Position Overview:
- Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family, operating within the plan of care as established by the hospice team and attending physician.
- Assesses the total needs of the patient/family during regularly scheduled and after hour home visits; Documents: assessment, identified problems, nursing interventions, goals, and outcomes of interventions.

Associate's degree from an accredited Physical Therapy Assistant program
Current Michigan Physical Therapist Assistant license in good standing (or eligibility)
Work experience in a hospital or acute care setting
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $23.41 to $41.83 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
