in multiple modalities not limited to hemodialysis, peritoneal dialysis, ultrafiltration, continuous renal replacement therapy, and Apheresis.
- Provides supervision and direction of Patient Care Technicians and clerical staff; provides direction and education to ICU staff for CRRT.
- Responsible for direct patient care of assigned patients in the Inpatient Renal Replacement and Apheresis Services setting and providing the appropriate treatment technique for assigned patients as prescribed in all modality procedures, as appropriately defined – hemodialysis, peritoneal, continuous renal replacement therapies, ultrafiltration, and Apheresis.
- Assesses patients’ responses to treatment therapy making appropriate adjustments and modifications to the treatment plan as indicated by the appropriately credentialed physician. Communicate problems or concerns to the Inpatient Program Manager, appropriately credentialed physician, patient hospital primary nurse and others as indicated.
- Delivers safe, effective care in a timely an efficient manner.
- Gives organized, concise shift reports for patients on assigned shifts. Apply high level thinking skills to shift patient schedules to meet needs of more critical patients due to labs, MD order, or patient assessment.
- Responsible for the implementation, administration, monitoring, and documentation of patient’s response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier.
- Collaborates and communicates with physicians and other members of the healthcare team to interpret, adjust, and coordinate daily patient care plan to ensure continuity of care.
- Initiates and communicates to FMS facility nursing staff and appropriate hospital personnel as needed.
- Accurately documents all treatment information in the individual patient record (ACES) and the hospital record, as needed.
- Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding areas between treatments according to inpatient renal services policies and procedures.
- Conducts all tasks necessary for preparation for dialysis treatment and documents where appropriate.
- Performs all required pre-treatment dialysis machine alarm testing.
- If applicable, initiates Solution Delivery System.
- Operates all related equipment appropriately and safely and provides minor trouble shooting when necessary; efficiently utilizes supplies to prevent waste.
- Understands, conducts, and documents appropriately Dialysis/Apheresis machine safety tests/alarm tests, equipment calibration, dialysate testing, machine safety tests, functional testing, and internal and external disinfection on all water & dialysis machines, and complies with the documentation/notification standards per FMS policies.
- Completes and documents ongoing participation in QAI activities.
- Participates in staff meetings as scheduled.
- Participates in staff training and orientation as assigned.
- Acts as a subject matter expert to hospital staff for dialysis related patient/equipment complications.
- Responsible for reporting all new or unusual incidents, information, complaints, or problems to the inpatient manager and other parties per the Adverse Event Report requirements. May be assigned to in-center patient care on an as needed basis
- Review and comply with the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations.

- Certification in Nephrology Nursing, Apheresis Nursing, or both is highly encouraged.
EXPERIENCE AND REQUIRED SKILLS:
- One year RN experience required and required nursing skills.
- Minimum 6 months nephrology experience required before working independently with no other Dialysis RNs to provide dialysis and ultrafiltration services.
- Minimum 6 months Nephrology experience required before working independently with no other Apheresis RNs to provide Apheresis services.
- Demonstrated strong assessment skills related to the critically ill patient.
- ICU experience preferred but not required.
- Successful completion of training course in the theory and practice of care of the ESRD patient – Progressive Renal Education Preparation.
- BLS CPR Recognition.
- ACLS CPR Recognition preferred, but not required.
- Demonstrates organizational and time management skills, and a competency toward completing continuing and ongoing Renal/Nephrology education.

Position Responsibilities
- Prepare for job setup by reviewing work orders, tie-in sheets, operator documents, blueprints, or CATIA plots for production orders, first lots, or tool tryouts.
- Review Work Orders (W/O) to confirm the correct type and quantity of materials and tools or fixtures are issued and free from visible defects, updating CMES as necessary.
- Identify the type of setup required based on documentation and check machine setups or parts completed by previous operators to assess job progress and ensure safe working conditions.
- Select and verify the condition of machine accessories and holding devices (e.g., chucks, vises, templates) and identify any required fixtures or shop aids.
- Attach air nozzles or suction devices for vacuum fixtures, securing them with bolts or clamps as specified in the operator setup documents.
- Use overhead cranes to load and unload parts and fixtures as required for the machining process.
- Upload and download CNC (Computer Numerical Controlled) programs to machine controllers using computing equipment from various servers or databases.
- Select and check cutting tools according to program specifications, ensuring they are the correct type, length, diameter, and in good condition before installation.
- Set machine coordinates or index points per CNC documents, including cutter offsets, tool heights, and probes, using appropriate measuring tools.
- Run CNC programs to machine rough and precision cut parts, monitoring machine operations for unusual vibrations, sounds, or warning indicators.
- Adjust machine settings (e.g., speeds and feeds) to correct machining issues or prepare the machine for subsequent operations, including head and spindle angles.

