SUMMARY:
The Paramedic is a frontline healthcare professionals who will provide emergency medical care in pre-hospital settings. They respond to calls from dispatch and assess patients, administer life-saving interventions, and transport patients to medical facilities.
RESPONSIBILITIES:
Responds to emergency calls, assess patients' conditions, and determine appropriate medical interventions, coordinating with dispatch as necessary.
Safely transport patients to medical facilities, monitoring their condition and providing necessary care during transit
Provide advanced life support (ALS), including airway management, intravenous therapy, defibrillation, and drug administration.
Communicate with emergency dispatchers, firefighters, law enforcement, and healthcare professionals.
Maintain accurate and detailed patient care reports, documenting assessments, treatments, and patient responses.
Ensure the cleanliness and readiness of emergency vehicles and medical equipment.
Educate and reassure patients and their families in crisis situations.
Adhere to legal, ethical, and safety protocols.
Interpret oral, written and diagnostic form instructions, including calculating weight and volume ratios.
Lift and move patients, often in challenging environments, such as extreme terrain or temperatures.
Participate in continuous education to stay current with medical protocols and enhance skills.
Ability to maintaining composure and professionalism in high-pressure situations.
Working Conditions and Physical Requirements (Expanded):
Work in a variety of uncontrolled and potentially hazardous environments (indoor/outdoor, inclement weather, confined spaces).
Maintain physical readiness to assist in lifting/moving patients using stretchers (up to 250 lbs with partner assist), medical equipment, and gear.
Walk, kneel, crawl, and climb over uneven terrain or in emergency conditions.
Transport essential medical equipment (e.g., EKG monitor, ALS/med bags) without assistance.
Remain calm and focused in high-stress, emotionally charged situations.
REQUIRED QUALIFICATIONS:
Current NYS Licensed Paramedic.
BLS required at hire, If not AHA, must obtain AHA BLS within 3 months of hire.
ACLS required at hire, If not AHA, must obtain AHA ACLS within 3 months of hire.
PALS required at hire, If not AHA, must obtain AHA PALS within 3 months of
hire
Valid Driver's license.

- Conducts an in-person initial comprehensive physical therapy assessment on all participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts an in-person physical therapy re-assessment minimally every six (6) months or as needed.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
- Provides treatment to participants and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
- Evaluates participantβs needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
- Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.
- Assists as liaison with Senior Community Care nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.

Job Description
Statement of Purpose
The Behavioral Health Technician fulfills the assigned role as a part of the treatment team, maintaining the well-being of clients and integrity of the program. Provides client supervision and staff/unit support as assigned.
Major Tasks, Duties and Responsibilities
- Completes admission procedures on new clients including tech paperwork, room assignments, and initial orientation.
- Conducts property searches of clients as required by program standards.
- Performs urine drug screen testing of clients as required.
- Performs medication call supervision as required.
- Provides transportation services including patient transportation, donation pick-ups, agency errands, etc.
- Attending in-service training and educational workshops.
- Attending all required staff meetings.
- Knowledge of mental health, drug and alcohol abuse, family systems and various recovery resources in the community such as Smart Recovery and others.
- Develop and maintain individualized records of client progress (shift notes) and participation in treatment including admission and discharge, individual sessions, group facilitation, family, vocational (both individual and group) literacy training and other occasions as appropriate. The technician will not be doing any treatment planning or counseling services unless registered to do so. The tech will have restricted EHR access.
- Ability to write clear (and concise) statements, summarizing clientβs feelings, expressions, resistance and response to their treatment plan, goals, and objectives. Includes letters to referral agencies, courts/probation.
- Ability to confront behavioral changes, identify a crisis, and recognize when additional assistance is necessary.

Educates/instructs/recommends patient, family members and other health team members of the proper techniques, for exercise program, precautions and self care activities to assure safety for caregivers and patient.
Participates in the interdisciplinary team meeting (ITM) by exchanging team information on the progress and discharge plan in order to provide the highest level of continuity of care for the patient.

The RN Case Manager - Home Health is responsible for the planning, coordination and delivery of quality home health care and services, ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Essential Activities and TasksΒ
Care Management and Service Delivery - 70%
Provides ongoing assessment and nursing care to patient in homes and health care facilities. Ensures an accurate evaluation of patient needs, develops and implements a comprehensive care plan that addresses current needs. For home health, identifies when patients need or qualify for additional services and notifies appropriate team member or team leader.
Follows current nursing professional standards of practice and agency approved protocols for home health.
Provides quality plan and symptom management for the comfort of patients.
Communicates with other health care providers about changes in condition and patient needs.
Assesses patient and caregiver needs and provides education when necessary.
Commitment to high quality customer service for internal and external customers.
Oversees the implementation of the patientβs plan of care. Evaluates the effectiveness of the plan on an ongoing basis.
