Housing and Homelessness Services

Housing & Homelessness Services works to build a robust, integrated, and coordinated system for housing and homelessness services and acts as the County's point of contact on homelessness strategic planning and program development. Formerly the Office of Homeless Care and Coordination (OHCC), Housing & Homelessness Services (H&H) works to improve health and housing outcomes among people experiencing homelessness.

H&H operates within Alameda County Health and alongside other County agencies and departments, as well as cities, community-based organizations, and other Continuum of Care partners. Housing & Homelessness Services oversees Coordinated Entry and System Access services, the Homeless Management Information System (HMIS), and works with 50+ providers across more than 145 contracts to provide comprehensive crisis response/diversion, interim, and permanent housing services throughout the County's homelessness response system. POSITION

Under direction, to provide program planning, technical assistance, review and evaluation functions to direct client service delivery programs where such services are provided by community-based organizations (CBO's), contract service providers and/or County staff in a wide variety of service areas; to act as County liaison with service providers and funding sources and ensure that program regulations and procedures are followed; and to do related work as required.

DISTINGUISHING FEATURES

This professional-level class provides technical programmatic services in a wide variety of client service areas; however, all positions are typically characterized by the following elements:

· The need for technical knowledge in the service delivery area to which assigned;

· The fact that client services are provided by CBO's, other contract providers and/or County staff; and

· The focus of responsibilities being related to program development and implementation, rather than to the provision of administrative or support to County departments.

EXAMPLES OF DUTIES

NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.

1. Develops and modifies techniques and formats to evaluate pilot or current program effectiveness and to determine the need for program modifications and/or new program development.

2. Researches program alternatives, funding sources, service delivery organizations and other elements for possible program inclusion; evaluates alternatives, prepares reports and makes recommendations.

3. Acts as the County liaison and provides coordination among community-based organizations and other service providers, County departments, State or other funding sources and community or business organizations; interacts with various planning councils or boards; answers questions and provides training and technical assistance as required.

4. Monitors assigned program or programs; ensures compliance with contract provisions and funding source regulations; recommends and facilitates implementation of procedural and operational changes to maximize service delivery and revenue reimbursement.

5. Compiles and maintains accurate records and files regarding program activities; prepares periodic and special statistical or narrative reports as required; may access multiple databases to prepare such reports.

6. Negotiates contracts with service providers; ensures that contracts are renewed in a timely manner; prepares, distributes and follows up on contract documentation.

7. Participates in departmental, Countywide and/or State planning processes; serves on a variety of committees and task forces.

8. Analyzes changes in regulations; evaluates the impact upon program operations and drafts policy and procedural changes as required.

9. Prepares a variety of correspondence, periodic and special reports, informational publications, program documentation, policies, procedures and other written materials.

10. Operates a variety of standard office equipment including a word processor and/or computer; may drive a county or personal vehicle to make site visits and attend meetings.

PROGRAM SPECIALIST

QUALIFICATIONS

Education:

The equivalent to graduation from an accredited four year college or university (180 quarter units or 120 semester units) with major coursework in business or public administration, a social science or a field related to the program area to which assigned.

(Additional experience as outlined below may be substituted for the education on a year-for-year basis.)

AND

Experience:

The equivalent to three years of full-time professional-level or supervisory work in the direct delivery of services to clients or the oversight of such services in the program area to which assigned, one year of which must have included program planning and evaluation, or in program administration, contract negotiations, grants management and similar financial services.

Substitution:

(Possession of a Master's degree in business, public administration, social science or a related field to the program area to which assigned from an accredited college or university, may be substituted for two years of the required experience.)

Licenses:

How You Will Contribute:

The Therapist is responsible for providing individual and group therapy sessions for clients in a specialized Outpatient facility. Position is responsible for assessing the therapeutic needs of the individuals and families, and for developing appropriate treatment. Therapists act as the primary contact for clients and families throughout the treatment process and serve as a client advocate with the treatment team. Position acts as a liaison with referral sources, clinical partners, and individuals to coordinate care throughout the treatment process.


Essential Responsibilities:

  • Demonstrates a high level of clinical skill in assessing the clinical needs of clients, determining appropriate interventions, and conducting individual and group therapy sessions in accordance with these needs.
  • Completes psychosocial evaluations, treatment plans, contact logs, discharge plans and other reports required for each client in compliance with State, accrediting bodies, quality guidelines and payor source guidelines.
  • Conducts weekly individual and family therapy sessions for assigned caseload.
  • Monitors behavioral interventions for each client and provides skills coaching as needed.
  • Conducts specialized groups as assigned based on education, training, and experience; supervises or co-leads groups with staff or interns when appropriate.
  • Selects appropriate interventions to meet the clinical needs of individual clients.
  • Maintains therapeutic relationship with clients using praise and other reinforcers to encourage appropriate behaviors and attitudes.
  • Sets limits and hold boundaries in a kind and firm manner
  • Demonstrates an understanding of identifying abuse/neglect in client population.
  • Demonstrates effective verbal crisis intervention and de-escalation, involving other staff as needed.
  • Collaborates with an interdisciplinary team to coordinate client care from admission through discharge.
  • Attends weekly clinical, consultation and business meetings; present cases to be discussed as pre-assigned.
  • Completes documentation in a timely and accurate manner reflecting ongoing assessment of client’s clinical presentation and behaviors.
  • Communicates client progress with the referral source, family, parent, or guardian informed of treatment goals and plans, as appropriate.
  • Documents in accordance with policies and procedures services provided, incident reports, shift reports, or department reports in a timely and accurate manner.
  • Communicates with referents, residents, and families appropriately and effectively.
  • Uses the referral process to build relationships with new referents and engages in collaboration with referents.
  • Addresses residents, visitors, physicians, and co-workers in a pleasant and respectful manner.
  • Responds to clients and families with empathy and positive professional skills.


Qualifications

What We Are Seeking

Position requires a master’s degree in social work, marriage and family therapy or psychology. Must hold CA licensure (LCSW, LPC, LMFT, or LMHC), active associate number or intern number from the Board of Behavioral Sciences, in good standing. Position requires a minimum of one (1) year experience working with clients with substance use disorders and/or borderline personality disorder. Certification or significant experience in providing Dialectical Behavioral Therapy (DBT), strongly preferred.


Other Requirements

  • Clearance of TB test, fingerprinting and state clearance, and any other mandatory state/federal requirements.
  • Current CPR and First Aid certification.

What You'll Do

The individual in this position has overall responsibility for hospital utilization management, transition management and operational management of the Case Management Department in order to promote effective utilization of hospital resources, timely and accurate revenue cycle processes, denial prevention, safe and timely patient throughput, and compliance with all state and federal regulations related to case management services.

This position integrates national standards for case management scope of services including:

Department Operations
/n • Maintains an adequate number and skill mix over seven days a week to serve the patient population and meet the goals of the department
/n • Implements and supports with business case staffing requests utilizing the Tenet Case Management staffing recommendations and hospital budgetary guidelines
/n • Holds regular departmental meetings with staff to provide updates and provides for ongoing education
/n • Completes initial and annual competency and evaluation review on all case management staff
/n • Follows the InterQual Inter-rater Reliability (IRR) Policy to determine initial and yearly competency for all employees performing InterQual reviews
/n • Develops action plan for case managers that fail to meet the IRR acceptable “match” rate to ensure improvement in the accurate application of InterQual criteria
/n • Ensures new case management staff complete department orientation including review of Tenet Case Management and Compliance policies and Allscripts training

/n • Participates in Revenue Cycle meeting, researching disputes, uncovering patterns/trends and educating hospital and medical staff on actionable items
