The person serving in the position of Physical Therapist is responsible for evaluating and treating disabilities, injuries and disease using a range of manual therapy, modalities and exercises as well as providing education and consulting services. The Physical Therapist complies with all regulatory, professional, ethical, quality improvement and facility standards, performs charting and patient progress notes as well as supervision, training and assistance to department personnel. The Physical Therapist serves in a multidisciplinary capacity providing services to a wide variety of populations which may include pediatric, medical/surgical, orthopedics, outpatient. Homecare services are also provided and may increase travel frequency.
Essential Key Job Responsibilities
1. Patient care including evaluation and treatment of patients referred by providers
2. Provides appropriate supervision for Physical Therapist Assistants and support personnel
3. Completes documentation related to patient care in a timely manner
4. Participates in and provides educational in-services for staff and providers
5. Collaborates with healthcare team and provides professional recommendations regarding patient care
6. Participates in patient related care coordination activities (e.g. care conferences, etc.)
7. Positive promotion of department and profession within hospital/clinic as well as community
8. Other duties as assigned by management

Position Purpose:
Our Physical Therapy Assistants, PTA, provide physical therapy services to Mt. Carmel patients under the guidance and supervision of a Physical Therapist and following a written plan of care established by the physician and physical therapist. Our PTAs assist in assessing our patients need for physical therapy devices and interventions and instructs our patients and their family members in the use and care of therapeutic appliances and devices, such as wheelchairs, braces, etc.

Minimum Qualifications:
- Graduation from a two-year college-level program approved by the American Physical Therapy Association.
- Minimum of six months experience under a qualified Physical Therapist.
- Home health experience preferred.
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agencys clients and their families.
- Must accept responsibility for maintaining clinical practice skills learning and adhering to agencys policies and procedures on an on-going basis.
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
Must have current Drivers license and reliable transportation to and from work site.
