RN HOSPICE Care at Home - Columbia County - Full time
The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families residing primarily in the Columbia County area. Here at St. Peter's Health Partner's, we care for more people in more places.

Per Diem On-Call Registered Nurse (RN ) Hospice in the Columbia-Greene county areas - Evenings/Overnights
The Community Hospice has a wonderful opportunity for a Per diem RN to work as part of our Hospice Care Team caring for patients and families in the homecare and nursing home setting after hours on call.
Workdays and hours are based on need but could include hours during the week Monday to Friday between hours of 4pm-8a and could also include weekend eve/overnight needs.

The pediatric patient may be an overnight observation after surgery.
We do not have a pediatric group as our Hospitalists are adult only.
We are primarily medical patients, and the designated Covid unit if the patient does not require PCU or ICU level care.
We see patients with Pneumonia, Congested Heart Failure, Abdominal Pain, Altered Mental Status, Chronic Obstructive Pulmonary Disease, Hypertension, Hypotension, Diabetes, Shortness of Breath, and so much more.
We also provide after care for patients that have gone down for a Heart Catheterization.
Many of our patients are on hemodialysis which is done in another unit in the hospital
- we take them to their HD and bring them back.
We take care of patients that have wounds and wound vacs.
We really have the full spectrum of medical diagnosis on our unit.
We work 12 hour shifts: Full time is considered three 12 hour shifts per week.
We ask that everyone makes themselves available every other weekend although there are times when we can pull teammates off the weekend to staff during the week, and we rotate this to be fair.
We have a Holiday schedule that is assigned when a teammate starts.
We alternate Holiday’s to be fair.
We have a designated charge nurse on each shift.

5HV – Cardiovascular Surgical Intensive Care Unit (CSICU) The Cardiovascular Surgical Intensive Care Unit is located on the 5 th floor of the Heart and Vascular Hospital.
The staff in CSICU, a 10 bed unit, provides care for critically ill patients requiring postoperative recovery of cardiothoracic, vascular surgery, ECMO and/or intensive care interventions.

3 East – Acute Care Medicine- A 28 bed unit created to help facilitate throughput for the UNC REX Emergency Department.
Our patients are admitted with a wide variety of medical diagnoses including but not limited to TIA, falls, syncope, nausea and vomiting, cellulitis, anemia and flu.
Patients admitted to 3 East should be medically stable for discharge within 24 to 72 hours from admission.

Position Overview
The Director of Nursing (DON) or Director of Patient Care Services provides clinical leadership for Loma Linda University Hospice’s daily care delivery operations across a market census, supervising a multidisciplinary clinical team comprising of nurses, aides, therapists and interdisciplinary team members. This role balances operational excellence with empathetic leadership ensuring each patient receives compassionate, high-quality end-of-life care while maintaining performance metrics that meet the standards of Loma Linda University Hospice’s joint venture partnership. The DON is accountable for clinical outcomes, workforce utilization, and compliance, driving a metrics-based culture where decisions are informed by data, not intuition.
Key Responsibilities
Clinical Leadership & Team Oversight
- Lead and coach an interdisciplinary team (clinical managers, RNs, LVNs, aides, therapists)
- Ensure high-quality hospice care aligned with regulatory and organizational standards
- Establish and enforce clinical workflows, standards, and expectations

Required Qualifications
- Active Registered Nurse (RN) license in state(s) of operation; BSN required, MSN preferred.

- Proven success managing teams of 20+ clinical staff and patient populations exceeding 100 active census.
- Demonstrated ability to use EMR systems and analytics tools to drive data-based performance improvement.
- Strong knowledge of hospice regulations, quality reporting, and compliance frameworks.
- Excellent communication, leadership, and conflict-resolution skills.
