You will provide psychosocial support and other Social Work services to Hospice patients and their families in the inpatient, home care/nursing home settings, in pre and post bereavement phases of Hospice service
You will serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff
You will develop social work plan of care, goals and makes recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
You will develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
Master Degree from the an accredited school of social work.
Minimum of one year experience in Social Work in a healthcare setting.

Supports the Hospice Associate Director of Patient Services by providing leadership and mentoring guidance to the nursing staff on a day-to-day basis to ensure the provision of Hospice and Palliative Care services are consistent with the philosophy, goals, and policies of the program and MJHS. Schedules staff assignments in conjunction with the Assoc. DPS in accordance to the (NHPCO) and MJH staffing ratios. Empowers RN Case Managers through mentoring excellence in pain and symptom management along with focusing on patient and family goals of care. Monitors and evaluates the quality and completion of nursing documentation according to the documentation policy and procedure and state and federal regulations. Makes community presentations in conjunction with Admissions/Outreach Departments.

Care Management Supervisor of Quality Review & Staff Education is responsible for quality assurance, accuracy, and overall integrity of the care management records completed by Care Management staff. This role is to ensure compliance with NYS DOH and CMS regulations through development of auditing tools and data validation. Responds to changes in members’ condition/sentinel events (i.e. level of care changes, changes in family support, changes in housing, etc.) by revising the PCSP and communicating changes with interdisciplinary care team and PCP.
Responds to requests for service within Medicare/Medicaid regulated time frames utilizing appropriate correspondence as per DOH/CMS regulations and departmental workflows.

The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.
- Provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care
- Work in the community environment, and may, on occasion, provide consultation in the long-term care environment or hospital.

- Help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage for when young adolescents are incorporated into adult hospice teams
- In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
- Other activities related to quality improvement projects may also be required, as appropriate
