Host Home Providers who have NO small children living in the home
We are in need of a couple or a dual-provider home with no pets or small children to provide Host Home services to a Tier 7 individual. Higher negotiated rates of pay are available.
Parker Personal Care Homes (PPCH) is looking for dedicated, honest, and responsible individuals to provide compassionate care and support to adults with Intellectual or Developmental Disabilities in your home and as a part of your family.
We look for folks who can offer services and supports that might include personal care, problem solving, safety in the community, learning new skills, increasing levels of independence, managing challenging behaviors and/or medical needs.
You must be willing to provide a home that is safe and supportive while assisting with activities of daily living, recreation and socialization, health and safety concerns and medical appointments. We provide all of the necessary training for our providers to be successful, including training in the areas of developmental disabilities, CPR, First Aid, medication administration, behavioral safety training, members' rights, confidentiality, and more.
PPCH is seeking Host Home Providers with NO pets or small children living in the home. PPCH needs providers who are willing to provide services to members with specialized behavioral needs. Must be willing or able to work with an individual that may engage in property damage. Provider will be compensated within the daily rate for the possibility of damages within the home.

Who lives in Host Homes?
PPCH offers host home settings for individuals requesting to reside in a home with their provider. Individuals who require verbal and/or physical assistance with daily needs often consider a host home setting.
What is a Host Home setting like?
PPCH has numerous host homes located throughout the metro area. Each host home is unique in its area of specialty, general layout and physical location.
What are the responsibilities of a Host Home Provider?
The Host Home Provider's main focus is to provide a safe and supportive environment for the member(s) in services in their home. Each Host Home Provider is responsible for ensuring the health and safety of the individual living in their home, while ensuring they will have plenty of opportunities for socialization and access to community resources.
What type of training does a Host Home Provider receive?
PPCH provides specialized trainings to all providers. These trainings include CPR/First Aid, Medication Administration, Customer Rights & many other areas. All host homes are monitored closely by PPCH to ensure that the member in services is receiving the highest level of care.
Qualifications
Must be able to provide services and support for 1 member in your own home on an on-going basis
Must successfully complete PPCH’s required training courses
Must be able to chart individual daily member activities and medications in a web-based system (training is provided)
Have professionalism and compassion
Able to focus on individualized care and support
Ability to teach others
Have good written and verbal communication skills
Have a valid driver's license and reliable transportation
Must clear background screening and reference checks
Be proficient with Microsoft Office, Google Office Suite, or similar software
Experience with intellectual or developmental disabilities is preferred, but not required

Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is dearly in the best interests of the patient.
Appropriately communicate to ensure adherence to professional standards in the provision of and availability of supplies, materials and equipment needed to safely and effectively implement the plan of care.

Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflects current plan of care. TB Statement:
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement:
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. At Enhabit Home Health & Hospice, an SLP may often work with patients who have experienced a stroke, and those with progressive diseases such as Parkinson's disease and dementia.

Home Health with Less Travel!
Our Physical Therapists (PT) supervise, evaluate, and provide therapy services to clients in various senior living communities, including assisted living and independent living facilities.
Our facility-based PTs enjoy:
* providing therapy services to our patients within the beautiful confines of the assisted living facilities where our patients reside

Our talented and highly skilled Physical Therapists work collaboratively with each assisted living facility's wellness team on a daily basis to maintain strong lines of communication and overall continuity of care for the residents and their loved ones. Our Occupational Therapists help patients who have debilitating conditions improve the functions of performing everyday tasks in the home. Occupational therapists use treatments to develop the daily living skills of their patients, the basic motor functions of patients as well as to compensate for any loss of function that may have occurred with the patient.

You’ll be driving to client’s homes, so a valid driver’s license and a dependable vehicle are required.

- Nashua – weekday and weekend; morning and afternoon
- Exeter / Kingston / Kensington – weekday and weekend, morning and afternoon; Monday/Friday evening
- Hampton / Rye – weekday and weekend; morning, afternoon, and evening
- Portsmouth / Newington – weekday and weekday, morning, afternoon, and evening; Saturday overnight
- Newmarket / Durham – Monday/Wednesday/Friday afternoon; Monday/Wednesday early morning; Tuesday/Friday late morning
- Salem / Windham / Derry – weekday and weekend, morning and afternoon; Saturday evening
- Auburn – weekday early morning
- Merrimak / Bedford – weekday and weekend; early morning, afternoon, and evening
- Milford – weekday morning and afternoon; Saturday early morning
- Brookline – weekday late morning and afternoon
- Rochester – weekday and weekend, early morning and afternoon
- Dover – weekday and weekend; morning, afternoon, and overnight
- Westford / Dunstable – Monday/Friday early morning; Monday/Wednesday late morning; weekend and Saturday afternoon; Tuesday evening
- Acton – weekday morning; weekend late morning; Monday/Wednesday afternoon
- Ayer – weekday evening; weekend afternoon
Pay & Benefits
- $19–$21/hr + $2/hr for weekend shifts
- Opportunity to earn up to $24.00/hr after your first 8 weeks*
- Weekly pay with direct deposit

*To be considered for the wage increase, you must meet Honor's requirements as a driver (valid U.S. drivers' license and clean driving record), have more than 6 months of paid/professional caregiving experience, work 20 visits in your first 8 weeks of employment, and remain in good standing with Honor. The $24.00 is for select regions and applies to weekend shifts
