Overview: These RNs will be doing a mix of standard quality audits, complaint initiated investigations, and more. When they are onsite, the amount of time that they are at the location is dependent on the audit that is required. It is expected that Health Service Reviewers will be traveling onsite about 85% of the time.


These individuals will be traveling to IDD housing to do state required Recertification (must be done every 15 months) or investigating specific complaints (disease outbreak, falls, etc.).


Travel: 85% of this role is traveling to sites. It is more location based and they will be traveling to the counties that surround their location. Essential Duties

Building Inspector/Plans Examiner III
Leads and coordinates members of the development review and inspection teams to ensure a timely, predictable, comprehensive and accurate plan review and inspection process for any development proposal.

Reviews and inspects residential, commercial, industrial, and multi-family development proposals to ensure compliance with State and City codes/ordinances, engineering/architectural, and fire/life safety principles.

Depending on the Division's needs, performs construction plan review and site inspections:

  • Conducts Pre-Development, Plan Intake, and Pre-Construction meetings.
  • Takes the lead in coordinating plan reviews and inspections internally, with customers, and with other departments and agencies.

A Brief Overview:
The RN Transitional Care Navigator (Population Health) is responsible for the case management, care coordination management, and utilization management of his/her population of patients across multiple care levels and settings. Serves as a catalyst to promote patients understanding their diagnosis, treatment options, and available resources and ensure that they are connected with the optimal resources across the continuum of care. This role will coordinate and facilitate smooth and safe care transitions while ensuring quality cost-effective patient outcomes. Serves as a liaison between their patient population and all other providers. Will be responsible for key metrics of success, which include improving the overall cost of care, length of stay optimization, reduction in excess days, reduction in SNF utilization and improvement in SNF care transitions, reduction in 30-day readmission rate and ED utilization.

* Guides high-risk patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care. Eliminates barriers to patient's access to health care services and facilitates continuity of care/care coordination.
* Establishes and documents an individualized plan of care for assigned patients using evidence-based treatment guidelines considering the patients individual health goals with a focus on wellness, health management, disease prevention and chronic disease management.
* Partners with the healthcare team to ensure clinical decision-making, implementation of recommendations, and discharge planning are timely and appropriate.
* Performs daily coordination between multiple departments, multi-disciplinary team, medical clinics, and community outreach to gain knowledge of patient, assure patient safety, smooth transitions of care, and manage utilization and total cost of care.
* Acts as advisor/educator by partnering with social work in providing emotional support including goals of care and counseling. Provides and/or arranges clinical education including medication management, community resources, financial resources, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
* Facilitates appointments for appropriate consultations and support services within established protocols
* Completes Utilization Management for assigned patients.

Summary of Responsibilities

A Behavioral Health Practitioner (BHP) works under the direction and supervision of a Clinical Manager, providing community-based behavioral health services to individuals with psychiatric illness, substance use issues and/or co-occurring disorders. The schedule is flexible, as determined by the person served’ s needs and may include early morning, evening or weekend appointments. The position may involve services provided in the designated BHS clinic, driving to persons served homes or various locations within the community where services are provided (if services are offered on-site according to the program and funding source.) It may include telehealth services utilizing both audio and visual communication with individuals. BHPs provide individual, family and group counseling, crisis intervention, conflict resolution, initial assessments to determine level of care, treatment planning, case presentations, presenting psychoeducational information to individuals regarding diagnosis, symptoms, medications, relapse prevention and safety planning. The BHP provides services to both children and adults.
