Essential Functions/Areas of Accountability
- Assist and collaborate with the Regional Director and other personnel to manage services and identify and correct issues and/or improve services.
- Ensure compliance with state regulations and policies including ensuring employees and contractors are credentialed by the state according to applicable practice acts.
- Plan, implement, and evaluate care provided.
- Participate, coordinate and manage client care conferences as needed.
- Serve as a local on-site clinical resource as needed and provide support to ensure client's home care needs are met.
- Assist and collaborate with Staffing Coordinators regarding the appropriateness of staffing and scheduling of personnel within scope of practice, competencies, client needs and complexity of home care.
- Adhere to nursing delegation guidelines as described in Agency Scope of Practice policy.
- Ensure adherence to Agency policies and procedures and making sure those are accessible to employees and contractors during hours of operation.
- Perform other functions as requested by the regional director which may include the following:
- Participate in interviewing, selection, and ongoing evaluation of clinical personnel as requested by the Regional Director
- Personnel training, education, and competency validation
- Review and evaluate clinical documentation and charts for accuracy and completeness
- Participate in all Agency performance improvement and quality improvement initiatives including but not limited to quarterly medical record review
- Collect, document, and submit data on infections, occurrences, complaints and grievances, and performance improvement activities.
- Review Nurse shift reports for adherence to policy and for opportunities for performance improvement.
- Home chart completeness.
- Timeliness of staffing cases post referral
- Equipment tracking and ensuring supplies and equipment are available and in working order
- Oversight and management of fiscal affairs Implementation of state specific regulations
- Maintaining current contracts and keeping them in compliance
- Ensuring accuracy of public information
- Assist with annual evaluations for employees
- Assist with marketing activities such as visiting with clients or physicians to discuss Agency programs as requested
- The Nurse Administrator, or similarly qualified alternate, shall be available at all times during operating hours and participate in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel.

Job Title & Role Description:
The Transitions of Care Nurse (RN) is a field-based role focused on patients experiencing an admission, discharge, or transfer (ADT) event. This nurse responds to real-time ADT alerts, engages patients during hospitalization, and coordinates seamless transitions across care settings. The role ensures safe discharges, prevents avoidable readmissions, and supports patients through the critical first 90-day post-discharge.
Key Responsibilities
- Respond to ADT alerts in real time and deploy to the hospital at admission to enroll patients into Upward Health services.
- Collaborate with hospital staff, providers, and discharge planners to create safe transition plans.
- Conduct a home visit within 2 business days of discharge to reconcile medications, confirm follow-up appointments, and assess home safety.
- Address post-discharge needs, including arranging home health, physical therapy, or durable medical equipment.
- Provide care management for up to 90 days post-discharge, with a focus on preventing readmissions and supporting patient goals.
- Educate patients and caregivers on care plans, treatment adherence, and community resources.
- Document all encounters in the EHR in real time and communicate care updates to the multidisciplinary team.
Skills Required:
- Registered nursing license (unrestricted)
- Experience in hospital-based care coordination, case management, or transitions of care.
- Strong clinical assessment and critical thinking skills.
- Ability to perform in-home visits and collaborate across hospital and community settings.
- Excellent communication and patient education skills.
- Proficiency with electronic health records and digital care coordination tools.
- Reliable transportation, valid driver’s license, and auto insurance.

Job Title: Supervising Physician
Location: Lake Mary
Position Overview: Sev Laser is seeking a qualified and experienced Supervising Physician to oversee our medical operations. The ideal candidate will be a board-certified dermatologist or plastic surgeon with a strong commitment to patient care, safety, and regulatory compliance. This role entails supervising medical staff and providing expert oversight in accordance with applicable laws and regulations.
Key Responsibilities:
- Oversee the provision of medical care within the practice and ensure compliance with state and federal regulations.
- Engage with patients for consultations and treatment as necessary, maintaining a high standard of patient experience.
Qualifications:
- Medical degree (MD or DO) with board certification in Dermatology or Plastic Surgery.
- Valid and active medical license in the state of Florida
- Prior experience in a supervisory or leadership role within a medical or clinical setting is preferred.
- Strong understanding of medical protocols, safety compliance, and patient care standards.
- Excellent communication, interpersonal, and organizational skills.
- All offices that are not the physician's primary place of practice must be within 25 miles of the physician's primary practice location or in a contiguous county, so long as the distance between any of the offices does not exceed 75 miles.
