System Transfer Center RN, Nights
*7:00 pm - 7:00 am weekdays and a weekend rotation.
Position Purpose:
In accordance with the Mission and Guiding Behaviors the registered nurse of the system transfer center should accurately listen and respond appropriately to requests made by transferring physicians, hospitals, and peers. To ensure patients are assigned rooms in an expedient manner, this position will utilize all critical resources within the organization to expedite this process. This position should give high priority to customer service and complete customer satisfaction. This position must ensure all paperwork and computer entry is checked for accuracy and continually enhances their knowledge regarding processes. This position will coordinate the successful transfer and placement of patients entering via the Transfer Center access. This position will effectively communicate with the surgery department, emergency department, nursing units, physician offices, bed schedulers, and outlying facilities to ensure appropriate placement of patients in a timely manner. This position must be able to adjust through a rapidly changing environment. This position will participate in personal and professional growth, development, and continuous improvement in all critical success factors of people, service, quality, financial, and growth. This position will be required to work weekends, holidays, and be on call as scheduled. Requirements/Qualifications
- Education: BSN degree preferred
- Licensure / Certification: Licensed as an RN in state of Ohio
- Experience: Minimum 5 years nursing experience, 2 years critical care experience preferred.
- Effective Communication Skills
- Ability to manage situations related to family and visitors, patients of varied ages and needs, physicians and other hospital departments

Position Purpose:
The Medical Intensive Care Unit (MICU) at Mount Carmel East has 16 beds. The Registered Nurse (RN) would expect to see cases involving sepsis, covid, pneumonia, diabetes, cardiac arrest, psychological disorders, drug overdoses, surgical, stroke, and more. This unit is family oriented and enjoys getting together for gatherings outside of work.
The Registered Nurse RN, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. What you will do:

Minimum Qualifications:
Education: Graduate from school of nursing.

Position Purpose:
In accordance with the Mission and Guiding Behaviors; the RN Administrative Supervisor, in collaboration with the management team, serves as a support person in planning, organizing, directing and coordinating optimal patient care activities on the nursing units; and functions as a resource person to other hospital departments on given tour of duty within the standards, policies, procedures and guidelines of the organization.

Minimum Qualifications:

These patients will include those who require complex care coordination for greater than 24 hours. The ACU will be managing patients who require services like telemetry monitoring, non-titrating vasoactive drips and stable Bi-pap patients. The ACU will have a high focus on organization and care coordination to decrease length of stay and avoid duplication of services.

The Accountable Care Unit at Mount Carmel East is expected to open in the Spring of 2022. The ACU, located on 3 Green, will care for patients from acute to intermediate levels of care. These patients will include those who require complex care coordination for greater than 24 hours. The ACU will be managing patients who require services like telemetry monitoring, non-titrating vasoactive drips and stable Bi-pap patients. The ACU will have a high focus on organization and care coordination to decrease length of stay and avoid duplication of services.
In accordance with the Mission and Guiding Behaviors; the Staff Registered Nurse, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse collaborates in the spirit of teamwork is accountable for the delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization.
What You Will Do:
Assess and evaluate patient care for an assigned group of patients, utilizing evidence-based practice; Assists provider with patient assessment, examinations and treatments; Perform basic medical procedures under the supervision of a provider, including set up of procedures.
Administer medications under the direction of the provider, according to the organization’s standards, policies, procedures, and/or guidelines.
Assist in emergency situations under the direction of the provider
Thoroughly educate and explain procedures to patients and family members (diagnosis, disease process etc.).
Document accurately and completely pertinent assessments, interventions and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.
Assume responsibility and accountability for care provided and documented by other licensed and unlicensed care givers as appropriate.
Minimum Qualifications:
Graduate of a school of nursing
Current license to practice as a Registered Nurse in the State of Ohio
Current BLS/CPR
Demonstrated ability to plan, organize, and manage patient care, including delegation to and supervision of other members of the patient care team
Basic computer skills required; prior experience using an EMR for charting preferred

Position Purpose:
The Disease Management Coordinator provides consultation to patients, families, and nursing personnel regarding the care of a specific disease entity/process and its impact to patients, assumes a leadership role in issues related to patient outcomes, and acts as a change agent to support institutional initiatives pertaining to disease management.

Hours of office: Monday – Friday: 8:00am – 4:30pm
What You Will Do:
Care Coordination – Coordinates inpatient care for specific disease/population. Rounds with physicians (as available and feasible) to insure continuity of care including discharge planning. Assures appropriate care coordination or plan of care is implemented for individual heart failure patients.
Continuity Planning – Identifies discharge needs, coordinates planning, and problem solves continuity issues to ensure seamless continuum of patient care; collaborates with hospital based case managers to ensure effective care continuum and discharge plans are met. Assures appropriate clinical information is provided to outside agencies
Collaboration – collaborates with the multi-disciplinary team, including physicians and other healthcare disciplines, and outside agencies regarding patient stay management and care coordination issues. Initiates appropriate consults, e.g., physician and other ancillary services.
Minimum Qualifications:
Education: Graduate from school of nursing; BSN or Master’s Degree preferred
Licensure / Certification: Current license to practice as a registered nurse in the State of Ohio
Experience: Minimum of five years recent acute care clinical experience in disease specialty nursing (in an acute care setting and/or physician office setting); med/surg and critical care experience helpful.
