Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health ( ), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.
We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.
Position Summary
Under supervision of the Medical Director, the PACE Physician provides assessment, planning, primary care, and continuous medical coverage to PACE participants in an interdisciplinary team environment and in accordance with Gary and Mary West PACE standards.
Essential Functions:
- Provide complex primary care and acute illness evaluation and treatment for participants on an as needed basis via in person, at home or telehealth visits.
- Perform in-person comprehensive assessments on participants and coordinate with the IDT to develop a comprehensive care plan for assigned participant panel of up to 110 participants.
- Be an active member of the IDT communicating participant changes, collaborating on care planning decisions and patient centered care in line with West PACE mission.
- Work collaboratively with case management RN's and other members of IDT to provide clinical input and care coordination in Transitions of Care scenarios and participate in facility and family care conferences as needed.
- Conduct post hospital and post ED discharge visits within 1 week of discharge as well as acute visits focused on ED and hospital utilization reduction for high risk patients.
- Provide clinical guidance and support to Nurse Practitioners and Physician Assistants. Supervising NP's and PA's may be required.
- Along with the Medical director, serve as a liaison outpatient and inpatient specialist to communicate and improve care for PACE participants.
- Participate in every 5-6 week rotating telephonic only nights and weekends.
- Provide home or SNF visits as needed based on clinical criteria and/or by Gary and Mary West PACE's standards and participants needs.
- At the direction of the Medical Director, review outcomes, coding and utilization data, including review of necessity of hospitalizations and use of specialists; establish best practices with the Medical Director, and collaborate with clinical staff, Community-Based PACE Physicians, and IDT to make policy and procedural improvements to reduce unnecessary utilization and improve participant health.
- At the direction of the Medical Director, special projects may be assigned to help improve care and experiences at West PACE.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.
- Follow Gary and Mary West PACE policies and procedures and participate in any required QI activities, staff training and meetings.
- Maintain professional licensure and certifications.
- Act only within the scope of practice.
Education and Certification:
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- MD or DO degree.
- Board Certification – Internal Medicine or Family Practice.
- Additional certifications in Geriatrics, Hospice and Palliative Care, Addiction Medicine or Pain is desirable but not required.
- Active CA physician license without restrictions.

Position Summary:
Under the supervision of the Center Director, the Dietitian displays a comprehensive knowledge of clinical nutrition and therapeutic diets; completes participant nutritional assessments and reassessments as needed; obtains participant food preferences and observes for adherence to dietary modifications; determines adequacy of current diet and supplements when necessary; conducts meal rounds and sanitation inspections; provides nutrition education to participants, caregivers, and PACE program staff.
Essential Functions:
- Participate as a member of the interdisciplinary team (IDT); perform initial, unscheduled, and annual assessments of participants; and develops and implements the nutritional plan of care, including physician orders for special diets, for each participant.
- Participate in the development and implementation of QAPI activities.
- Responsible for oversight of dietary aide and kitchen operations.
- Monitor therapeutic diets and special dietary modifications and communicate concerns with interdisciplinary team; determine each participant's need for liquid nutritional supplements; and order as needed.
- Monitor participant food intake; follow up with dietary aide, interdisciplinary team, and participants with comments/concerns.
- Assess participants' access to food and make appropriate referrals to interdisciplinary team for: meal preparation evaluation, Meals on Wheels, sack lunches, grocery shopping needs, and meal preparation provided.
- Communicate with in-home service providers regarding participants' meal preparation skills, safety in the kitchen, stock of groceries, and intake of food and fluids at home as needed.
- Monitor adequacy of lunch and snack menus for the center.
- Update sack lunch list, supplement sheet, and feeding room guide as needed.
- Monitor stock in refrigerator, freezer, and dry storage as needed.
- Communicate regularly with vendors.
- Monitor dietary expenditures, complete budget reports, anticipate projected budgetary needs, and discuss with supervisor.
- Maintain safe and sanitary working environment; and follow PACE program safety policies and procedures; handle potentially infectious specimens with appropriate biohazard precautions and practice Universal Precautions.
- Provide basic nutrition and sanitation education for participants, families, primary caregivers, and/or PACE program staff as needed.
- Participate in wound care meetings with updates on participants' nutritional status; family meetings; and work area development and department meetings.
- Responsible for staff hiring, orientation, training, and development, and performance appraisal; set clear expectations and assign appropriate authority to staff; hold staff accountable for their scope of work and compliance to company policies and procedures.
- Interact in a courteous, pleasant, and friendly manner with participants and families, PACE program staff, and staff at contracted facilities, as applicable; and demonstrate effective partnership management skills.
- Maintain the confidentiality of all company procedures, results, and information about participants or families.
- Attend and participate in all staff meetings, in-services, projects, and committees as assigned.
- Adhere to and support the center's policies, practices, and procedures.
- Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.

