Position Overview: The purpose of this position is to perform a variety of imaging procedures, including CT and X-Rays, at a technical level not requiring constant supervision.

Essential Job Functions:

Provide technical assistance and supportive patient care to assist the physicians, nurses and other technical and administrative staff in meeting the needs of individual patients throughout the facility.
Operate and maintain all imaging department equipment including but not limited to X-Ray, CT, CR reader, and PACS
Perform all imaging exams in accordance with established policies, procedures, regulations, and laws and ensure the physician on duty is notified of the results
Perform basic clinical procedures under the direction of the physician and/or nurse on duty
Maintain all required documentation, logs, charts, forms and records in paper and electronic formats
Collect laboratory samples and perform lab testing in accordance with established policies and procedures, as well as COLA and CLIA regulations
Maintain an adequate supply of all reagents and consumables to perform quality testing
Perform all routine daily, weekly, monthly, and periodic maintenance and function checks following established protocol for all imaging equipment
Perform Quality Control as established by this laboratory
Perform Proficiency Testing and/or Split Sample analysis periodically as established by this facility
Retain records of all analytic activities performed for a minimum of two years
Perform all Quality Assessment activities assigned to this position and document these activities for periodic review by the Laboratory Supervisor and/or the Laboratory Director
Other Job Functions:

Perform duties as ER Technician as needed
Maintain an adequate supply of all reagents and consumables to perform quality testing
Perform all routine daily, weekly, monthly, and periodic maintenance and function checks following established protocol for all imaging equipment
Travel to all facility locations as required
Attend staff meetings or other company sponsored or mandated meetings as required
Perform additional duties as assigned
Basic Qualifications:

High School diploma or GED, required
Graduation from an AMA-approved school of Radiology Technology, required
Associate's Degree in Radiology, preferred
Certified as a medical radiologic technologist as required by the state in which practicing, required
Registered Technologist with the American Registry of Radiologic Technologists (ARRT) in radiography required
Registered Technologist with the American Registry of Radiologic Technologists (ARRT) in computed tomography, preferred

CT certification required within 12 months of hire (Pennsylvania market)

Current BLS certification through the American Heart Association, required

The primary purpose of the Registered Nurse (RN) is to utilize the nursing process in planning and providing direct care to residents as assigned. The nurse will plan, direct and carry out shift routines in assigned nursing unit and observe, direct and evaluate the activities of personnel as the person in charge according to the Nurse Practice Act. Will assume Building Charge responsibilities as directed by the DON or Designee.

QUALIFICATIONS:

  • Current licensure in the state of Colorado as a Registered Nurse.
  • Current CPR Certification.
  • Maintenance of required CEU's.

ESSENTIAL FUNCTIONS:

  • Provides Leadership and Direction on Assigned Unit
    • Role models positive communication skills with NA/R's, C.N.A.'s, STNAs, TMA's (if applicable), interdisciplinary team, supervisory and management team, customers, residents and families.
    • Listen to shift report and keep open lines of communication with supervisor, co-workers and on-coming shift.
    • Make rounds to residents to observe their physical and psychosocial status.
    • Assign personnel appropriately to meet resident needs.
    • Ensure that unit personnel on your shift are carrying out their assignments.
    • Attend nurses and team meetings as scheduled and/or assigned.
    • Initiate disciplinary action when the performance of personnel warrants such action.
    • Report occupational exposures to needlesticks, blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policy and procedures governing accidents and incidents.
    • Brings concerns/improvement suggestions to facility leadership and QAA/QAPI committee.
  • Resident Care Functions
    • Maintains competence in all required skills within assigned area(s).
    • Treats and ensures that residents are treated with respect and dignity.
    • Immediately reports any suspected abuse or neglect per company policy
    • Perform nursing techniques for the comfort and well being of the resident.
    • Administers prescribed medications and treatments per MD orders.
    • Is drug knowledgeable and reports resident response, side effects, responds to drug considerations and reports problems or potential problems to the physicians.
    • Maintains all required documentation in an accurate and timely manner according to policies and procedures of the facility.
    • Follows all shift routines, policy and procedures regarding monitoring of vital signs, weights, administration of medications and treatments.
    • Make physician/provider calls to report resident change in condition.
    • Keep Resident Representative and families updated on resident conditions.
    • Comply with acute emergency protocols and tasks involved in the event of a 911 call.
    • Follows all Advance Directives: DNR/DNI/CPR/Comfort Care protocols and orders.
  • Charting, Documentation and Assessment Functions
    • Contribute toward completing and updating resident's care plans.
    • Evaluate and reports the performances of all personnel working under your supervision and direction.
    • Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
    • Accurately transcribes physician's/provider's orders as per facility protocol and forwards necessary information to other departments.
    • Document and report all incidents, accidents that occur involving residents, visitors, and staff.
    • Chart nurses' notes in an informative and descriptive manner that reflects the care provided as well as the resident response. Follows VOA Policy and Procedures.
    • Provides ongoing resident assessments as necessary and as assigned and documents these assessments per facility policy. Utilizes E-Interact COC & Care Paths for change in condition.
  • Personnel Functions
    • Attend nurses meetings as scheduled.
    • Maintain a clean, safe and efficient work environment.
    • Maintains continuing education requirements.
    • Initiate disciplinary action when the performance of personnel warrants such action.
    • Participate in employee performance evaluations, determining staffing requirements, and making recommendations to the Supervisor concerning employee counseling etc.

