Our unit features a 1:5 nurse-to-patient ratio, ensuring that our nursing staff can provide personalized, high-quality care to each patient. While the majority of our patients are adults, we also occasionally care for pediatric patients, offering a diverse range of clinical experiences.

RESPONSIBILITIES:
Provides high-quality, patient-centered, clinical care
Performs initial and ongoing assessment of the patient, utilizing appropriate data (i.e. physical, psychological, educational and risk-screen data appropriate for the chronological / developmental age of the patient).
Communicates and documents assessment of patient with the interdisciplinary team. Utilizes all available resources in order to meet the needs of the patient and family.
Develops and documents plan of care, treatment and services which is individualized and appropriate for the patient needs, strengths, limitations and goals.
Complies with organizational policies, procedures and protocols including policies governing safety, medication administration and documentation.
Revises plan of care based on ongoing evaluations and updates documentation appropriately.
Completes at least one clinical research utilization (CRU) form for submission to Nursing Research Department utilizing a research article from the last 5 years from research performed in the United States.
ComplianceCompletes a minimum of 5 nationally approved contact hours (CNE or CEU) per year.
Completes mandatory in-services and health screens in timely manner.
Maintains Basic Life Support (BLS) certification.

Nurses on this unit are experienced in managing vasoactive intravenous medications, including Cardizem, Lasix, and other critical therapies for heart and stroke patients.

Job Summary: The COC Peer Coordinator is an active member in the FLACRA team and provides peer support services to clients/families with serious mental illness and or substance abuse.
Under the supervision of the Sr.
Director of Crisis Services and Mobile Clinic and support from the Housing and Employment Manager and Connections Regional Coordinator, the Peer Provider will function as a peer outreach worker by engaging with individuals who are homeless, on the coordinated entry list, and continuing engagement until housing placement is confirmed.
The Peer Provider will function as a role model to peers; exhibiting competency in personal recovery and use of coping skills; serve as a consumer advocate, providing consumer information and peer support for clients/families in outpatient and inpatient settings.
The Peer Provider performs a wide range of tasks to assist peers of all ages in regaining independence within the community and mastery over their own recovery process.
Recovery resources such as booklets, tapes, pamphlets and other written materials will be utilized by the Peer Provider in the provision of services.
The Peer Provider will also conduct street outreach to identify and engage individuals in unsheltered settings, providing immediate support, harm reduction resources, and connection to services.

Also looking for Youth and Family Peer Advocates Job Summary: FLACRA is seeking a compassionate and motivated Peer Advocate to join our team.
As a Peer Advocate, you will provide peer support services to individuals experiencing serious mental illness and/or substance use challenges.
Under the supervision of the Director of Housing and Vocational Rehabilitation Services, you will serve as a role model, using your lived experience in recovery to offer guidance, advocacy, and support to help clients navigate their own recovery journeys.
In this role, you will be an integral part of the client’s treatment team, helping to facilitate recovery plans and promote independence within the community.
You’ll also play a key role in engaging clients in early stages of treatment and assisting with transitions as they progress through their recovery.
Must-Have Qualifications: High School Diploma or GED equivalent.
Valid NYS Driver’s License with the ability to use a personal vehicle for client visits.
Lived experience in recovery or experience caring for/supporting someone facing mental health or substance use challenges.
Willingness to participate in peer certification training, with the goal of obtaining CRPA-P certification within 4 months of employment.
Nice-to-Have Qualifications: Certification as a Certified Peer Support Specialist (NYCPS) through OMH or OASAS.
Certified Peer Advocate (CRPA).
Key Responsibilities: Actively participate in client treatment teams, including team meetings and treatment planning.
Assist clients in developing recovery plans, including crisis plans.
Provide psycho-education focused on illness management and peer support through one-on-one sessions.
Share your own recovery experiences to serve as a role model, teaching coping techniques and self-help strategies.
Offer case management support by coordinating services, resourcing, and collaborating with external providers.
Maintain accurate and thorough records on all assigned clients, using documentation to track and support progress.
Advocate on behalf of clients, ensuring their needs are addressed within the treatment team.
Facilitate outreach to clients who are struggling to engage in treatment, using early engagement strategies.
Lead outreach efforts and provide support to peers in early stages of recovery and treatment engagement.
Support and mentor other peers within the agency, helping Tier 1 peers grow into leadership roles.
Participate in required trainings to maintain certifications and improve skills.

The CCBHC will deploy a high-quality, consistent set of protocols including screening, intake, outreach, navigation and coordination services that help connect clients in need to critical services.
Duties: · Services will be driven by the individualized goals of the client and the primary role of the associated projects is to support coordination of care with clients and service providers.
The methods used to accomplish this goal may include: o Engage with client currently in agency Residential programs and educate clients of services available through enrollment in CCBHC programs through group and individual sessions.
o Assist Residential teams in discharge planning and referrals to mental health, substance use and primary care appointments, through collaboration for clients soon to be engaging in Community based programs.
o Developing strategies with the client to improve attendance at mental health, substance use and primary care appointments, including attending initial appointments with the client o Developing and completing a thorough review of the service client’s existing treatment and crisis plans o Modeling strategies and behaviors that will promote successful engagement in the health care delivery system o Supporting coordination of care and referrals for clients with complex and/or emergent needs for support.
o Promoting enrollment into Health Home Care Management and/or Home and Community Based Services (HCBS) as eligible.
· The impact of such a role may include: o Keeping people out of hospital and emergency rooms by supporting well-care connections and utilization of services before needs are emergent.

Client Follow Up: · Introduce and meet with client and CCBHC Care Coordinator for initial appointment and assist for smooth transition primary assistance from Transitions Coordinator to Care Coordinator for on-going support.
· Provide phone and face-to-face follow-up to clients and service providers.
Support Practice Change and Education : · Provide feedback (both written and verbal) regarding what has been learned from the outreach and engagement process as requested.
