Responsibilities
The Ambulatory Float Licensed Practical Nurse (LPN) works in variety of dynamic specialty clinics and primary care including, but not limited to, the following locations: Baldwin, Gonda, Mayo Clinic, NE Clinic, NW Clinic, SE Clinic, and Kasson. Ambulatory floats provide direct patient care to support the unique needs of the clinical practice. The LPN role in the float practice consists of assisting with patient visits by providing support in a variety of ways including assisting with procedures, rooming patients, administering immunizations, among other skills related to direct patient care. The Ambulatory Float Pool supports 16 different LPN practices serving patients from pediatrics through adulthood. An individual LPN within the Ambulatory Float Pool will train and work in six to ten different practices, specialties, or locations based on FTE and practice mix. Hours are Monday through Friday, with varied start times from 7-8 am. The majority of the schedule is 7:45-4:45 based on unit need.
The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. Mayo Clinic's Division of General Internal Medicine in Rochester is one of the largest academic general internal medicine divisions in the world - serving as the gateway in evaluating and caring for adult patients that may have unexplained symptoms and/or complex medical conditions. Consultative Medicine Clinic (a section of GIM) provides consultative medical care to patients that reside outside of Olmsted county in Minnesota. Consultative Medicine sees new patients to Mayo Clinic for an initial comprehensive evaluation. The LPN role in Consultative Medicine includes, but is not limited to, patient visits (face to face and via telephone) review of patient health history, medication review, immunization administration, focused nutrition assessments, and blood pressure visits. The Cardiac Cath Lab Registered Nurse performs as an integral part of a multidisciplinary team providing care to patients during their journey through the Cardiac Catheterization Laboratory. Provides age specific care to patients ranging from infants to geriatric patients. Registered Nurses interact with patients in the outpatient, inpatient hospital and emergency settings. Per protocols, independently screen patients for various Cardiovascular, Cerebral vascular and Peripheral vascular diagnostic or interventional procedures. The RN Case Manager will function within the Mayo Clinical Nursing Professional Practice Model, which includes accountability for assessing, planning, implementing, evaluating, and communicating the patient care plan progression. The RN Case Manager utilizes the principles of mutual respect, patient/family advocacy and provides leadership within the team of internal partners and outside agencies to facilitate best practices that achieve quality clinical, financial, and patient satisfaction outcomes. The RN Case Manager provides leadership through education on case management/utilization management concepts, committee work, research, and community involvement. The RN Case Manager bridges communication between providers, patients and families, members of the interdisciplinary team, and outside agencies to assure high-quality care that meets the patient's needs and is delivered in a cost-effective and timely manner. The ANA Nursing: Scope and Standards of Practice and Code of Ethics provide a basis for the practice of the RN. The American Case Manager Association Standards of Practice and Scope of Services for Health Care Delivery System Case Management and Transitions of Care Professionals (2013) are reflected.

Qualifications
Graduate of an accredited, or those in the candidacy process, baccalaureate nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). One year of RN experience required.
Additional Qualifications:
Three years recent acute medical/surgical hospital nursing experience preferred. Must have in-depth understanding of anatomy, physiology medical terminology and disease processes. Knowledge of CPT coding, resource management systems, electronic medical record and personal computers beneficial. Demonstrated leadership, independent problem solving, conflict resolution, critical thinking skills, and team building skills required. Must be self-motivated and able to work as part of a team with minimal supervision. Must possess the ability to work well within a creative and challenging work environment. Experience in utilization review, ability to navigate medical records, value based purchasing metrics, or pre-certification helpful. Additional nursing experience in specialty area beneficial. Certification in Case Management (CCM or ACM-RN) preferred.

BLS (Basic Life Support) must be valid for 90 days post start date; acceptable certifications include: American Heart Association (AHA)/ Heartcode® BLS, Military Training Network, Red Cross (CPR/AED for Professional Rescuers and Health Care Providers).