Essential Job Functions: Nursing Care Functions Conduct the daily nursing functions in accordance with current rules, regulations, and guidelines that govern the long-term care facility/assisted living communities.

Ensure that all nursing personnel assigned to you comply with the written policies and procedures including the safety program.

Admit, transfer, and discharge residents with documentation regarding care and service provided.

Participate in the orientation of new residents/family members to the facility.

Make rounds with health care provider as needed.

Utilize nursing personnel qualifications according to the resident's care plan, service plans and health care provider orders.

Communication/Documentation Functions Complete daily clinical and environmental rounds to evaluate resident care being provided and report significant events.

Meet and resolve issues with residents and/or family members.

Report significant issues to the supervisor.

Participate in facility surveys (inspections) as requested by management.

May be responsible for clinical programs as directed, e.g., dementia, wound care, quality assurance, infection control, admissions.

Complete an initial assessment and assist or develop interim care plan/service plans.

Update, review, and transcribe the care plan that identifies the goals, problems, approaches, and revisions based on nursing needs and new health care provider orders.

Assist with service plans as directed by the Director of Nursing.

Communicate with health care provider to review treatment plans, complete orders, progress notes, etc., in accordance with established policies.

Review the resident's medical record for specific treatments, medication orders, diets, labs etc., as for implementation and follow up.

Requisition and arrange for diagnostic and therapeutic services, as ordered by the health care provider, and in accordance with our established procedures.

Communicate the department's policies and procedures to personnel, residents, and visitors.

Communicate with written (24-hour report) and oral reports/recommendations concerning any changes in resident condition and other activities of your shift to nursing management.

Fill out and complete accident/incident reports involving residents and submit to Director of Nursing.

Complete assessments and all other medical record documentation as required to provide care services.

Provide resident and family information for care conferences or other meetings.

Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the supervisor.

Complete clinically accurate documentation according to policy.

Review the resident medical documentation and progress notes from the interdisciplinary team to determine if the care plan/service plans are being followed.

Report any concerns to your supervisor.

Inform nursing personnel of new admissions and the resident's care needs.

Ensure that rooms are ready for new admissions.

Greet newly admitted residents upon admission.

Notify the resident's attending health care practitioner and family when there is a change in the resident's condition.

Assist in arranging transportation for residents and other third-party services.

Medication Administration Functions Prepare and administer medications ordered by the health care provider.

Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents.

Report needs to the supervisor.

Review medication administration record for completeness of information, accuracy in the transcription of the health care provider's order.

Report concerns to your supervisor.

Dispose of drugs and narcotics as required, and in accordance with established procedures.

Personnel Functions Supervise your nursing unit to ensure that all personnel are performing their work assignments in accordance with acceptable nursing standards.

Work with the Director of Nursing to resolve staff performance issues including those that may lead to termination.

Review complaints and grievances, and work with the Director of Nursing for a plan of action and resolution.

Safety and Sanitation Functions Assure and make available protective equipment for staff.

Maintain nursing service work areas (i.e., nurses' stations, medicine preparation rooms, etc.), resident rooms, and treatment areas in a clean and sanitary manner.

Recommend to the Director of Nursing the equipment and supply needs of the nursing department.

Ensure that a stock level of medications, medical supplies, equipment, etc., is maintained on the premise at all times to adequately meet the needs of the residents.

Perform other job-related duties as assigned.

Knowledge and Critical Skills: Be able to make independent decisions and follow instructions.

Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public.

Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.

Communicate effectively in a manner that is sufficient for effective communication with supervisors, team members, prospects, residents, and families.

Education and Experience: Must meet all applicable state and federal requirements for this position.

Must possess a current, unencumbered, active license to practice as an RN in applicable state, in good standing throughout employment with no disciplinary action in effect against professional license.

Prefer one year of experience in nursing in a home care, assisted living, or long-term care facility.

Supervisory Responsibility: This position requires the RN to ensure resident care is provided per standards of practice following policies and procedures of the facility by CNAs/Caregivers but the CNAs/Caregivers do not directly report to them.
