About the Role

Record the medical history and provide physical exams and follow-up care according to treatment plan and medical protocols.

Responsibilities

• Provide individual patient instruction in accordance to prescribed treatment plan and medical protocols.
• Prescribe appropriate medications to patients as needed according to medical protocols and standard medical practice.
• Provide medical services, within scope of practice as determined by licensure and/or certification.
• Provide additional medical services, as determined by Chief Medical Officer, within your scope of practice, such as Medication Assisted Therapy (MAT) or Pre-Exposure Prophylaxis (PrEP).
• Accurately complete all necessary charting according to clinic protocols and mandated by funding source guidelines and standard medical practice.
• Identify physical, nutritional and/or lifestyle problems and refers to appropriate on-site and off-site resources.
• Identify psychosocial and emotional problems/issues and refers to on-site or off-site resources.
• Must be in compliance with all clinical and administrative quality measures such as, HEDIS metrics and patient productivity.
• Consult with supervising/collaborating physician for patient evaluation as needed.
• Perform timely documentation/action within Electronic Health Record (EHR) including but not limited to, diagnosis, treatment plan, orders, medications, etc.
• Take timely action on pending EHR PAQ items.
• Issue timely responses to E-Consult specialist reviewers.
• Able to provide medical services on telehealth platform provided by EVCHC.
• Adhere to Quality Improvement guidelines and participates in Quality Improvement programs for chronic diseases.
• Work closely with Registered Dieticians/Health Educators in order to provide appropriate education to patients.
• Participate in monthly Provider Meetings as required.
• Serve as a preceptor instructor to Nurse Practitioner, Physician Assistant, or medical students as assigned.
• Attend conferences and seminars when requested and will comply with continuing education requirements.
• Participate in provider recruitment activities.
• Participate in activities that foster the development of clinical support team members.

Qualifications

• Satisfactory completion of an accredited Advanced Practice Provider training program.
• Current license in the State of California as a Nurse Practitioner/Physician Assistant.
• Current DEA certification.
• Current Board Certification from accredited organization for the certification of Advanced Practice Providers.
• Current CPR certifications.
• Must relate well to all cultural, ethnic, and socio-economic groups within the East Valley service area.

Required Skills

• In collaboration with the Regional Chief Clinical Officer (RCCO), train new PCPs in a market on using our technology/ EMR

• In collaboration with the Regional Chief Clinical Officer (RCCO), provide bridging care in centers that are understaffed with providers or need clinical coverage by seeing patients in clinic or virtually, addressing workflow items and returning patient phone calls

• Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education.

• Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient’s cultural background, level of understanding, personality and support systems. Serves as a patient advocate.

• Patient management includes the following:

  • Communicating with patients virtually or in a clinic

  • Ordering and interpreting appropriate laboratory and diagnostic studies.

  • Referring patients for consultation when indicated

What you'll be working on:

• Managing a patient panel with a broad array of patient needs; conducting a mix of acute, chronic, and well visits
• Treating patients in-office or in testing centers as well as conducting occasional tele-health visits
• Continuous learning during weekly Clinical Rounds and through other modalities
• Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams

Education, licenses, and experiences required for this role:

One Medical is committed to fair and equitable compensation practices. The base salary range for this role is $252,000 to $267,800 per year. Total compensation packages may be based on factors unique to particular candidates, such skill sets, depth of experience, and work location. Reporting to the group’s research head and collaborating with international research/analyst teams, as Research Operations Lead you’ll play an integral role in business transformation, building out and steering ‘operational command’ to essentially ensure/standardise global research performance.

Through establishing/improving dashboards, research operational metrics etc, you’ll develop scalable processes that are embedded into how research runs on a daily basis. You’ll become the ‘custodian’ of data quality and methodological consistency across global teams, effectively overseeing the group’s research data dictionary, definitions, and methodology standards. In so doing this will not only improve the group’s market intelligence output to clients – from market forecasting to greater depth reporting/analysis – but also enhance actionable insights within the business for senior stakeholders to reliably act upon. Additionally you’ll naturally work closely with product and commercial teams as services and capabilities continue to advance.

It’s very likely you’ll have gained significant expertise in change management and/or operations within a market information/data intelligence firm or related, and are now seeking to take some greater ownership. You’ll be a strong project manager (Six Sigma/Lean proficiency advantageous); have advanced Excel; demonstrable data visualisation skills; experience in auditing large datasets; have managed third party data partnerships; standardised for global teams. You’ll also be a superlative communicator, highly flexible, precise, strategic and big picture orientated!
