What you’ll do in this role:

- Hospice patient identification, follow-up, and management in the hospital
- Manage Hospice referrals for assigned area, which may come from several sources including but not limited to: Hospitals, Discharge Planners, Physicians
- Work closely with the insurance department
- Review charts to determine eligibility for program(s)
- Determine Hospice level of care needs for each referral
- Call/ conduct visits with patients/caregivers to explain and discuss Hospice services
- Process appropriate documents including physician orders/certification
- Communicate with Hospice Supervisor/Manager regarding status of referral to facilitate continued admission process and transition of care as needed by the patient

Qualifications:


Valid Class A CDL required. Applicants must be domiciled in the U.S. and licensed in their state of residence


Currently hold, or ability to obtain, a non-excepted interstate DOT medical card required


1 year of documented tractor-trailer experience in the last 3 years required, or 2 years of documented tractor-trailer experience in the last 5 years
