About the job:
The Registered Nurse (RN), within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Staff RN is accountable for the delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of Mount Carmel Health System.
What you will do:
- Assess and evaluate patient care for an assigned group of patients, utilizing evidence-based practice; Assists provider with patient assessment, examinations and treatments; Perform basic medical procedures under the supervision of a provider, including set up of procedures.
- Administer medications under the direction of the provider, according to the organization’s standards, policies, procedures, and/or guidelines.
- Assist in emergency situations under the direction of the provider
- Thoroughly educate and explain procedures to patients and family members (diagnosis, disease process etc.).
- Document accurately and completely pertinent assessments, interventions and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.
- Assume responsibility and accountability for care provided and documented by other licensed and unlicensed care givers as appropriate.
What we are looking for:
- Graduate of an accredited school of nursing
- Current license to practice as a Registered Nurse in the State of Ohio
- Current BLS/CPR
- Demonstrated ability to plan, organize, and manage patient care, including delegation to and supervision of other members of the patient care team
- Basic computer skills required; prior experience using an EMR for charting preferred

System Transfer Center RN, Nights
*7:00 pm - 7:00 am weekdays and a weekend rotation.
Position Purpose:
In accordance with the Mission and Guiding Behaviors the registered nurse of the system transfer center should accurately listen and respond appropriately to requests made by transferring physicians, hospitals, and peers. To ensure patients are assigned rooms in an expedient manner, this position will utilize all critical resources within the organization to expedite this process. This position should give high priority to customer service and complete customer satisfaction. This position must ensure all paperwork and computer entry is checked for accuracy and continually enhances their knowledge regarding processes. This position will coordinate the successful transfer and placement of patients entering via the Transfer Center access. This position will effectively communicate with the surgery department, emergency department, nursing units, physician offices, bed schedulers, and outlying facilities to ensure appropriate placement of patients in a timely manner. This position must be able to adjust through a rapidly changing environment. This position will participate in personal and professional growth, development, and continuous improvement in all critical success factors of people, service, quality, financial, and growth. This position will be required to work weekends, holidays, and be on call as scheduled. Position Purpose:
In accordance with the Mission, Core Values and Vision; the Nurse Liaison Home Infusion will be a Registered Nurse responsible for assessing patients and their suitability for the Home Infusion program. This position will assume responsibility for verifying that appropriate pre-certification documentation is available to ensure reimbursement, quoting rates for therapy within established guidelines, and coordinating care with applicable agencies and the Mount Carmel Infusion Services pharmacy staff. This person will function with a high level of independent judgment, using skills and knowledge acquired through education and customer service expectations to enhance patient care. In addition, the nurse will contribute clinical expertise to organizational planning and decision-making and may also be involved in business development activities as they relate to business expansion or new program development.
What You Will Do:
Maintains daily communication with case managers/discharge planners and units to help ensure that patients referred for services are followed up with in a timely and efficient manner.
Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of Mount Carmel Infusion Services.
Provides and utilizes clinical expertise and skills to assist referral sources &/or patients with:
The identification of appropriate candidates for home infusion &/or home health care
The identification of the patients’ and/or caregivers ability to provide care
Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home.
Medication connection at the hospital / facility in order to expedite the discharge into the home setting, as appropriate
Coordinates and communicates all discharge planning activities for new patient referrals with Mount Carmel Infusion Services personnel and referral sources.
Appropriately documents the coordination of care activity including, but not limited to:
Home infusion &/or nursing services utilized
All communication with patients, referral sources, physicians and Mount Carmel Infusion Services personnel
All patient &/or caregiver education & training
Interacts with healthcare team members to answer any questions related to home infusion and community-based services as they plan for patient discharges from the acute care setting.
Functions as a liaison between nursing and other disciplines and departments in resolving patient issues.
Minimum Qualifications:
Education: Bachelor’s degree in nursing (BSN) or Registered Nurse (RN) from an accredited college or school of nursing with equivalent experience required
License / Certification: Current license to practice as a Registered Nurse in the State of Ohio required.
Current Certified Registered Nurse Infusion preferred
(CRNI) credential preferred or be willing to obtain within 6 months of hire
Must have valid and current Ohio Driver’s license and reliable transportation to and from work site
BLS Healthcare provider training required
Experience: Must have a minimum of two (2) years' experience in infusion care setting and experience with processing referrals for homecare and pre-certification.
