The administrative component of this job will consist of internal case management, completing patient intake and discharge to home, our start of care process and nurse competency validation. The field-based aspect of this position will create opportunities for you to spend quality time with field staff in patient homes developing, mentoring and providing professional guidance.  

Work Location: Plainville, CT location, in-home visits and Potential Hybrid (varies based on branch need)    

Compensation: $77-83k base salary, plus bonus potential (up to 10% of salary based on branch performance), on call pay, cell phone and mileage reimbursement.   Up to 80 hours of vacation accrual in the first year, 48 hours of sick time accrual, 1 8-hour fun day and 1 8-hour inclusion day. Vacation hours accrual will increase to 120 hours of vacation after 1st year.

Responsibilities of the Nursing Supervisor   

  • Internal case management, evaluation, education and development of nursing talent   
  • Participating in the transition of our patients from the facility to their home through “Start of Care Assessments” and “Re-evaluations”   
  • Review physicians’ orders and update the plans of care and documentation, as necessary   
  • Assist with selecting, training and validation of new clinical staff   
  • Promote and enhance the level of clinical expertise of staff to provide safe and quality nursing care for our patients   
  • Collaborating with the operations and business development team   
  • Ensure adherence to State, Federal, Local regulations and Aveanna policies and procedures   

Qualifications of the Nursing Supervisor:   

  • Must have good standing license (RN) in the state in which the clinician will practice   
  • Must have at least 3 years of hands-on nursing experience in a hospital or home care setting
  •  Must be able to travel to patients’ homes in designated territory   
  • Must have reliable transportation, valid driver’s license, and pass MVR check   
  • Current CPR certification from AHA or ARC 
  • Must be willing to supervise caregivers providing care to infants, children, adolescents and/or adults

The Physical Therapist under the supervision of the Center Manager, provides physical therapy services to program participants. Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting. Integrates the physical therapy treatment plan into the Interdisciplinary Team plan of care. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.


 


Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.   • Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a Physical Therapist in state where practicing
  • Minimum of one-year experience working with a frail or elderly population required.
  • Must clear the background check.
  • Must have a valid driver’s license and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

 


Essentials: 



  • Conducts an in-person initial comprehensive physical therapy assessment on all participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person physical therapy re-assessment minimally every six (6) months or as needed.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Provides treatment to participants and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
  • Evaluates participant’s needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
  • Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.
  • Assists as liaison with Senior Community Care nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.

Care Manager and Guardian Position

Job Summary
Kinsman Care Management, LLC is hiring a full-time care manager. We are looking for a person who is caring, good with details and follow-through, loves older adults, capable of directing family and other professionals, and has good interpersonal skills.
Desired Qualifications:
1. Five years of experience in the senior care industry. At minimum, two to five years of care management or care coordination experience
2. Knowledge of the senior care industry and mental health services: Senior benefit systems, local resources, housing options, dementia, behavioral health, and other age-related conditions
3. Skills to assess level of care for seniors and make recommendations
4. Bachelor's level of education or higher in human services related field. May be substituted for work experience
5. Ability to work independently and manage multiple clients effectively
6. Ability to follow-through on seeing that client needs are met; i.e., holding providers accountable and closing the communication loop
7. Promptness in returning calls and emails
8. Ability to track billable time and document visits—we have software and systems to help w/ this
9. Ability to use care management software, word and Excel, email/scan documents
10. Openness to learn about guardianship and work as a team
Job Duties:
Provide care management and guardianship services for adults with cognitive, mental health, and physical impairments. Use our proprietary systems to assess client needs, develop plan of care, implement and monitor services received. Includes home visits, transport clients to appointments, attend medical appointments, arrange for services for clients, supervise home care, attend court hearings, communicate with family and collateral contacts, write monthly summaries, maintain client file and online documentation, placement assistance, emergency visits.
Other:
Must have a valid driver's license and own transportation
Physical Demands:
Must be able to lift, carry, or move equipment, including wheelchairs, as needed for client care.
Work Environment:
Work is primarily off-site with travel to clients' homes, facilities or medical appointments. The schedule will be business hours with rotating on-call weekends and evening hours.

Salary Range: Annually 65K to 69K plus benefits.
