ABOUT THE ROLE
As an LVN or LPN at Belmont Village, you will administer medications, support wellness initiatives, help ensure regulatory compliance and assist with quality monitoring in our Assisted Living and Memory Care community. You will communicate with healthcare practitioners, ancillary providers, pharmacies, family members, and team members as needed ensuring timely clinical interventions and follow-up. You will lead and provide direction to Caregivers and Medication Technicians where applicable. The ideal candidate is reliable, takes pride in their work, and demonstrates empathy. YOUR TYPICAL RESPONSIBILITIES
Follow up on physicians orders, coordinate services with ancillary providers and report to physicians and families any changes of condition or other pertinent clinical care information
Provide medication administration to an assigned group of residents according to physicians orders and report any adverse reactions or clinical observations documenting as necessary
Ensure resident care and service plans are followed by overseeing tasks of Medication Technicians and care providers
Make appropriate situational decisions and manage conflict in a positive and constructive manner
Update and review resident medical records, clinical documentation, and incident reports
MINIMUM QUALIFICATIONS
Active LVN or LPN license in the State in which this community is located
CPR Certified or ability and willingness to obtain within 30 days of hire
One year of medication administration experience preferred
Must be able to communicate clearly in verbal and written English
Intermediate computer and mobile device skills
Professional, pleasant and team oriented attitude

- Evaluate residents’ functional, cognitive, and psychosocial needs using standardized assessments.

Summary:
The Registered Nurse Lactation Counselor provides expert lactation support and education to breastfeeding mothers and infants, utilizing the nursing process to assess, plan, implement, and evaluate individualized care. This role involves providing in-person consultations, telephone follow-ups, and outpatient services to address lactation challenges, enhance breastfeeding success, and ensure mothers and infants receive optimal care. The Lactation Counselor works closely with lactation consultants, physicians, nursing staff, and other healthcare providers to coordinate care, provide education, and ensure continuity of lactation support after discharge. The role also includes patient education, staff training, and promotion of lactation resources.
Responsibilities:

Patient Education and Counseling:
- Educate mothers and families about proper breastfeeding techniques, infant feeding cues, and the benefits of breastfeeding.
- Develop and implement personalized care plans that support breastfeeding success.
- Provide emotional support and encouragement to empower families to meet their breastfeeding goals.
- Document all interactions, assessments, and care provided in accordance with hospital policies and patient privacy regulations.
- Offer timely and responsive assistance to mothers requiring follow-up care, ensuring they feel supported in their breastfeeding journey.
Collaboration and Coordination of Care:
- Collaborate with physicians, nurses, and other healthcare providers to ensure seamless continuity of care for breastfeeding families.
- Communicate and coordinate effectively with hospital staff to ensure lactation needs are addressed during hospital stays and post-discharge.
- Follow up with patients to monitor progress, address any concerns, and adjust care plans as needed.
Outpatient Lactation Support:
- Provide individualized lactation support to mothers and infants through outpatient follow-up, including in-person visits and telephone consultations.
- Assess breastfeeding techniques, infant latch, milk supply, and overall breastfeeding goals.
- Address common lactation challenges, including sore nipples, low milk supply, latch issues, and breastfeeding pain, using evidence-based strategies.
- Provide guidance and support for mothers and infants experiencing more complex issues.
Training and Education for Staff:
- Assist in the training and education of hospital staff, including nurses and other healthcare providers, to ensure accurate and consistent lactation information is provided to patients.
- Promote best practices in lactation care across the hospital through in-service training and educational resources.
Program Development and Improvement:
- Contribute to the development and improvement of lactation services, ensuring they align with current evidence-based guidelines.
- Participate in quality improvement initiatives and audits to enhance patient outcomes and satisfaction.

Job Requirements:
Education/Skills
- Associate Degree in Nursing (ADN) required
- Bachelor's degree in nursing or related field preferred
Experience
- In hospital experience preferred
- Outpatient follow-up care experience preferred
- In-depth knowledge of lactation practices, breastfeeding management, and maternal-infant health required
- Must have strong clinical assessment and critical thinking skills to address patient needs effectively
- Excellent communication, counseling, and interpersonal skills required
- Must be able to work collaboratively in a multidisciplinary team and support diverse patient populations
- Strong organizational and documentation skills needed
Licenses, Registrations, or Certifications
- RN License in the state of employment or compact required
- IBCLC (International Board-Certified Lactation Consultant) certification required

Responsibilities:
1. Administers, monitors, and weans mechanical ventilation; makes ventilator recommendations based on clinical assessment, technical assessment, and laboratory values; manages airways.
2. Administers bronchial hygiene and treatment procedures including aerosol therapies, chest physical therapy, and therapeutic gas administration; analyzes gases where applicable; procures and delivers gases using central piping systems and gas cylinders.
3. Assesses patients clinically before and after the administration or application of any medication, procedure or device; recommends appropriate therapy including initial therapy, changes in therapy, and discontinuance of therapy.
4. Applies CPAP and BIPAP and makes recommendations based on patient compliance, response, and laboratory values.
5. Participates in CQI and QA initiatives including work audits, abstracts, research, Daily Management System (DMS) projects, committee participation, or other activities to improve the clinical and work environments.
6. Develops patient driven protocols in conjunction with department leadership. Documents work timely, completely, and accurately, including documentation in the patient medical record, documentation in the department records, and patient billing.
7. Assesses patient oxygenation, ventilation, and hemodynamic status through clinical observation, patient assessment, physiologic monitoring including oximetry, capnography, ventilator waveform analysis, and cardiac output, ECG, and laboratory values; provides respiratory care interventions within the scope of the Rapid Response Team, performs CPR.
8. Performs bedside spirometry including spontaneous breathing parameters, PEFR, and other limited pulmonary function; identifies normal values for patients.
9. Provides patient/family education.
10. Performs ECGs and arterial puncture.
11. Participates in on-going maintenance of the blood gas equipment, QC, calibration and proficiency testing.
12. Performs bronchoscopy assist.
13. Provides clinical supervision of respiratory care students on rotation.
14. Demonstrate initiative in optimizing patient care by participating in physician rounds and documenting assessment findings and therapy changes