Education and Certification:

This is a key role that will directly support the financial health of the organization by managing collections, ensuring accurate cash application, and coordinating with both internal teams and external customers.
You will also play an important leadership role by managing a team of 3-4 offshore AR clerks and partnering closely with the accounting team to maintain accurate, timely financial data.

REQUIREMENT: You must be able to work on-site 3 days per week in Solana Beach, CA.
What You’ll Do ● Own AR Operations: Monitor overdue receivables, customer payment trends, and dispute resolution across thousands of monthly invoices in multiple currencies.
● Cash Application Oversight: Ensure accurate and timely application of 100+ daily customer payments, partnering with offshore staff as needed.
● Team Leadership: Manage and support a team of 3–4 offshore AR clerks, providing guidance, problem-solving, and accountability.
● Customer & Supplier Communication: Serve as the primary point of contact for AR-related questions, discrepancies, and payment issues.
● Reconciliations: Perform monthly AR reconciliations and assist with general accounting tasks as needed.
● Cross-Functional Collaboration: Work closely with Accounting, Sales, and Operations to maintain data accuracy and streamline AR workflows.
● Process Improvement: Help refine AR processes, reporting, and internal controls in a growing, evolving environment.

ICU Veterinary Technician
Specialty & Post-Operative Critical Care
Are you an experienced Veterinary Technician who thrives in a structured ICU environment and enjoys managing complex specialty cases? Join Veterinary Specialty Hospital North County and work alongside board-certified Surgeons, Neurologists, Internists, Cardiologists, Oncologists, Radiologists, Ophthalmologists, and Criticalists in a collaborative, high-acuity setting.
Our ICU is focused on advanced post-operative and medically intensive patients, not general kennel-style monitoring. This is a high-responsibility role requiring strong technical skills, critical thinking, and confidence managing complex cases, especially during overnight shifts.
Schedule
- Full-time
- 3 x 12-hour shifts per week
- Daytime and Overnight ICU opportunities available
- Overnight shift differential applied per hour
Compensation
- RVT Level 2 & 3: $32–$42/hour based on experience and demonstrated ICU competency
- VTS: $38–$45/hour

About Our ICU
Our ICU primarily manages:
- Neurosurgical recoveries
- Advanced orthopedic and soft tissue post-operative cases
- Internal medicine critical patients
- Escalated ER transfers requiring specialty oversight
This is a structured ICU environment with strong interdepartmental communication and clear expectations. Technicians are trusted to function at a high level and are supported by board-certified Criticalists and specialty teams.
What You’ll Do
- Independently manage post-surgical and specialty ICU patients, including overnight care
- Monitor advanced pain protocols and CRIs
- Support anesthetic recovery for high-acuity cases
- Administer blood products and manage neurologic patients
- Communicate directly with Criticalists and specialty teams
- Provide clear, compassionate client communication during critical hospitalizations
- Maintain detailed and accurate medical documentation
Preferred Experience
Minimum of 2 years ER or specialty ICU experience
Demonstrated competency in:
- CRI setup and management
- Advanced anesthetic recovery monitoring
- Blood product administration
- Neurologic patient monitoring
- Independent clinical decision-making within scope
