Job Duties Provides nursing care to Life Flight.

Follows and adheres to Act 37 standards and protocols.

Provides ongoing patient care and education for patients in the adult Hem/Onc Department Serves as a resource person for patient and family regarding the perception of the diagnosis and medical plan of care for the management of disease as designated by the physician.

Serves as a patient liaison between the patient and the care team as they encounter issues/obstacles/ changes to their plan of care.

Performs pre-visit RN triaging for new and patients to ensure that decisions can be made on the first visit and treatment planning for the Hematologic/Oncologic care can be initiated.

Assists with supportive care referrals such as referrals to Palliative care, Psychology, and social work.

Directly responsible for the completion and documentation of the initial chemotherapy teach session in EPIC as required by regulatory bodies Participates in performance improvement activities to ensure high quality care delivery and assists providers with completion.

Assists with orientation of new personnel to ensure that they are appropriately trained to support patients and provider.

Executes and documents ongoing patient communication.

Provides instruction to ensure patient's understanding of medical condition and compliance with treatment instructions.

Performs and assists with triaging telephone calls concerning health issues and offers appropriate telephone management advice to patients as liaison to physician recommendations.

Communicates lab, radiology and other testing results to patients and via telephone and electronic medical record letter notification.
