General Role Description: The Home Health Patient Care Manager/RN is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.

Primary Responsibilities:

* Receives referrals and ensures appropriate clinician and/or therapist(s) assignments for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visits.

* Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.

* Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician orders.

* Manages and documents phone calls and new orders from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians.

* Uses coordination notes to document, as needed and appropriate.

* Receives report from weekend and after-hours clinicians admitting new patients.

* Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.

* Follows-up on lab and other clinical diagnostic test , physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such clinicians.

* Schedules, prepares for, facilitates, and documents case conference/SOC reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary.

* Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician.

* Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs.

* Follows-up with On-Call events daily

* Assures payer change documentation is completed properly and timely, and required. Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff.

* Takes necessary action to correct adverse findings and communicates trending to clinical director .

* Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences.

* Enters infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy.

* Assists in the orientation of new agency personnel and provides direction and leadership to clinical team members in collaboration with the clinical director.

* Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care, and in coordination with other members of the health care team.

* Consistently meets expected productivity at 50% of full time RN level as defined in the Visit Productivity Point Policy.

* Accurately and timely completes the comprehensive assessments (OASIS) including medication reconciliation. Makes the initial and/or comprehensive nursing evaluation visit, ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source, accurately determines primary focus of care, develops the plan of care within State specific guidelines with the physician, and submits accurate documentation.

* Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, provides instruction as appropriate, and assigns tasks according to State and federal regulations.

* Provides required supervisory visits.

* Initiates, develops, implements, and makes necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care.

* Communicates relevant information timely and effectively with appropriate agency staff including but not limited to: any patient care issues or needs, visit assignments, dates of scheduled visits, and schedule changes to scheduler, orders and OASIS data sets, coding requests, schedule home visits, to coordinate care with other clinicians.

* Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence.

Required Qualifications:

* Current RN licensure in state of Oregon

* Current CPR certification required

* Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. Ascension Providence is looking for a Physical Medicine and Rehabilitation physician to serve as the Medical Director for our new inpatient rehabilitation unit. Ideally, this physician also could establish an outpatient clinic practice and see pain management patients as well.

This is a great opportunity for the right candidate to partner with Ascension Providence to provide medical services to patients in Waco, Texas. Waco has been recognized as the Heart of Texas," and is situated on the I-35 corridor between Dallas and Austin with scores of shops and restaurants galore along with excellent parks, a supportive business environment, a low crime rate, diverse housing options, and top-ranked schools.

Responsibilities:
Serves as the Medical Director for inpatient rehabilitation services. Leads interdisciplinary team conferences.
Monitors ongoing health and progress of patients, and changing treatment plans as necessary.
Evaluates and treats patients with appropriate medical diagnostic and treatment skills. Coordinates discharge planning with case management.
Recommends, participates in, and works to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.
Accepts patient referrals from other physicians. Would also prefer candidates that are open to integrate and round on the medical/surgical acute floors to consult on potential rehabilitation admits. Model established allows for PM&R physician to design practice style as admitting, attending and/or consulting physicians for patients who require hospitalization for primary medical diagnoses, depending on needs of patients, medical staff and hospital. Have an existing medical staff comfortable with attending and medically managing patients, allowing PM&R to not have call coverage expectations, manage PM&R directed goals and gains, and run weekly case conferences.
Provides appropriate documentation that meets insurance company requirements; appeal all denials in a timely manner.
Participates in inpatient and outpatient consultation for specialty, which may include on call schedules.
Provides outpatient clinic follow up, and pain management services for patients seeking treatment.

How you'll make an impact in this role

Ascension Seton is looking for a Board Certified/Board Eligible Physical Medicine & Rehabilitation provider with Board Certification/Board Eligibility for Pain Management to join the Seton Brain and Spine Institute. A non-ACGME accredited spine fellowship may also be considered though there will also be an expectation for medication management as well as interventional management.

Position Highlights

:
Outpatient responsibilities include clinic evaluation and management of PM&R and pain management patients in the north Austin market
Rotation will be between Georgetown/Temple & Round Rock/Williamson MOB clinics
Primarily non-operative management supporting surgical specialties (i.e. orthopaedic surgery, neurosurgery) as well as Ascension providers as a whole
Multi-modal pain management (i.e. medication management, including opioids if clinically indicated and interventional management)
Access to in-office ultrasound and fluoroscopy
Spasticity Management optional
EMG/NCS optional
Resident/Fellow clinical supervision/education encouraged
Affiliated with the Georgetown Surgery Center where there may be potential for investment
Coverage with another Board-Certified PM&R & Pain Management physician
Closely affiliated with local Ascension Seton hospitals (Williamson)
Ascension Seton Williamson recently broke ground on an expansion that will add 216,000 square feet to the existing facility along with a new six-story tower that will feature 160 beds upon completion, along with two new operating rooms, an additional MRI, a new cath lab, an observation unit, and an expanded emergency room.

Job Description:

Salary Range: $73,394.83 - $112,102.00 depending on experience

We’re hiring Paramedics that are passionate about delivering compassionate, high-quality service and basic, as well as advanced, patient care to our customers.

RESPONSIBILITIES

- Assess each call situation to determine the best course of action while working with progressive Paramedic protocols.
- Utilize your Paramedic skills on medical equipment and procedures including defibrillator, EKG monitor, oxygen and suction devices, and intravenous fluids to provide advanced medical care.
- Communicate with patients and loved ones to provide information and assurance that care is being given.
- Act as Paramedic team leader and take responsibility for the scene and unit management as needed.
- Drive the ambulance on 911 responses.
- Work collaboratively and in a professional manner with all allied health and public safety personnel as well as your fellow Paramedics.

OTHER RESPONSIBILITIES

- Participate in community programs to maintain AMR image and establish strong community relations.

MINIMUM QUALIFICATIONS

- High school diploma or equivalent (GED)
- State Paramedic License
- State Driver’s License
- BLS, ACLS, NREMT-Paramedic
- Driving record in compliance with company policy
