Job Description
ESSENTIAL FUNCTIONS
- Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson. Integrates Evidence-Based Practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal healthcare.
- Partners with patients, their family/designated care partner(s)) in identifying learning needs and priorities and provides education in ways that are useful, understandable, and affirming. Minimizes risk of harm to patients and others by following established nursing practices and protocols and individual performance.
- Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision making. Uses data to monitor the outcomes of care processes and uses improvement methods to design and test changes to continuously improve the quality and safety of patient care.
- Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care. Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.

Education - Required
- Specialized Diploma Graduate of an accredited school of nursing required
Education - Preferred
- Bachelor's Degree BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred

- 1 year Previous experience as an RN in an acute care setting strongly preferred

- Discharge Planning Knowledge of the patient's post-discharge needs and the ability to partner with the patient and family/designated care partner(s) in developing a discharge plan and arranging for follow-up care.
- Assessment and Diagnosis Knowledge of various tools and techniques for patient assessment and nursing diagnosis or clinical judgement, ability to explore and analyze reasons behind symptoms and generate resolutions according to available drugs, facilities and techniques.
- Health Information Documentation Knowledge of the relevance of health information and appropriate documentation methods, ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form.

Summary
- The Patient Services Representative III Floater PSR III represents the face of our company to patients who come in, both as part of their health routine or for insights into life defining health decisions.
- The PSR III draws quality blood samples from patients and prepares those specimens for lab testing while following established practices and procedures.
- The PSR III has direct contact with patients and creates an atmosphere of trust and confidence while explaining procedures to patients and drawing blood specimens in a skillful, safe and accurate manner.
- The PSR III will demonstrate Leadership Behaviors while focusing on process excellence skills and sensitivity to confidentiality and accuracy to patient information.
- Successful applicants may be assigned to a doctors office, a patient service center or as business needs dictate.
- Under the direction of the area supervisor, perform daily activities accurately and on time.
- Maintain a safe and professional environment.
- Performs with confidence, both the forensic and clinical specimen collection and processing duties following established practices and procedures.
- Perform verification of patient demographic info initials including patient signature post venipuncture to verify tubes were labeled in their presence and that the name on the label is correct.
- Maintains required records and documentation.

Job Requirements
- Ability to provide quality, error free work in a fast paced environment. Ability to work independently with minimal onsite supervision.
- Excellent phlebotomy skills to include pediatric and geriatric.
- Flexible and available based on staffing needs, which includes weekends, holidays, on call and overtime.

Required Education
- High school diploma or equivalent.

Work Experience
1. Five years phlebotomy experience required, inclusive of pediatric, geriatric and capillary collections.
2. Minimum 3 years in a PSC/IOP environment preferred.
3. Customer service in a retail or service environment preferred.
4. Keyboard/data entry experience.

It enhances our clinical excellence with advanced, easy-to-use remote monitoring technology and 24/7 access to our Virtual Care Center RNs.
Home Care RN position summary
Provides primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.

Graduate of an approved nursing education program

One (1) year experience as a professional care nurse

This position works three 12 hour shifts/paid 40 hours.
This position will work weekends and either Friday or Monday and be responsible for 4 start of care visits each day.
