As the Senior Living Physical Therapist, you will provide age-appropriate services for adult to geriatric patients, including the following: comprehensive assessment, direct treatment to individuals and groups, supervision of treatment delivered by the PTA or tech, development of a home exercise program, patient/family instruction, participation in team and family conferences, evaluation and selection of adaptive equipment, participation in PI programs, and documentation of services according to department and facility standards. This position involves direct patient care.
Primary Responsibilities:
* Evaluates the patient to determine rehab potential, identifies needs, establishes an individualized treatment plan of care, and sets goals to be used in the team plan of care
* Sets initial STG's and LTG's
* Utilizes equipment in a proper and safe manner, maintains equipment inspection, maintenance and cleaning logs, reports broken or faulty equipment
* Maintains equipment logs for required equipment in the outpatient clinic
* Follows medical regimen developed for the patient. Is aware of and adheres to precautions (THR, TKR, Cardiac, Wound, etc.)
* Utilizes the following safe treatment techniques: transfers, positioning, feeding, splinting, paraffin bath, cryotherapy, ultrasound, electrical stimulation, etc
* Completes D/C summary on each patient on their final visit to outpatient PT

Required Qualifications:
* Current Physical Therapy licensure in state of practice

Pay Range
$77,400 - $171,000 annual total cash target pay
$44.65 - $98.65 per visit point
$37.21 - $82.21 hourly rate
Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. Join Pilgrim’s as a Live Haul Driver Supervisor, leading the catching operations team along with drivers and forklift operators. You’ll coach and support your team, drive continuous improvement, and ensure safe, efficient, and on‑time operations that keep our plant running smoothly. This role is key to maintaining excellence in safety, quality, training, retention, productivity, and overall departmental performance.
Essential Duties & Responsibilities
- Plans and directs daily, weekly, and monthly schedules for bird catching and transportation, ensuring a consistent supply of live poultry for plant processing.
- Supervises live haul truck drivers and coordinates with contracted catching crews to ensure timely delivery of birds to the plant.
- Ensures strict adherence to USDA guidelines, animal welfare standards, all safety program, and Department of Transportation (DOT) regulations.
- Manage departmental budgets and strategic objectives, including tracking, record-keeping, and reporting on financial variances.
- Communicate daily updates, progress, and issues to the processing plant and management.
What You’ll Need
- Education: High School Diploma/GED; College Degree in related field preferred.
- Experience: 1 or more years of previous supervisory experience with leadership skills of 20+ employees preferred. Poultry or food industry experience is a plus. Valid CDL preferred.
- Skills: Effective communication both orally and in writing, strong analytical and problem-solving skills, capable of independent decision-making, must have basic computer knowledge, and an ability to manage multiple priorities.

St. Paul’s Assisted Living is seeking a compassionate and skilled Qualified Medication Aide (QMA) to join our dedicated healthcare team. The QMA will play a critical role in providing exceptional care and support to our residents, ensuring their well-being and maintaining a safe and comfortable living environment.
Responsibilities
- Administer medications and treatments as prescribed by healthcare providers in compliance with state regulations.
- Monitor residents for changes in health, reporting observations to nursing staff or healthcare providers.
- Assist residents with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility as needed.
- Maintain accurate documentation of medication administration, treatments, and resident care activities.
- Adhere to infection control protocols and safety procedures to maintain a clean and healthy environment.
- Communicate effectively with residents, families, and team members to ensure coordinated care.
- Support the nursing staff in responding to emergencies and providing prompt care to residents.
- Foster a warm, respectful, and supportive atmosphere for residents and their families.
Qualifications
- Current certification as a Qualified Medication Aide (QMA) in the state of Indiana (required).
- Experience in a long-term care or assisted living setting is preferred.
- Knowledge of medication administration practices, resident care standards, and state regulations.
- Strong attention to detail and ability to maintain accurate records.
- Compassionate, patient, and empathetic demeanor with a genuine desire to serve the senior community.
- Excellent communication and teamwork skills.
- Ability to work flexible hours, including weekends and holidays, as needed.
