Seeking BE/BC Family Medicine or MedPed Physician or Advanced Practice Provider to join rural health clinic in coastal OR town, an hour west of Salem, that has been providing care to the community for almost 50 years.

Details:

* 1 other MD on staff
* Must be interested in seeing children and women's health cases, in addition to family med
* Outpatient only; there is a small hospital in town that has an ER, surgeon, radiology, etc. that patients are referred to.
* EMR: PracticeFusion
* Currently seeing 8-15 patients per day, 100 per week but always have more people wanting to be seen so you have plenty of opportunities for a higher level, if desired.
* Mostly adult/geriatric population, but do see children as well. There is no nursing home in town so someone that has a desire to help the geriatric population is highly desired.
* Dermatology and Hematology Oncology providers rent space to provide care 1-2 times per month
* Clinic is affiliated with area mental health providers, along with a social worker and therapist are available a couple days a week as well on-site
* Area does have a high population of Spanish speaking individuals, so if you speak Spanish you will have a high number of new patients in that population as well

Compensation & Benefits:

* This is a pay for service model. Salary is a very wide range - depends on how much work you want to do - the more patients you see the more you make.

Key Responsibilities


Clinical Care

  • Perform comprehensive history and physical examinations
  • Evaluate, diagnose, and manage acute and chronic conditions
  • Develop individualized treatment plans

  • Prescribe medications and therapies within scope of practice
  • Provide post-hospital follow-up care
  • Address cognitive, behavioral, and functional changes
  • Educate patients and families on treatment plans

Skilled Nursing & Regulatory

  • Complete admission assessments and required documentation
  • Conduct routine and medically necessary follow-up visits
  • Participate in interdisciplinary care plan meetings
  • Support initiatives to reduce hospital readmissions
  • Ensure compliance with CMS and state regulations

Qualifications

Required

  • Graduate of an accredited NP/ARNP program

  • Patient management includes the following:

  • In collaboration with the Regional Chief Clinical Officer (RCCO), provide bridging care in centers that are understaffed with providers or need clinical coverage by seeing patients in clinic or virtually, addressing workflow items and returning patient phone calls

  • Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education.

  • Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient's cultural background, level of understanding, personality and support systems. Serves as a patient advocate.

  • Patient management includes the following:

    • Communicating with patients virtually or in a clinic

    • Ordering and interpreting appropriate laboratory and diagnostic studies.

    • Ordering appropriate medication and treatments.

    • Referring patients for consultation when indicated

    • Documenting in-depth progress notes and summaries. Performing invasive procedures independently upon the completion of documented competency.

    • Participating in patient care rounds and conferences, communicating patient management strategies to members of the patient care team; collaborating with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs.

    • Recognizing situations that require the immediate attention of a physician and initiating life-saving procedures when necessary.

Starting Salary: $50,000 - $57,000 (Salary)

Join a service oriented, close-knit team providing Functional Family Therapy (FFT), a nationally recognized, evidence-based intervention to children and families in the KCMO area. The successful candidate for this position will receive training and certification in FFT, along with excellent clinical support.



The successful candidate for this position will work well independently, as the position is very autonomous with much flexibility in scheduling sessions. Caseloads are very manageable, as the FFT model supports caseloads of 10-14 families at a time.





WHAT YOU WILL DO:




  • Engages clients and families in a manner that demonstrates immediate responsiveness to family needs and maintains a strength-based relational focus. Persistent outreach efforts may be required.
  • Utilizes the CSS system to document services and to plan treatment according to the FFT model.
  • Utilize other systems required, such as MyAvatar, to manage confidential client information.
  • Monitors progress in treatment and adjusts the phase of treatment as indicated.
  • Demonstrates fidelity to the FFT model by use of the phase system

Responsibilities:

In a manner consistent and supportive of our values, the Primary Care Provider is responsible for delivering high quality health care within Fast Pace Health’s scope of services while achieving optimum patient satisfaction. The Primary Care Provider is responsible for preventative care for the patients as their first point of contact and takes continued responsibility for providing the patient’s ongoing comprehensive care, focusing on their long-term health and well-being. The Primary Care Provider must be able to work in a team-oriented environment, be flexible to adapt to new technologies and protocols in a quickly evolving practice setting. They must have the ability to respond quickly and accurately to changes in condition or response to treatment and is responsible for providing outstanding patient service within the clinic and through various virtual communication channels, while maintaining a compassionate and welcome atmosphere.

Primary Care Essential:

1. Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up.
2. Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
3. Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
4. Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.
5. Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
6. Prescribe medications, recommend treatments, and provide written home-going instructions.
7. Refer patients to specialists or other healthcare resources as needed.
8. Educate patients and their families on health maintenance and preventive care.
9. Promote patient engagement, health literacy, and shared decision-making.
10. Participate in quality improvement initiatives and value-based care programs.
11. Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships.

Essential Functions:

1. Ability to provide quality care meet patient volume goals, expectation of +18 daily on average.

2. Lead clinic staff, in behaviors, actions and attitude (e.g. X-Ray Technologists, Nurses, and Front Office Specialists) in delivering excellent patient care. Provide guidance as necessary to ensure quality professional services and excellent patient satisfaction.

3. Discuss and review patients’ medical history, symptoms, allergies, and current medications. Asking patients situation-specific questions to formulate accurate diagnoses in order to provide guidance as necessary to ensure quality professional services and excellent patient satisfaction.

4. Actively engages with clinical leadership, elevating to management where appropriate to ensure strong patient care and resolution of concerns to ensure adherence to our company values.

5. Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis.

6. Ability to work efficiently in a fast-paced autonomous environment.

7. Dedication to evidence-based medicine and staying current with medical advancements.

8. Implement clinical protocols as outlined CMO and Supervising Physicians.

9. Ability to perform responsibilities within standard NP protocols.

10. Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses.

11. Deliver excellent patient care through in-person diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information.

12. Ensure that the activities of the Primary Care Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues, including but not limited to HIPAA.

13. Responsible for learning the aspects of compliance in the company by completing all mandatory compliance training in order to meet and exceed our continued quality of care.

Experience Requirements and Preferences::

Basic Requirements:

Education:

- Master’s degree in Nursing (MSN) and/or Master’s degree in Physician Assistant Studies (MMS)

Experience:

- Nurse Practitioner (NP) / Physician Assistant (PA) with 1 year of dedicated experience in direct primary care settings as a provider.
- Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures.
- Understanding of HEDIS quality metrics relate to preventative screenings, vaccinations and chronic conditions.
- Required to be proficient in performing pediatric exams and see children aged 3 years and up.

Current License or Certification:

License and DEA must be active, in good standing, and verifiable with the proper regulatory agency; Primary Care Providers must be able to treat all ages and must meet any credentialing requirements needed; and DOT certification required to be obtained within 120 days of employment and maintained during employment.
