MDS Coordinator
We are seeking a detail-oriented, organized individual to fill the position of MDS Coordinator with one of our long-term care facilities. The ideal candidate will be a LVN and two to three years of experience in the medical field. They will work closely with medical providers to maintain medical records, including medication, allergies and physical limitations. The MDS Coordinator is also responsible for tracking the progress of patients and communicating with medical members to make necessary adjustments.
Responsibilities:
- Maintain a patient master list of information.
- Help families and physicians with family-determined care instructions.
- Review and update patient records based on historical data and new information.
- Communicate with patients, their families and/or physicians, agents, or other healthcare providers.
- Maintain a designated area for the placement of medical supplies; track and record their movement and use.
