Job Description Summary:
This is a position is with our Behavioral Health Integration program within OPG (OhioHealth Physician Group). This position will be located in Primary Care practices and will work collaboratively with medical providers and a Psychiatric Consultant. Applicant can be LISW or LPCC.This position is responsible for providing behavioral health services to patients, including, as appropriate, completion of brief biopsychosocial assessments, formulation of treatment plans and brief treatment interventions, in order to support and/or enhance biopsychosocial functioning. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, other and members of the care team). The clinician can practice independently and without supervision as permitted by the State of Ohio licensure.

Responsibilities And Duties:
30% Advanced Clinical AssessmentConducts patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret information pertaining to patient/client biopsychosocial functioning. Assessment to include, but not be limited to: developmental and age-specific needs; ability to care for self; ability to care for dependents; ability to meet basic custodial and financial needs; ability to engage in meaningful social and emotional relationships; and ability to comprehend and comply with treatment recommendations. Develops care plan, including discharge plan, incorporating medical plan of care. Participates in identification of cases of abuse, neglect or exploitation and refers such cases to the legally mandated authorities when applicable.Β Provides crisis intervention, information and referral and linkage to community resources as needed.

25% Advanced Population Management
Staff is accountable for coordinating clinical and financial outcomes of an assigned population.Β Works collaboratively with interdisciplinary staff and external organizations to achieve timely, cost effective management of patient care.Β Monitors for quality and type of services delivered to patients, ensuring management within established parameters. Functions as a liaison to the payer to manage the post-discharge financial resources and ensure expected treatment process, progress and discharge plan. Monitors and ensures completeness of all assigned patients in areas of treatment plan and timely reports.Β Β Help patients coordinate care for the highest quality of life, while managing to the lowest cost.Β Working with families to ensure access to knowledge and resources.

Job Description Summary:
Competently interpret the patientsβ history and baseline level of function and then complete an assessment appropriate to the patientsβ area of deficits. Based on problems identified, the occupational therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. After provider referral, evaluates patient motor, cognitive, sensory integrative and/or psychosocial function, develops and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment.
Implements plan of care based on OT practice framework and appropriate occupational performance frame of reference(s): biomechanical, sensorimotor, and/or rehabilitative.
Carries out plan of care with reassessment and changes to plan of care as appropriate.
Responsible for maintaining/meeting the productivity expectation of their department.
Plans and helps coordinates discharge plan in collaboration with the multidisciplinary team:
Instructs patient and caregiver/family.
Recommends DME required for discharge.
Provides information to physicians, nurses, case managers, social workers, and other team members.
Involved in the multidisciplinary patient plan of care and discharge readiness plan.

OhioHealth Doctors Hospital is a progressive facility with the one of the largest DO Medical Training facilities in the area. The pharmacy practice model is mixed with a high degree of clinical services and desire for growth. The scope of the position is focused on clinical pharmacy services, inclusive of the operations to support it, oversight of the infusion center and relationship with the oncology service line, interface with provider leadership, seated representative to system Pharmacy and Therapeutics Committee and leading local formulary efforts, engaged with the PGY1 Pharmacy Residency program, and matrixed relationship with system clinical services and quality. The position has approximately 30 direct reports (~22.5 FTEs) comprised of acute care clinical pharmacists, pharmacy interns, infusion center pharmacists and technicians, and medication reconciliation technicians.Responsibilities And Duties:

This position is responsible for improving disease management in our OhioHealth Physician Group β Primary Care, CIN, and Direct to Employer programs. The pharmacist will collaborate with primary care providers to provide disease state management programs to primary care patients; currently working under a consult agreement for the management of patients with diabetes and hypertension. This position supports HEDIS quality metrics and care gap closure while also delivering asthma and diabetes management services to our employee sponsored health and wellness patients.

Job Description Summary:
To competently interpret the patient's history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. The RN delegates nursing tasks to licensed practical nurses (LPNs) and unlicensed assistive personnel (UAPs) using the criteria written in the Ohio Nurse Practice Act.
According to department policies and nursing care standards, provides professional nursing care services to patients.
Responsible for assessing, planning, implementing, and evaluating nursing care provided to assigned patients.
Responsibilities And Duties:
Assessment/Diagnosis - Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual (30%).Outcomes Identification/Planning - Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs (30%).
Implementation/Evaluation - Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data (20%).
Leadership - Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance (10%).
Operations (10%).

Job Description Summary:
This position ensures delivery of evidence-based practice by professional nursing personnel and other staff in designated areas of responsibility. The clinical nurse manager plans, organizes, directs and evaluates the unit's delivery of evidence-based patient care in a cost-effective manner, providing leadership and clinical management to members of the health care team. The manager participates in integration of the Nursing Philosophy along with the mission, vision, values, goals and objectives of OhioHealth in unit operations.Responsibilities And Duties:
50% Patient Care:1. Assists the Manager in accountability for ongoing delivery of patient care in area(s) of responsibility; assures application of the nursing process by Registered Nurses in the clinical setting (assessment, planning, implementation and evaluation); assures documentation of patient care in the medical record.Β Addresses concerns and resolves problems.Β Uses data from various sources to initiate continuous quality improvement within the department/unit.Β Coordinates nursing care in collaboration with other healthcare disciplines and assists in integrating services across the continuum of health care.Β Ensures nursing practice in a safe environment.Β Participates in process improvement activities and root cause analysis investigations.Β Assists the Manager with fiscal responsibility at the unit level.Β
2. Assists Manager with planning, assessing, implementing and evaluating patient care as appropriate to department/unit.
3. Assists Manager with planning, reviewing and coordinating staffing time schedules and allocating staff as appropriate for volume and patient care needs.Β Assists Manager in daily staffing plans.
4. Assists Manager to coordinate nursing care with other health care disciplines across the continuum of health care.
25% Operations and Personnel Management:
1. Maintains daily unit operations including the status of staffing, patient visits and/or admissions, discharges and transfers, serving as a resource to department/unit staff to guide patient care delivery.
2. Participates in recruitment, selection, retention and evaluation of personnel.Β Participates in staff performance via written performance appraisals and disciplinary procedures.Β Ensures appropriate orientation, training, competence, continuing education, and professional growth and development of personnel.Β Maintains staff records.
3. Assists manager in planning and contributing to fiscal management of unit by utilizing human and material resources and supplies in an efficient, cost-effective manner.Β Assists Manager in development and implementation of services.
15% Professional Development and Leadership:
1. Practices as colleague with medical staff, other members of the interdisciplinary team, and other disciplines to initiate and support collaborative and cooperative clinical management practices.Β Actively participates in interdepartmental relationship building.

3. Participates in collection, analysis and use of data for quality and process improvement activities at the unit level.
