Summary:
All pharmacists fill orders for drugs, monitor patient drug therapies and provide drug information. Pharmacists provide pharmacy services with compassion, excellence and efficiency. In addition, level II pharmacists participate in management of patient pharmacotherapy and quality medication management within assigned service lines.
CHRISTUS Santa Rosa Hospital - Westover Hills (CSRH-WH) is a 150-bed hospital serving the fastest growing area of San Antonio. Responsibilities:

Operational Duties and Responsibilities
- Prepares and dispenses drug orders per physician request according to established policies, procedures and protocols.
- Interprets drug orders (verbal and written) and transcribes/verifies into computerized patient medication record accurately. Maintains, accurate, complete patient drug record.
- Compounds and dispenses pharmaceuticals including non-sterile and sterile products, chemotherapy and parenteral nutrition products accurately.
- Issues controlled substances to patient care areas and maintains records as required by law and institutional policies.
- Provides services efficiently and in a timely fashion.
- Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians
- Maintains competency required for current job title/position
- Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files. Attends staff meetings.
- Completes all competency/skills assessment requirements.
- Provides analysis of medication use processes and contributes at least one related project per year.
- Ambulatory care Pharmacist
- Provides ambulatory pharmaceutical care services utilizing approved protocols and/or collaborative agreement with other providers, including managing patient drug therapy, anticoagulation management, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. Performs duties in the clinical areas assigned such as but not limited to: anticoagulation clinics, intensive medical home, transition of care, Discharge Medication reconciliation, etc.
Clinical Duties and Responsibilities
- Ensures safe, appropriate, cost effective drug therapies for patients according to established policies, procedures, and protocols.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
- Assists with pharmacokinetics consult service and renal drug dosing per hospital protocol.
- Reads, extracts and interprets information in patient medical records accurately.
- Detects and reports suspected adverse drug reactions accurately and in a timely manner.
- Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
- Provides clinical consultation and clarification to practitioners as appropriate.
- Provides accurate, adequate and timely drug information to the hospital’s professional staff.
- Provides drug education to patients and their families per institutional protocol.
- Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned.
- Conducts designated interventions as defined in department Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation)
- Baseline departmental standards for reporting interventions for clinical productivity as defined by departmental goals.
- Participates in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned
- Attends interdisciplinary rounds when assigned

- Contributes to the advancement of the pharmacy department and medication management of the organization through (minimum):

Education/Skills
- Doctor of Pharmacy or BS Pharmacy degree required.
- One of the following required: BPS Board Certification, Residency, Fellowship or at least 5 years of hospital experience.
Experience
- PGY-1 and PGY-2 residency completion, Board certification, or 5 years hospital experience required at hire.

- Pharmacy registration in the State of practice.

Clinical Intervention
* a. Maintains competency in all aspects of drug therapy for patients of all age groups.
* b. Provides pharmaceutical care by assisting other health care providers or providing direct patient care by:
* Assessing patient demographics

Drug Distribution
* a. Residents provide the equivalent of approximately 20 hours per month (excluding orientation and transition rotation) of pharmacy practice staffing during the residency program.
* b. Promotes use of WHS formulary by converting non-formulary orders to formulary when possible.
* c. Resident is scheduled to work their required shifts primarily in the Central Pharmacy or Resource Zone to gain a working knowledge of hospital pharmacy operations.
* Checks unit dose medications for accuracy
* Reviews and verifies medication orders for appropriateness and checks first doses of IV admixtures and unit dose medications prepared by technicians
* Ensures proper technique and accurate preparation of all pharmaceutical products, including oral, IV admixtures, chemotherapeutic, and investigational agents

* b. Assist in preparing for and maintaining compliance with regulatory agency (e.g. The JC) standards/requirements.

Summary:
The Pharmacy Technician III assists licensed pharmacists in the practice of pharmacy performing the duties described in the Technician II position description as well as serving as a role model for other technicians through added responsibilities, specialization in technician pharmacy practice, demonstrating leadership and initiative, and fulfilling the needs of the department.

Job Summary:
Prepare, compound, dispense medications, and perform other technical duties in the pharmacy department under direct supervision of a Pharmacist. Must demonstrate competency through supervised observation and/or any other necessary annual required competencies. Must be proficient in all responsibilities and duties of certified pharmacy technician.
Core Responsibilities and Essential Functions:
Medication Distribution
* Single Dispense - must prepare medications and/or IV for dispensing, select correct medication, correct strength, and correct form. Fill, label, and initial medication bag for a pharmacist verification.
* Bulk Dispense- Must fill multiple patient specific medications in accordance with department procedure.
* Transportation- bulk fill, first dose, and missing doses will be delivered securely to the appropriate units, while practicing appropriate procedure to ensure confidentiality for all patients.
* Missing Doses- upon request, must properly generate label, fill and have checked by a pharmacist, then dispense to the appropriate floor or unit.
* Product Preparation- must properly prepare, measure, count, medications including compound ointments, creams, oral solutions, oral solids, IV, etc.
(ADC) Automatic Dispensing Cabinet
* User- must be capable of accurately adding new users to the ADC system.
* Product- must properly load ADC with correct medications after medication is checked by a pharmacist. Monitor par levels and increase or decrease as needed.
* Reports- must be able to generate needed reports to fill, track, and identify/report all discrepancies.
* Minor Maintenance- must trouble shoot minor problems with ADC.
* Restock- must accurately fill medication, fluids and stock outs as needed.
* Recall- must assist the pharmacist and inventory specialist in retrieving recalled medications.
* Controlled Substances- Must follow all policies, procedures, and applicable laws with controlled substance medications.
IV Admixture
* Aseptic technique- Demonstrate knowledge of aseptic technique including; proper hand washing and proper use of IV Room attire. Understand Joint Commission, hospital, and departmental policies and procedures. Must complete an aseptic technique competency to show compliance with USP standards, conducted by supervisor or Lead Pharmacy Technician.
* Small volume- Demonstrate knowledge of preparing small volume admixtures
* Specialty solutions- Demonstrate knowledge of preparing epidural, intrathecal, intravenous, intramuscular, sterile syringes and chemotherapy (when applicable)
* Syringes- Demonstrate knowledge of preparing neonatal syringes (when applicable)

patient diodes daily therapist QA.Coordinate with alternate physicists to ensure full clinic coverage manage vacation/illness schedules and provide emergency consultation as needed.Ensure compliance with all applicable local state and federal regulations and accreditation standards related to medical physics.Collaborate with Radiation Oncologists Dosimetrists and Therapists to ensure safe and effective patient treatments including initial physics consultations plan reviews and specialized dose measurements.Administer maintain and oversee all physics-related software systems including treatment planning monitor unit calculation and QA software manage updates and upgrades.Perform daily monthly and annual QA testing on linear accelerators CT simulators and treatment planning systems per regulatory requirements serve as the primary contact for equipment issues or downtime.Oversee dosimetry operations including plan reviews and quality assurance for each patient with a focus on IMRT and advanced planning participate in weekly chart rounds.Perform dosimetry functions in the absence of the dosimetrist to ensure continuity of care.Be present for all HyperArc SBRT SRS SRT and other high-dose treatments as determined by the radiation oncologist.Commission and maintain HyperArc and stereotactic systems in the treatment planning system manage related QA equipment and act as the radiation safety liaison during regulatory inspections.Conduct weekly patient chart reviews and maintain accurate documentation of patient consults QA activities and treatment records.Manage physics testing credentialing and RTOG protocol participation lead planning and implementation for new treatment modalities accreditations or certifications.