Determines type and frequency of wound/ostomy care provided to the patient using their expert judgment, including modifications as the patients condition changes
* Upon receiving physician order for consult, marks patients for ostomy surgery, selecting the best and alternative sites for the surgeons consideration, provides education to the patient and family pre- and post-op and guidance for psychosocial, sexual and body image adaption to promote optimal client outcomes.
* Performs the Nursing Process (assessment; diagnosis; identification of outcomes; planning; implementation and evaluation) in the performance of clinical care using evidence-based practice; analytical/critical thinking insuring that care is individualized; coordinating care through effective partnerships; caring is central to achieve optimal patient care outcomes
* Provides relationship-based care delivery patient-centered care by partnering with the patient and significant others (as appropriate) using comprehensive hand-off communication processes; interdisciplinary evaluation and communication (i.e., unit rounds, case reviews, peer review, etc.); just in time and planned patient teaching; keeping the patient informed using appropriate communication tools and making the patients goals the focus of the plan of care.
* Practices using current clinical practice standards with a focus on safety and quality
* When performing duties in outpatient Wound Care and Hyperbaric Unit:
* Demonstrates knowledge and understanding of the Healing Strategies for wound care and the role of HBO therapy, and the different approved clinical indications and treatment protocols for HBO treatment.
* Delivers care to the patient immediately prior to, during, and immediately following the HBO
* treatment that is consistent with existing policies and Healing Strategies within his/her scope
* of practice and documents all aspects of that care.
* Monitors and ensures the proper delivery of treatment gas as ordered.
* Monitors the patients continually during treatments for symptoms of oxygen intolerance, claustrophobia, hypoglycemic reactions, decompression sickness or any other adverse reaction to HBO treatments.
* Under the direction of the program manager or designee, appropriately intervenes in the
* event of an adverse reaction and follows established protocols.
* Demonstrates competence in reinforcing age specific patient and family education on
* common topics related to the HBO therapy and related underlying disease processes.
* Demonstrates competence in performing ordered treatments including but not limited to
* vital signs, blood sugars, and emergency procedures, consistent with existing policies
* and within his/her scope of practice.
* Documents required elements of each treatment in a clear, concise manner that includes a description of the chamber settings, duration of treatment, patient response to the procedure,
* and any other relevant observations.
* Chamber Operations
* Demonstrates and applies basic knowledge about HBOT chamber operations to ensure
* efficacy of treatment and safety of the patient and those in the immediate area.
* Demonstrates knowledge of the basic physiological principles/concepts of the HBO
* treatment and incorporates them into the treatment process.
* Operates controls to pressurize and depressurize the chamber according to prescribed
* policies for clinical indications and physician orders.
* Maintains accurate logs and records of the time, pressures, depths, etc. of chamber operations according to established protocols.
* Performs maintenance of the hyperbaric chamber (s) and related support systems under the guidance of the Program Director/Safety Coordinator.
* Cleans chamber following each treatment, according to recommendations of manufacturer,
* and in accordance with established facility infection control policies.
* Maintains emergency equipment and supplies located in the HBO area. Documents equipment checks as required by established policies.
Teamwork and Collaboration
* The WOCN serves as clinical expert in various hospital committees, community groups and organizations (United Ostomy Association, Diabetic Outpatient Clinic and local home care agencies); leads outcome evaluations including quarterly NDNQI pressure ulcer prevalence; participates in improvement initiatives related to WOC nursing practice; evaluates and develops product formularies and algorithms to support best practices for the prevention and treatment of wound, ostomy and continence conditions and facilitates the needs of the bariatric population with specialty bed management.

This role often meets and supports families in circumstances of grief while advocating for donation authorization and ensuring all potential donor families receive information about organ and tissue donation.
Duties & Responsibilities
The following job duties and responsibilities are stated in broad terms and not intended to be inclusive. Those in bold are essential duties of the position:
- Approach families of potential donors primarily in person and onsite. While also responding to onsite assignments of potential donor referrals.
- Serve as an ambassador and partner with hospital care providers through education and support with the goal of obtaining authorization for organ, tissue, and eye donation.
- Maintain an individual authorization rate, which demonstrates best practice skills in the area of obtaining authorization.
- Complete the authorization process/paperwork with the legal decision maker and educate the family about specifics relevant to each authorization for the family to make an informed decision about donation.
- Work with the legal next of kin, in cases where the potential donor has indicated their decision to be an organ donor by first-person authorization, to honor their loved one's decision to be a donor.
- Collaborate with the hospital care team to evaluate and understand family dynamics & to overcome any barriers with the objective of being optimally prepared to engage with families as timely as possible to offer donation.
- Collaborate with Gift of Life clinical personnel to coordinate family conversations when response and readiness triggers have been indicated.
- Determine when a referral may require consultation/escalation, seek appropriate assistance, and follow provided guidance.
- Assess donor history by accurately completing a Donor Risk Assessment Interview (DRAI) with family member(s) or other appropriate historian(s).
- Post donation follow-up, as scheduled.

Required
- Combined total of 7 years' work experience and education. Work experience should be in a mental health-related field, nursing, a first responder role, pastoral and/or spiritual care, or healthcare.
