Tri City Health Partnership (TCHP) is the only completely free medical and dental clinic in Kane County, Illinois. We provide high-quality, compassionate care to uninsured individuals who live or work in Central Kane County. Every patient is treated with dignity, respect, and kindness.

We are seeking a Registered Dental Hygienist who is passionate about patient care and community impact to join our small, mission-driven dental team. The ideal candidate will have a passion for helping people, excellent patient care skills, and is able to build a positive rapport with the patients we serve. Required Qualifications:

• Current dental hygiene license in Illinois

Preferred Qualifications:

• Bilingual Spanish and English as the majority of patients we see speak Spanish as their primary language. Spanish proficiency is a plus, but not required.

Salary and Benefits:

• Salary will be based on relevant experience and qualifications.
• This is an hourly position at $38-42 per hour.

Signify Health, part of CVS Health , is hiring part-time board-certified nurse practitioners to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

In this role, you will:

• Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area

• Primary coverage areas include but are not limited to: Springfield MA

• Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month

• Execute a strong visit completion rate, pacing 35+ completed visits per month

• Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m.

Other role expectations and requirements:

• Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)

• Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics

• Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)

• Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)

• Maintain all required supplies and associated diagnostic equipment related to DPS offerings

• Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)

• Maintain board certification

• Maintain multi-state licenses (preferred)

• Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)

• Be flexible to travel locally and within licensed states, as needed (up to 25%)

• Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required

• Accommodate field observations and/or ridealong experiences, as requested

During our In-Home Health Evaluations (and other associated follow-up visits), you'll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.

The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.

Do you have a passion for engaging with people and helping them on their journey to better health? Signify Health, part of CVS Health , is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

You'll play a critical role in building trusted relationships to make people healthier, helping bring flexible, comprehensive and personalized health evaluations right to people's front doors.

Job highlights

The visit, which lasts about 45 minutes, allows for time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You'll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people's current treatment regimen.

In this role, you will:

• Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes

• Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people's unique needs

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role

• Be flexible to travel locally and within licensed states, as needed

• Conduct virtual visits as needed

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)

A Note on Our Work Environment:

As an in-home clinician, your primary workplace is the private residence of health plan members. We are dedicated to meeting health plan members where they are, wherever they call home. These environments are diverse and may present a variety of conditions. Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.
