Basic Qualifications

Primary Level Salary Range: $122,800.00 - $184,200.00The above salary range represents a general guideline; however, Northrop Grumman considers a number of factors when determining base salary offers such as the scope and responsibilities of the position and the candidate's experience, education, skills and current market conditions.Depending on the position, employees may be eligible for overtime, shift differential, and a discretionary bonus in addition to base pay. Job Summary:
This is a liaison position requiring clinical experience, self-direction, attention to detail and follow-up, and ability to communicate effectively with staff, patients, families and the public. In conjunction with physicians and members of the Sleep Center Teams, the Sleep Navigator is responsible for guidance and coordination of services for sleep disordered screening patients and their caregivers through the continuum of care.
Core Responsibilities and Essential Functions:
Patient Coordinator and liaison * Receive incoming referrals from others. * Help coordinate and schedule patients for appropriate clinical follow up. * Coordinate patient follow through and provide daily status updates to WellStar Sleep Medicine and the WellStar Sleep Centers Ex. Director * Work with Program/Unit Manager, and others to address Program expectations related to patient care outcomes. * Provide clinical guidance and administrative support to others on team when needed. * Work with referring physician /office staff to ensure patients have completed all diagnostic tests and results are available. * Observe in other related units as clinically warranted or requested by patient or physician. * Contact patients for missed appointments. * Serve as a liaison between patient and physician team for information related to results. Position Summary
Under general administrative direction, the Gastroenterology (GI) Nurse Navigator coordinates and manages treatment plans for patients with gastrointestinal conditions. This role provides a significant level of patient education related to illness management and treatment, facilitates communication among multidisciplinary teams, ensures continuity of care, and optimizes patient outcomes.
The GI Nurse Navigator supports patient care coordination, assists with clinical follow-up, orders necessary labs and tests, and addresses barriers to effective care delivery.
Essential Duties and Responsibilities
- Serve as a liaison between patients, GI providers, and healthcare teams to ensure seamless coordination of care.
- Address barriers to care, including scheduling challenges, insurance concerns, and patient education needs.
- Educate patients on gastrointestinal conditions, treatment plans, medication adherence, dietary considerations, and follow-up care.
- Follow up on diagnostic tests and communicate results and next steps to patients.
- Provide clinical support, counseling, and education; escalate urgent clinical situations to providers as needed.
- Collaborate with the GI team to develop and implement care plans focused on improving patient outcomes.
- Ensure thorough documentation of patient encounters, laboratory results, and treatment plans in the electronic medical record (EMR).
- Work with the multidisciplinary team to reduce readmissions and address chronic disease management and social or environmental factors impacting patient care.
- Assist GI providers in addressing any gaps in care by coordinating follow-ups, procedures, and referrals.
- 5% of responsibilities will involve direct clinical duties, including ordering labs, reviewing patient records, and supporting procedures as needed.
Minimum Qualifications
Education:
- Bachelor’s degree in Nursing (BSN) required.
- Current D.C. licensure as a Registered Nurse (or evidence of application for licensure in D.C.).
Experience:
- Minimum of 2 years of clinical nursing experience, preferably in gastroenterology, internal medicine, or patient education settings.
Physical Requirements:
- Ability to walk, stand, and reach above shoulder level frequently in a clinical setting.
- Must be able to occasionally lift or carry, and frequently push or pull up to 100 lbs.
- Regular exposure to healthcare environments requiring personal protective equipment.

Community Health Worker certification preferred. Knowledge and experience working with programs of similar scope and focus preferred. Spanish fluency is preferred.
As part of the Baby Bexar Healthy Families Community-Based Perinatal Health Initiative, this position will serve as liaison between health system providers and community based resources in order to improve maternal health outcomes for University Health patients. This position will work with a clinical care team and a team of social service agencies to assist and guide patients through prenatal, delivery and postpartum services. This position will provide screening services, education, support, and patient care coordination. Candidate will ensure appropriate information is disseminated, patient is navigated to medical appointments, and follow-up with health care needs. All functions and behaviors are applicable to the clinical and community settings. Additional activities will include patient tracking, data entry, and reporting.
EDUCATION/EXPERIENCE
Bachelor’s degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered in lieu of the Bachelor’s degree. Community Health Worker certification preferred. Knowledge and experience working with programs of similar scope and focus preferred. Spanish fluency is preferred.
