Overview:
Under the general supervision of Therapy Leadership and according to departmental policies and procedures regarding referral, standards of treatment, performs objective assessments, as well as comprehensive individualized treatment programs for adult and geriatric patients. Responsible for direct supervision to patients, indirect supervision of rehabilitation technicians. General supervision of PT assistants in accordance with State Practice Acts. Qualifications:
- Education: Bachelor, Master of Science in Physical Therapy or Doctorate of Physical Therapy.
- Licensure: Current/Temporary license of Physical Therapy in state of practice.
- Certifications: BLS certified. Required certifications must be current before assigned to care for patients independently.
- Preferred Experience: Six months to one year experience is preferred for Rehabilitation Physical Therapist. Prefer experience with care for adult and geriatric patients.

Job Summary:
Clinical Dietitian - Kennestone - Part Time
Practices as a member of the interdisciplinary health care team responsible for the provision of medical nutrition therapy. Fosters continuity of the Nutrition Care Process across all phases of the continuum of care as recommended by the Academy of Nutrition and Dietetics. Performs nutrition assessment for patients identified to be at nutrition risk based on criteria outlined by the hospital and best practice standards. Calculates metabolic nutrient requirements according to the age and disease specific needs of patients. Performs nutrition focused physical exam to further evaluate risk of malnutrition as indicated. Develops a nutrition diagnosis as appropriate and consistent with the Nutrition Care Process. Creates and implements a nutrition intervention and care plan for all patients with a nutrition diagnosis. Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention. Provides nutrition education to patients, family, and/or members of the healthcare team on recommended nutrition intervention(s) to accomplish nutrition goals; which may include, but is not limited to any of the following topics: specialized diet, enteral/oral medical nutrition supplements, food selections, eating behaviors, and community nutrition resources. Registered Dietitian / Nutritionist (RD, RDN) through the Commission on Dietetics Registration (CDR) within 6 months of hire. Licensed Dietitian (LD) through Georgia licensing board obtained within 6 months of hire. AHA-BLS preferred.
Required Minimum Experience:

In this role, you will:
Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area
Primary coverage areas include but are not limited to: Springfield MA
Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
Execute a strong visit completion rate, pacing 35+ completed visits per month
Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m.
Other role expectations and requirements:
Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics

During our In-Home Health Evaluations (and other associated follow-up visits), you'll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.
The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.

Job Summary
The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. Ability to work effectively with professional and supportive personnel as a member of an interdisciplinary care team. Ability to communicate clearly and maintain confidentiality of sensitive information. Counsels residents, family members and/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans.
