Supports the Hospice Associate Director of Patient Services by providing leadership and mentoring guidance to the nursing staff on a day-to-day basis to ensure the provision of Hospice and Palliative Care services are consistent with the philosophy, goals, and policies of the program and MJHS. Schedules staff assignments in conjunction with the Assoc. DPS in accordance to the (NHPCO) and MJH staffing ratios. Empowers RN Case Managers through mentoring excellence in pain and symptom management along with focusing on patient and family goals of care. Monitors and evaluates the quality and completion of nursing documentation according to the documentation policy and procedure and state and federal regulations. Makes community presentations in conjunction with Admissions/Outreach Departments.

• Graduate from an accredited School of Nursing, BSN or BS degree preferred.
• Minimum of five years of Hospice nursing experience.
• Supervisory or leadership experience preferred.
• New York State RN licensure required.
• National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtains certification within one year of hire.
• Word, Outlook, and Windows experience required.
• Ability to effectively supervise and mentor Hospice nursing staff.
• Ability to work effectively within an interdisciplinary team environment.
• Coverage at our Garden City, Long Island office will be required.

• Oversee and manage clinical care and emergency responses

Provides supervision during a specific shift to all Nursing Staff to assure quality of care
• Monitor productivity throughout the year and participate in annual budget preparation
• Evaluate new admissions and ensure company compliance
• Ensure for the control of medication as per policy and procedure
• Ensure appropriate staffing and resources to support departmental needs
• Interview and participate in the selection process for qualified staff
• Review and perform staff allocations for all nursing personnel on assigned units
• Manage performance of direct and indirect reports
• Participate in new hire orientation, coaching, training and ongoing development
• Communicate resident clinical events or changes
• Respond to any patient, resident family concerns, issues or complaints
• Perform unit rounds at the beginning and end of shifts
• Investigate and resolve any issues identified and notify appropriate management as needed
• Gather necessary documentation for NYS DOH and internal investigations

Care Management Supervisor of Quality Review & Staff Education is responsible for quality assurance, accuracy, and overall integrity of the care management records completed by Care Management staff. This role is to ensure compliance with NYS DOH and CMS regulations through development of auditing tools and data validation. This Supervisor will analyze collected audit data, identify trends for staff re-training, and implement corrective action plans in collaboration with Clinical Management and Staff Education. They will oversee and conduct orientation, training, and education to all members of the Care Management team. Provides support to Director and Managers of Coordinated Care to ensure that all reporting requirements are prepared, submitted, and maintained in a professional and well-coordinated manner.

• Baccalaureate Nursing Degree from an NLN-Accredited School of Nursing
• Experience and knowledge of Managed Care: A minimum of two years nursing experience in Community Health or related field and/or minimum of two years of progressive job-related experience, including care management and coordination, education and supervision
• Demonstrates strong critical-thinking, problem-solving skills, and knowledge of Medicare and Medicaid
• Effective communication skills both written and oral
• Possesses strong critical thinking skills and knowledge of Medicare and Medicaid regulations
• Excellent analytical skills, interpretation of data
• Ability to set priorities and to handle multiple assignments
• Working knowledge of audit techniques and methodologies
• Secures relevant information to identify potential problems and makes recommendations for appropriate solutions
• Work effectively within interdisciplinary team environment
