About the Opportunity


An established PACE program in Georgia is seeking a compassionate and mission-driven Primary Care Physician to join its interdisciplinary team.


This role is focused on delivering comprehensive, coordinated care to an aging population, enabling patients to maintain independence and quality of life while remaining in their homes and communities.


PACE, Program of All Inclusive Care for the Elderly, is a nationally recognized model designed to support older adults with complex medical and social needs through an integrated care approach.


Position Overview


The PACE Physician provides direct primary care to program participants while collaborating closely with an interdisciplinary team to develop and implement individualized care plans. This role blends clinical care, care coordination, and leadership within a highly supportive, team-based environment.


Key Responsibilities


  • Provide comprehensive primary care to PACE participants, including initial assessments and ongoing reassessments
  • Collaborate with the interdisciplinary team to develop and manage individualized care plans
  • Manage chronic conditions and address acute medical needs
  • Oversee medication management and treatment plans
  • Coordinate care across settings including hospitals, specialists, and long term care facilities
  • Support care transitions and discharge planning
  • Monitor changes in participant health status and communicate updates to the care team
  • Maintain accurate and compliant documentation within the electronic health record
  • Educate participants, caregivers, and staff on health conditions and care plans
  • Participate in team meetings, quality initiatives, and on call rotation


Qualifications


  • MD or DO with active Georgia medical license
  • DEA registration required
  • At least one year of experience providing primary care to elderly, frail, or long term care populations
  • Strong clinical skills in chronic disease management and geriatric care
  • Ability to work effectively within an interdisciplinary team environment
  • Board certification in Internal Medicine or Family Medicine preferred
  • Geriatrics experience or certification preferred
  • Experience in managed care or value based care environments is a plus
