Job Description

  • Join an established and multicultural diverse interdisciplinary team at the Charlotte Hungerford Center for Youth and Families – a Nationally Accredited Child Advocacy Center - that provides a safe and therapeutic healing environment for children, teens, and their families.
  • The children served in Child First range in age from prenatal up to 6 years of age.
  • The Center for Youth and Families supports clinicians work by providing, weekly individual and team clinical supervision, weekly clinical group supervision to review cases and interact with colleagues, professional development opportunities that include training in the Child First model, Child-Parent Psychotherapy, and additional clinical topics, committed focus on employee wellbeing, work-life balance, and professional growth, and regular engagement with Department Medical Director & Administrator.
  • The Child First Clinician engages with families and the Child First Care Coordinator in the collaborative assessment process.
  • The clinician uses all available information to develop a thoughtful, well-integrated clinical formulation and Child and Family Plan of Care in partnership with the Care Coordinator and family.
  • Provides Child First home-based psychotherapeutic intervention with young children and their caregivers using relational, dyadic psychotherapy (CPP) and other modalities.

Program Summary
In the Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client's treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Nuvance Health, formerly known as Western Connecticut Health, has as excellent opportunity for a full-time, employed Hematology/Oncology Nurse Practitioner (NP)/Physician Assistant (PA) in Danbury, Connecticut. The position follows a Monday through Friday schedule with mostly outpatient coverage. No evenings, weekends, or call.
The ideal candidate will be a board certified Nurse Practitioner/Physician Assistant with previous Hematology/Oncology experience and looking to join a dedicated and collegial team. This position requires a minimum formal education of Nurse Practitioner/Physician Assistant training, State of CT license, and Federal DEA and CT Controlled Substance Registration. We offer a highly competitive compensation package, as well as allowances for CMEs and Dues.
Our long established and busy practice includes seven physicians and four Advanced Practitioners. Our office is based in Praxair Cancer Center within the Danbury Hospital Campus. We offer multidisciplinary cancer care, collaborating with Surgical and Medical Subspecialties, Radiation Oncology, and Palliative Care. Additionally, we offer Diagnostic Imaging, Cancer Genetics, Radiation Therapy, Cancer Research and Trials and Psychological and Social Support Services. Our employed practice sees a broad spectrum of Hematology/Oncology patients in the office and hospital.
Established in 1885, Danbury Hospital is a not-for-profit community hospital providing Greater Danbury area residents convenient access to exceptional care. Danbury Hospital has 371 beds and is a Level II Trauma Center with approximately 70,000 ED visits per year. Program Summary
Natchaug's Joshua Center Programs treat children as young as six and as old as 17 who are struggling with psychiatric or substance abuse issues, but do not require inpatient care. Services Include: Comprehensive assessment, which may include: Comprehensive aftercare planning, family, group, and individual therapy, occupational and creative rehabilitation therapy. Natchaug Hospital provides transportation from school to the Joshua Center Program, and then to the student's home.

Job Summary

Lead Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Child/Adolescent partial hospitalization program. Clinical services include individual, group and family therapy. Maintains ethical principles and professional standards of practice in adherence to the discipline's professional code of ethics. Assumes leadership responsibilities as assigned by clinical leadership. May assume clinical oversight of daily operations in the absence of Program Manager.

Key responsibilities of Lead Behavioral Health Clinician:

  • Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
  • Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.
  • Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
  • Assists in the coordination of programming which may include staff scheduling, scheduling clinical activities, program development, utilization review, case load assignment, and attending meetings as directed by manager.
  • May assist in the hiring process for new staff, supporting expansion of initiatives, supporting clinician productivity, leading team meetings, conducting staff education and training.
  • Independently able to provide clinical social work interventions to all patient populations.
  • Provide social work field supervision to second year social work students.

***Lead Behavioral Health Clinicians are eligible for a Sign On Bonus up to $2,500.00, in accordance with program guidelines***

Qualifications

  • Master's degree in social work, counseling, marriage and family therapy or related Human Service field required.
  • Minimum of one year of experience as a licensed Clinician working with behavioral health and/or chemical dependency population.
  • Experience in leading clinical care teams preferred.
  • Current Connecticut license; LCSW, LPC, LMFT, or PsyD.
  • Excellent communication skills, with the ability to establish relationships within and outside their entity.
