In this field position you will perform initial physical and psychosocial assessments of potential hospice patients in order to develop individualized plans of care, goals, and make recommendations based on each patient's needs. Assessments may take place in private homes, hospitals or nursing facilities. As a Hospice Field Admission Nurse (RN), you will admit eligible patients onto the Hospice program following the appropriate standards of practice established by the National Hospice and Palliative Care Organization (NHPCO).

We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
In this position, you will provide psychosocial support and Social Work services to Hospice patients and their family members who are at an inpatient unit, nursing home or home care setting
You will provide pre and post bereavement services
You will serve as a social work member of hospice IDG and as liaison between hospice, inpatient and nursing home staff
You will develop the social work plan of care, set goals and make recommendations based on the patient and family needs
You will maintain accurate and current documentation reflecting palliative social work assessments and
interventions
To take part in discharge planning when appropriate for patients admitted into inpatient facility
Master Degree from the an accredited school of social work
Minimum of one year experience in Social Work in a healthcare setting
Knowledge of Word and excel preferred
Licensed Master Social Worker (LMSW or LCSW) required in the state of New York
Valid Driver license and car maybe required based on job location

The NP Provider may work in a nursing home, hospital or community environment.
Specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and may work independently or as part of a treatment team.
Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family.
The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family.
Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation.
Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed.Implements specialist palliative care consultation services as a provider.Maintains effective communication with referral sources.Performs timely, accurate and compliant documentation.Identifies self- learning needs and outlines a plan for continuing education.Demonstrates a professional, courteous, and respectful attitude in dealing with referral sources, healthcare professionals, patients and family members.Performs other clinical or administrative tasks consistent with professional scope of practice.Performs other clinical or administrative tasks consistent with scope of practice from time to time, as requested by management.• Master's degree in Nursing and Graduate of an approved Nurse Practitioner Program required.
