Job Summary and Qualifications
The RN CM Care Coordinator will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. The RN CM Care Coordinator will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. The RN CM Care Coordinator will identify potential barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.
ESSENTIAL FUNCTIONS:
- Performs a comprehensive assessment of psychosocial and medical needs of assigned patients
- Develops a case management plan of care to include identified clinical, psychosocial and discharge needs; coordinates plan of care; plan is documented in the medical record; plan is communicated to appropriate clinical disciplines
- Assumes a leadership role with the interdisciplinary team to manage care, through criteria driven processes, for the appropriate level of care, patient status and resource utilization
- Conducts interdisciplinary team meetings to provide a mechanism for all clinical disciplines to collaborate, plan, implement, and assess the plan of car; patient selection should be criteria based and interventions will be documented
- Evaluates admissions for medical necessity using approved criteria at defined intervals throughout the episode of care; escalates medical necessity and admission status issues through the established chain of command
- Evaluates and assess observation patients for appropriateness in observation status
- Performs utilization management reviews and communicates information to third party payors
- Acts as a liaison through effective and professional communications between and with physicians, patient / family, hospital staff, and outside agencies
- Demonstrates knowledge of regulatory requirements, facility ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives as it relates to the provision of Case Management Services
- Makes appropriate referrals to third party payer disease and case management programs for recurring patients and patients with chronic disease states
- Documents professional recommendations, care coordination interventions, and case management activities to effectively communicate to all members of the health care team
- Facilitates patient throughput with an ongoing focus on quality and efficiency
- Tracks and trends barriers to care; makes recommendations and develops action plans to improve processes and systems
- Involves patient, family/responsible/significant others in identifying and clarifying needs and expectations to develop mutual and realistic goals

Qualifications:
- Candidates are required to have a minimum of 3 years of RECENT (Within the last year) Case Manager experience in an acute care setting.
- Also open to candidates with 3 years of experience on the following units: Med/Surg, Tele, Neuro, ICU, PCU, or ED. will also consider candidates with Case manager experience in home health or insurance. For home health and insurance, they must have 3 years of acute care experience total and must have at least 1 year of acute care experience within the last 5 years.
- Associate's degree in nursing or Diploma in Nursing required

Description
We are seeking an enthusiastic Associate Synthetic Chemist to support the discovery of small molecules for next-generation 3D printing resins. You will work hands-on in the lab, executing syntheses of novel molecules, characterizing products, and exploring photochemical and photophysical structure–property relationships. This role is ideal for highly motivated early-career chemists who love synthesis and are excited to accelerate innovation through fast, iterative chemistry. You will be closely mentored by experienced synthetic chemists and will collaborate with analytical scientists and engineers developing our printing systems.
Responsibilities
- Design, execute, and troubleshoot small molecule synthetic routes, with a focus on divergent or modular approaches to rapidly generate lead candidates.
- Purify products using chromatography and crystallization techniques.
- Characterize compounds via UPLC, NMR, UV/Vis and IR spectroscopy, and mass spectrometry.
- Work with cross-functional teammates to explore photochemical and photophysical structure–property relationships that drive 3D printing performance.
- Maintain excellent electronic lab notebook documentation.
- Contribute to management of chemical databases (e.g., commercial, proprietary, and virtual compound libraries).
Minimum Qualifications

Overview & Responsibilities
- As a Senior Linux Systems Engineer, you will serve as the technical authority for a Red Hat Enterprise Linux (RHEL) environment supporting the Cameo (Dassault Systmes/No Magic) suite. This role requires more than standard administration; we are looking for a specialist with CLI-level mastery capable of deep-tier kernel tuning and performance optimization. You will ensure the integrity of server resources, manage a complex Virtual Desktop Infrastructure (VDI), and provide high-level technical support to a large community of system engineers and partners.
You will be responsible for
- Advanced Systems Engineering & Kernel Optimization
- Perform kernel-level tuning (sysctl), I/O scheduling, and memory management to optimize RHEL for high-demand MBSE workloads.
- Execute all administrative tasks, file management, and system troubleshooting via the command-line interface (CLI) in a headless or cloud environment.
- Proactively monitor system health and performance to ensure 99.9% availability of mission-critical server resources and hardware.

This is an unpaid internship through the Department of Defense (DoD) SkillBridge Program. Job Requirements: Education/Skills Graduate of an accredited program for Registered Nursing Current knowledge and skills appropriate to age/type of patient population served Knowledgeable and sensitive to patients' rights in the delivery of care Communicates in a clear concise manner appropriate to the developmental age of patient.
Experience All newly hired nurses with two or less years of experience as a Registered Nurse will be hired in at a CN I status.
A new hire can be considered for a higher clinical ladder level (level II and III only) based on years of relevant experience to the clinical area, national professional nursing certification, achievement of additional nursing education (B.S.N.
and MSN).
Licenses, Registrations, or Certifications Current New Mexico RN license or current Compact state license.
BLS Certification required or within 2 weeks of hire date.
All other required certifications must be obtained within 6 months of hire date.
If nurse has the required certifications at time of hire, they must maintain the certifications and will not have a grace period to renew.
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all Associates are required to obtain the required certifications for their respective positions within the designated time frame.

The Tele Sitter verbally redirects the patient from engaging in at-risk behaviors and calls the nursing staff if the patient requires assistance.
Responsibilities: • Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
• Uses systematic processes to facilitate prompt placement of tele sitter devices and validates criteria for use.
• Maintains visual observation at all times, whether individually or observing multiple patients at one time on a monitor.
• Monitors the patient via tele sitter and uses voice command functionality to redirect patient while contacting nursing associates if patient requires additional intervention to maintain safety and documents intervention.
• Escalates caregiver notification when a patient is at risk for harm to self or others and participates in collaborative identification and reporting of patient safety concerns.
• Communicates effectively with patients and staff while maintaining discretion, courtesy and an understanding of patient privacy.
• Activates the tele sitter alarm to notify care providers in the event a care provider cannot be reached.
• Ensures all interventions are documented within the functionality of the tele sitter software.
• Communicates with patient's family and visitors explaining the safety of the tele sitter device.
• Communicates effectively with patients and staff while maintaining discretion, courtesy and an understanding of patient privacy.
• Demonstrates understanding and proficiency in the use of the Tele sitter program/ software and basic computer operation.
• Maintains visual observation at all times, whether individually or observing multiple patients at one time on a monitor.
• Assures patient environment safety.
Seeks assistance with removal of any potential safety hazard from room and reports findings to Charge nurse or nurse caring for patient.
• Maintains vigilance in observation of patients for safety by not engaging in any distractions, including but not limited to, the use of personal cell phones, computer access, reading material or other visual material not directly involved with the care of the patient.
• Participates in hand-off of pertinent assigned patient information and status when arriving and upon completion of shift.
Job Requirements: Education/Skills • High School Diploma or equivalent required.
• Basic computer operations.
• Ability to communicate effectively in English- both in writing and verbally.
• Effective communication for interdepartmental collaborative relationship-building.
• Maintains HIPPA compliance.
Experience • One (1) year in a hospital environment preferred.
Licenses, Registrations, or Certifications • BLS (Basic Life Support) preferred.
