Full-Time Hospice Homecare/Case Manager RN (Albany/Schenectady area)

The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families in the west Albany and Schenectady county areas.  This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).

Monday - Friday 8a-4:30p

What you will do:

Work as part of our Hospice Homecare Team caring for families residing in the West Albany and Schenectady County region.

Responsibilities:

• Coordinate with an interdisciplinary team,  to develop a comprehensive plan of care to provide added support to patients and families in their homes. 
• Work closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).

What you will need:

• A current license to practice as a Registered Nurse in the State of New York
• Associates Degree in Nursing, BSN preferred
• 1 year experience as an RN required-preferably in Hospice/Home Care or acute care.
• Valid Driver's License and Reliable vehicle
• The RN must be able to communicate effectively, verbally and written
• High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
• Understanding of Hospice principles preferred

Pay Range: $79,040 ($38/hr)- $104,374 ($50.18/hr) Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location

WHAT YOU’LL BE DOING (and doing well!)

• Manage the service department of assigned Division.
• Supervise, train, and develop all branch associates, including technicians, service coordinators, billing coordinators, and dispatchers.
• Responsible for the scheduling, execution, billing and completion of service, warranty, and emergency jobs.
• Responsible for executing service inspections on time and resolution of system deficiencies.
• Responsible for developing a budget and meeting revenue and gross margin targets.
• Responsible for delivering projects within the original budgeted cost.
• Responsible for executing monthly financial performance analysis. Reports the information in an effective manner to management and takes corrective action as needed.
• Responsible for efficient asset management, such as inventory and company service vehicles.
• Responsible for building a high-performance culture to include annual performance reviews and development initiatives.
• Responsible for manpower planning and allocation.
• Responsible in part for customer satisfaction and cash collections.
• Works closely with the sales and installation teams to support the growth and profitability of the Division.
• Responsible for control and calibration of inspection, measuring, and testing equipment.

WHAT WE LIKE ABOUT YOU

• Two to five years of experience in a service operations manager role within the fire detection, fire protection, security, or electrical environment.
• Ability to effectively communicate to a diverse group of individuals, including company leadership, regional staff, customers, and vendors.
• Strong, positive team builder with leadership ability.
• Knowledge of current fire alarm, clean agent/special hazard fire suppression, security, and electrical systems.
• Strong understanding of Profit and Loss statements and key financial drivers.
• Ability to attract, develop, grow, and retain a team.
• Ability to understand the technical aspects of the product lines and applications as necessary to make informed management decisions.
• Valid driver’s license.
• Must be able to pass a background check and drug screening.
• Able to work independently.
• Excellent organizational, decision-making, and communication skills.
• Proficient in NFPA codes and standards.
• Strong computer skills; proficient at Microsoft Office.
• Knowledge of OSHA safety standards.
• NICET Level II.

Pay Rate: $75,000- $100,000 annually depending on experience.

The position also supports a digital platform designed to facilitate charitable giving. The team works to enhance the client experience by helping manage grant activity, supporting operational processes, and contributing to the development of tools that allow clients to efficiently direct charitable contributions.

Responsibilities:

• Review and perform due diligence on charitable grant recommendations to ensure compliance with applicable regulations and eligibility requirements.
• Research nonprofit organizations using public and proprietary databases to verify charitable status, financial standing, and organizational documentation.
• Identify potential compliance risks, red flags, or conflicts of interest and escalate complex issues when necessary.
• Communicate with nonprofit organizations and internal stakeholders to gather documentation and clarify information related to grant requests.
• Maintain accurate records of due diligence activities, findings, and communications within internal systems and reporting tools.
• Prepare summaries and reports outlining research findings and compliance reviews.
• Support regulatory and compliance requirements related to charitable giving programs.
• Follow established policies, procedures, and internal controls while contributing to improvements in operational processes.

Requirements:

• Bachelor’s degree required.
• 1–3 years of professional experience, preferably within nonprofit organizations, donor-advised funds, financial services, compliance, or operations.
• Strong research and analytical skills with high attention to detail.
• Excellent written and verbal communication skills.
• Ability to manage multiple tasks in a fast-paced environment.
